Conditions of registration: 1. 1 resident shall be aged 55 or over Date of last inspection 8th January 2006 Brief Description of the Service: The fees for 2006/07 are £1382 per month-£1695 per month. Rose Lodge Care Home is located close to West Bridgeford town centre and the main local amenities. The home offers care for up to fifteen older people in single bedrooms. All rooms have a call alarm. Communal space includes two lounges and two dining areas. There is a stair lift to the first floor and the home is well maintained throughout in terms of decoration and furnishings. This is an overview of what the inspector found during the inspection. The focus of the inspections undertaken by the Commission for Social Care Inspection is upon outcomes for service users and their views of the service provided. The primary method of Inspection used was ‘case tracking’ which involves selecting clients and tracking the care they received through looking at their records, talking with them where possible, and looking at their accommodation. The inspection was unannounced and took place over 6 hours one Thursday in July 2006, and was conducted by one inspector as part of the annual inspection process. A tour of the building took place and a selection of residents’ bedrooms was inspected. Residents’ records were inspected and relatives, residents and staff on duty were spoken with. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 The quality in this outcome area is good. Residents are assured that their care needs can be met before moving into the home. EVIDENCE: Intermediate care is not provided in this service. Three care plans were viewed as part of this inspection, they all contained pre admission assessments either carried out by the manager or by social services. The assessment covered all aspects of daily live and looked at the residents’ personal safety and if there were any risks associated. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 The quality in this outcome area is good. Residents’ health, personal and social care are set out in an individual plan. Residents’ health needs are met and medication is administered according to policy and procedure. Residents feel they are treated with respect and their right to privacy is upheld. EVIDENCE: Residents care plans are generated from the assessment, it details what care should be provided to meet their health, personal and social care needs. Plans look at minimising the risk of falls in line with relevant clinical guidelines. Plans are reviewed regularly and frequency changes if the residents needs require it ensuring the plan always reflects their needs. Not all residents could remember seeing their care plans, one resident said ‘I think I have seen it but I’m not sure when’. Some risk assessments simply stated there was a risk but not why there was a risk. It recommended that where risk assessments are carried out that the registered person ensures that more detail is available regarding why there is a risk. Residents’ health care needs are well documented showing where district nurses are involved as well as other health care professionals. Residents are weighed initially on entering the home but

Six requirements were set at the last inspection regarding medication administration; these have all been met satisfactorily. The community pharmacist is happy with the standard of administration within the home. Residents spoken with were positive about the care their received from staff and said that they felt that they were respected and staff knocked on their bedroom doors before entering to maintain privacy. Residents said that they wore their own clothes and that they could make phone calls in private if they wanted to. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The quality in this outcome area is good. Residents find the lifestyle experienced in the home matches their expectations and preferences. Residents are supported to maintain contact with family and friends and the community as they wish. Residents exercise control over their lives and receive wholesome and balanced diet in surroundings and times that are convenient to them. EVIDENCE: Residents spoken with were happy with the amount of activities and said that they could choose how they spent their day, including what time they got up and what time they went to bed. Care plans indicated social activities that residents enjoyed and there was evidence that residents were given opportunities to participate in a variety of things. Residents said that they were able to receive visitors in private and a visitor spoken with said that they were made to feel welcome and given refreshments. One resident spoken with said that if they wanted to go for a walk outside the home they were supported to do that as well. The meal was observed and it was clearly homemade and appeared appetising, residents spoken with said that food was usually good and that they were asked what they wanted. Evidence was seen that fresh fruit and vegetables were ordered regularly and that a bowl of fruit was available for residents to

The dining room is light, airy and clean. Meals are taken in an unhurried manner with staff available to provide support if they are needed. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 The quality in this outcome area is good. Residents and their relatives are confident that their complaints will be listened to, taken seriously and acted upon. Residents are protected from abuse. EVIDENCE: The Commission has not received any complaints regarding this service since the last inspection and has no concerns with the service. Residents spoken with said that if they had a problem they would speak to a member of staff or the manager and all felt that their concerns would be dealt with. A relative spoken with said that they felt sure that any concerns would be dealt with promptly. Since the last inspection most staff have undergone adult abuse awareness training, staff spoken with had a good understanding of what abuse and what to do about it. Residents spoken with said that they feel safe and well supported by staff. The home does not hold any money for residents. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 The quality in this outcome area is good. Residents live in a safe, wellmaintained environment that is clean, pleasant and hygienic. EVIDENCE: A tour of the building took place and it was clean well maintained, where requirements had been met regarding the carpet these are now considered met. Residents’ bedrooms were clean and well maintained. Residents spoken with said that they were happy with their rooms. The grounds are kept tidy, attractive and are safe and accessible. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 The quality in this outcome area is adequate. Residents’ needs are met by the numbers and skills mix of staff and staff are trained and competent to do their jobs. Poor recruitment practices mean that residents are not supported and protected by the home. EVIDENCE: Staff spoken with that usually they had enough staff on duty to meet the needs of residents. Staff spoken with said that everyone works as a team and there is generally a low turnover of staff thereby maintaining the continuity of care. A rota was available that showed who was on duty in what capacity. Domestic staff are available in sufficient numbers to ensure that standards relating to hygiene and food are met. Staff spoken with said that they were able to access training, including NVQ if they wanted to. Staff files were viewed and found to lack references and Criminal Records Bureau checks. The Registered Person must ensure all appropriate recruitment checks are carried out on all staff who work in the care home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38 The quality in this outcome area is good. Residents’ benefit from a well run home. Their financial interests are safeguarded and their health, safety and welfare are promoted and protected. EVIDENCE: The manager has now applied for registration with the Commission and it is currently being processed. Thos has been an outstanding requirement and is now met. Staff spoken with felt that the home was well managed and that a clear guide to standards expected was given. Although there is a quality questionnaire the manager reported that they have some difficulty in getting relatives and other stakeholders to reply to copies sent to them. This means the Registered Person will have difficulty in including those views in any annual development plan. Residents money is not kept in the home and family bring in what is required when requested.

Evidence was seen that appropriate health and safety checks are made including fire checks and water temperature tests. The requirement is met. Security in the building is in line with needs of the residents, maintaining their safety at all times.
