CSCI found this care home to be providing an Good service.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides a good level of care for adults with a mental disorder. The home places people in the centre of its care planning and the manager and staff encourage people to be as independent as possible. People are assisted to make decisions and to have the most control possible over the way they live their lives. They are kept safe by good risk assessments and health and safety procedures. The home is pleasant and well decorated providing a homely environment for people to live in. A health care professional said: `They do a great job.`

What has improved since the last inspection?

The environment has improved as part of the rolling program of work. New furniture has been purchased, some of the rooms have been redecorated and new carpet has been laid in one communal area. The resident area has a new fridge and a new dishwasher has been purchased. Staff training has continued with palliative care a latest addition. Staff have recently carried out infection control training also. More staff have completed the NVQ level 2 in care training than at the last inspection. This ensures people receive care from well trained staff.

The service has a well established internal audit and quality assurance system. Improvements are identified and monitored through this. This report has produced no requirements.

An enclosed private corridor links the two properties. No.22 occupies four floors and provides single flat accommodation, all with kitchenette, sitting room, bedroom and en-suite facilities. No. 24 has five floors, the upper three providing single accommodation with adjacent toilets and bath or shower facilities. There are communal sitting and dining rooms for all people on the two lower floors. Neither property has a garden to the front but small, enclosed rear gardens are provided with suitable seating. The staff offer care and services to persons who have had or who are experiencing mental health problems. They seek to provide a holistic regime offering personal care, advice and guidance on daily living skills and activities, a catering service, a laundry service and domestic and cleaning services. The home has a statement of purpose and service user guide, which provide information about the scope and nature of the care and facilities on offer. This means the people who use this service experience good quality outcomes. The inspection for this service took twelve hours. This includes time spent gathering information, examining documentation before and after a site visit. It also includes the time taken to write the report. The site visit took place on 5th February 2009 between 9:30 and 15:30. Information for this inspection was gathered from the following: 1. A visit to the home. 2. Speaking with people living at the home. 3. Speaking with staff. 4. Speaking with Adult and Community care staff. 5. Case tracking three people on the day of the site visit. 6. Reading survey forms from people living at the home and staff. 7. Looking at information provided by the manager prior to the site visit. 8. We have reviewed our practice when making requirements, to improve national consistency. They reflect the things that people have said are important to them: People are confident that the care home can support them. This is because there is an accurate assessment of their needs that they, or people close to them, have been involved in. This tells the home all about them, what they hope for and want to achieve, and the support they need. People can decide whether the care home can meet their support and accommodation needs. This is because they, and people close to them, can visit the home and get full, clear, accurate and up to date information. If they decide to stay in the home they know about their rights and responsibilities because there is an easy to understand contract or statement of terms and conditions between the person and the care home that includes how much they will pay and what the home provides for the money. We have made this judgement using a range of evidence, including a visit to this service. Prospective residents and their representatives have their needs assessed and are given the information needed to choose whether the home can meet their needs. Evidence: The manager meets with a prospective resident of the home prior to a decision begin made about admission. The home consults with everyone who helps with the care of the person, including family when relevant and any health care professionals. If the manager assesses that care needs can be met, the person is invited for a day or overnight stay to get a feel for the home. This also helps others living at the home to meet the person and gives staff the opportunity to assess whether admission would be compatible with continuing to meet the needs of the others at the home. The assessment is signed by the person coming to live at the home to show agreement. This includes risk assessments and management plans to ensure people can be cared for safely. They reflect the things that people have said are important to them: People’s needs and goals are met. The home has a plan of care that the person, or someone close to them, has been involved in making. People are able to make decisions about their life, including their finances, with support if they need it. This is because the staff promote their rights and choices. People are supported to take risks to enable them to stay independent. This is because the staff have appropriate information on which to base decisions. People are asked about, and are involved in, all aspects of life in the home. This is because the manager and staff offer them opportunities to participate in the day to day running of the home and enable them to influence key decisions. People are confident that the home handles information about them appropriately. This is because the home has clear policies and procedures that staff follow. We have made this judgement using a range of evidence, including a visit to this service. The health and personal care which a resident receives, is based on their individual needs. Care is appropriate for each individual. Evidence: A care plan is drawn up from each assessment. The information in the care plan is shared with each person who signs the plan to show agreement. Care plans cover all areas of need, and include comprehensive risk assessments where necessary, which are tailored to each individual. Needs are identified and expected outcomes and goals are included with agreed interventions in risk assessments.The care plans emphasise capacity and encourage independence. Each member of staff has received mental capacity training to ensure they understand the implications of the mental capacity act and its requirements when planning care for people. Care plans are reviewed regularly with the involvement of the person, staff and specialist health care workers where necessary. This ensures that the care provided is responsive to changing needs. The manager said the home had very good support from mental health professionals at the Ellis centre, One health professional wrote: The staff at Rockfield do a good job. They are quick to ask advice and in my experience they always act on it to the benefit of those in their care. Another wrote: Communication is always excellent. The service is committed to providing holistic interventions for clients, identified in care plans. They reflect the things that people have said are important to them: Each person is treated as an individual and the care home is responsive to his or her race, culture, religion, age, disability, gender and sexual orientation. They can take part in activities that are appropriate to their age and culture and are part of their local community. The care home supports people to follow personal interests and activities. People are able to keep in touch with family, friends and representatives and the home supports them to have appropriate personal, family and sexual relationships. People are as independent as they can be, lead their chosen lifestyle and have the opportunity to make the most of their abilities. Their dignity and rights are respected in their daily life. People have healthy, well-presented meals and snacks, at a time and place to suit them. People have opportunities to develop their social, emotional, communication and independent living skills. This is because the staff support their personal development. People choose and participate in suitable leisure activities. We have made this judgement using a range of evidence, including a visit to this service. People are enabled to choose their life style, social activity and keep in contact with family and friends. Social, cultural and recreational activities meet peoples expectations. Residents receive a healthy, varied diet, which they enjoy. Evidence: People are encouraged to live as independent a life as possible. They are enabled to maintain contact with family and friends, evidence for this being written into daily notes for each person and an activities file. If a person chooses to attend day centre, they are assisted to do so. They participate in crafts, baking, woodwork swimming, knitting and crochet according to their interests. People are supported to maintain personal relationships, and risk assessments are written into care plans where necessary. People also go out with staff or each other to local cafes and to the shops in town. One person helps out in a local charity shop and holidays are planned at the Care Homes for Adults (18-65 years) Page 13 of 26 Evidence: regular house meetings. Last year a number of people went to Butlins at Skegness. This ensures people have the opportunity to become involved in fulfilling activities with the people of their choice. One person said: I can go out when I want to. I have enought to do. Meals are planned after consultation with people living at the home. Healthy eating is promoted and salads are always on offer at each meal. People said they enjoyed the food and that they could choose meals they liked. Any special dietary needs are recorded on file, to ensure people receive the correct diet for their health needs. One person said: The food is fine. Care Homes for Adults (18-65 years) Page 14 of 26 Personal and healthcare support These are the outcomes that people staying in care homes should experience. They reflect the things that people have said are important to them: People receive personal support from staff in the way they prefer and want. Their physical and emotional health needs are met because the home has procedures in place that staff follow. If people take medicine, they manage it themselves if they can. If they cannot manage their medicine, the care home supports them with it in a safe way. If people are approaching the end of their life, the care home will respect their choices and help them to feel comfortable and secure. They, and people close to them, are reassured that their death will be handled with sensitivity, dignity and respect, and take account of their spiritual and cultural wishes. We have made this judgement using a range of evidence, including a visit to this service. People receive care appropriate to individual needs. Their wellbeing is safeguarded and they make decisions about the way assistance is offered. Evidence: People said the staff helped them to live their lives the way they preferred. People can get up and go to bed when it suits them and get involved in whatever daily activities they choose. People are assisted to express their individuality through the way they dress and present themselves, and are encouraged if they become demotivated due to mental health fluctuations. One person said: They are very kind and help me when I feel down. Any specialist health care interventions are quickly sought and any changes in health needs are responded to and recorded on care plans and risk assessments. One health care professional wrote that the service provided: A stable environment which sets boundaries. Safety and security is provided by continuity and a caring team. All access to GPs and other health care professionals is recorded on file along with notes to monitor changes. The home has recently dealt with a situation regarding a persons health care which required them to inform CSCI. They dealt with this appropriately and ensured the person received the correct medical care when they Care Homes for Adults (18-65 years) Page 15 of 26 Evidence: discovered a problem. The mental health professional in this case commented upon how well the staff team had managed to protect the person from further medical complications. People have had risk assessments regarding medication and those who have the capacity to handle their own medication are assisted to do so. Medication is well kept and handled according to policy to ensure people are kept safe. They reflect the things that people have said are important to them: If people have concerns with their care, they or people close to them, know how to complain. Their concern is looked into and action taken to put things right. The care home safeguards people from abuse, neglect and self-harm and takes action to follow up any allegations. We have made this judgement using a range of evidence, including a visit to this service. Residents have access to an effective complaints procedure; their complaints are listened to and acted on. People are protected from abuse and have their legal rights protected. Evidence: The home has a robust complaints policy and procedure. People said the manager and staff were approachable and would listen to any complaints. One person said: They are very good at listening. They all bother about you and they would look into any problems if I had any. The home has regular residents meetings and the minutes of these are recorded. Any complaints can be brought to these meetings. This open approach allows people to feel confident about saying if there is a problem and expecting it to be put right. The home has an abuse policy and procedure. Staff have received abuse awareness training and have also undertaken conflict management , alerter training, break away techniques training and other courses relevant to the safety of this group. One member of staff said: I feel confident to deal with any behaviour at the home and that the manager will always support us and help if necessary. They reflect the things that people have said are important to them: People stay in a safe and well-maintained home that is homely, clean, comfortable, pleasant and hygienic. People stay in a home that has enough space and facilities for them to lead the life they choose and to meet their needs. The home makes sure they have the right specialist equipment that encourages and promotes their independence. Their room feels like their own, it is comfortable and they feel safe when they use it. People have enough privacy when using toilets and bathrooms. We have made this judgement using a range of evidence, including a visit to this service. People live in a safe, well-maintained and comfortable environment, which they like. Evidence: The home is clean, warm, hygienic and well maintained. It is a safe and secure for people to live in. Peoples rooms are furnished and decorated to their own taste. They have access to all communal areas of the home. One person said.My room is great I can have it as I like. The home complies with fire regulations and the environmental health department. The laundry is suitable for the needs of those living at the home. The washing machines are domestic rather than industrial in design so that people can do their own laundry. This ensures people are enabled to live as independenty as possible. They reflect the things that people have said are important to them: People have safe and appropriate support as there are enough competent, qualified staff on duty at all times. They have confidence in the staff at the home because checks have been done to make sure that they are suitable. People’s needs are met and they are supported because staff get the right training, supervision and support they need from their managers. People are supported by an effective staff team who understand and do what is expected of them. We have made this judgement using a range of evidence, including a visit to this service. Staff in the home are trained, skilled and in sufficient numbers to fulfil the aims of the home and meet the changing needs of residents. Evidence: Staff have received suitable training in induction following skills for care guidelines, foundation and other training specific to caring for people with a mental health difficulty. Some have completed NVQ training and each has an individual training plan. Staff commented on how well they were encouraged to take part in training and that the manager was enthusiastic about continual learning. Staff have also received training in palliative care to ensure that a person at the home received appropriate care last year, supported by health staff. Staff are well recruited and people living at the home are involved in the selection process. Staff and people at the home all said there were sufficient staff on duty to carry out their role without having to rush, allowing for time to spend chatting with people and assisting them with their hobbies and activities. There is low staff turnover and all said how much they enjoyed working at the home. This ensures that people receive continuity of care and the help they need to live the life they choose. One member of staff said: I really love working here. Another said: Its a lovely place, freindly. Its a good working team. Another wrote. We have a strong and experienced Care Homes for Adults (18-65 years) Page 20 of 26 Evidence: staff team. When interviewed staff had obvious regard for one another and faith in each others abilities. This ensures people receive a well coordinated and capable service. They reflect the things that people have said are important to them: People have confidence in the care home because it is run and managed appropriately. People’s opinions are central to how the home develops and reviews their practice, as the home has appropriate ways of making sure they continue to get things right. The environment is safe for people and staff because health and safety practices are carried out. People get the right support from the care home because the manager runs it appropriately, with an open approach that makes them feel valued and respected. They are safeguarded because the home follows clear financial and accounting procedures, keeps records appropriately and makes sure staff understand the way things should be done. We have made this judgement using a range of evidence, including a visit to this service. People benefit from an open style of management based on respect and from the experience of the manager. Peoples views inform practice. Welfare is protected by robust health and safety systems. Evidence: The manager has NVQ at level 4 and the Registered Managers Award. She is well qualified to run the home. A deputy manager has been recruited in the last year which has helped the manager to fulfil her role effectively. The quality of information provided in the AQAA was high and it helped to show that the home is good at monitoring its own performance, identifying and planning improvements. Staff were very positive in their comments about the managers ability. One wrote: We have a strong and experienced staff team, we have excellent support from management. and My manager has an open door approach so she is always available if I need help or advice. Another wrote. I feel we are very well supported. and I think our manager is doing a very good job. The people living at the home clearly knew the manager well Care Homes for Adults (18-65 years) Page 22 of 26 Evidence: and responded very positively to her on a tour of the premises. It was clear that the relationship between people living at the home, the manager and staff was both warm and professional. Several people manage their own finances, those who have support with money have all transactions recorded and signed by each person. Money held was examined against records and was up to date and accurate. The home has a good quality assurance system, with questionnaires being sent out to residents and others. A business plan is drawn up annually based on the findings of the surveys and the homes own internal quality audits. This ensures the service is based on the views of people who live there. A number of health and safety documents were examined. All seen were up to date and in order. This along with a robust approach to staff training, and the updating of policies and procedures relating to health and safety ensure that people are kept safe.
