This inspection was carried out on 29th June 2005.

The inspector found no outstanding requirements from the previous inspection report, but made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

All service users spoke positively about the support that they received and said, "I am happy with the staff","They are very friendly and helpful" and "I feel safe". Service users were encouraged and supported to live as independently as possible. Daily routines were flexible and service users were encouraged to spend their day as they wished. All service users spoke positively about the many social groups, day centres and activities that they attended. The home was warm, well maintained and was furnished in a homely manner. All areas throughout the home were very clean and it was evident that the manager and staff team were committed to encouraging service users to maintain a clean and hygienic environment. Service users were encouraged and supported to maintain positive relationships with their families and friends. Service users confirmed that their relatives were welcome to visit them and "I visit my family at the weekend". Through discussions and observations it was evident that service users enjoyed a shared living environment. Service users said that they enjoyed living together and stated "We help each other". The manager had good links with local colleges and training agencies, which enabled her to keep up to date with training courses that were available for the staff team. Refresher training was offered to staff on a regular basis, which enabled them to develop their skills and keep up to date with changing legislation. The registered manager/owner had many years experience within the caring profession which, enabled her to contribute to the care of service users and communicate a clear sense of leadership to staff. All service users spoke positively about the manager/owner and it was evident that they were confident in the way in which she managed the home and the support and advice that she offered.

What has improved since the last inspection?

Mrs Janet Barlow and the staff team continue to provide a good service. One previous requirement to provide locks to bedroom doors had not been met.

The registration includes 3 beds at 2 Rosebery Terrace and 3 beds at 4 Rosebery Terrace. Date of last inspection 9 March 2005 Brief Description of the Service: Rosebery House comprises of two domestic properties at 2 and 4 Rosebery Terrace. The home is one of several owned by Mrs Janet Barlwow. The houses are in a cul-de-sac within walking distance of Barnsley town centre. Rosebery House is registered for a maximum of six persons with mental health needs. Each property can accommodate three persons and each has three single bedrooms. This was an unannounced inspection carried out from 12 p.m to 3 p.m. Three service users and the registered manager/owner were spoken to. A sample of records was examined and an inspection of the property was carried out. Throughout the inspection positive and professional relationships were observed between the manager and service users. The inspector wishes to thank the manager and service users for their time and welcoming the inspector into their home. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2 and 3. Prospective service users and their relatives were provided with the information that they needed. Prospective service users were given the opportunity to visit the home for trial visits before making a decision to live there. EVIDENCE: A Statement of Purpose and Service Users Guide were available, these provided service users and their relatives with the information that they needed to make an informed choice about living at the home. There was an admission policy to ensure that prospective residents were able to visit the home before making a decision to move there. Introductory visits were offered which included day visits and overnight stays. A record of ‘settling in’ periods was maintained in service user files, which detailed service users progress and their compatibility with other service users living at the home. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7 and 9. Service users individual needs were assessed and their changing needs were reflected in their plan of care. Service users were encouraged and supported to live as independently as possible and to make informed decisions about their daily lives. EVIDENCE: The Care plans checked set out in excellent detail the action that was required by staff to ensure that all aspects of service users personal, social support and healthcare needs were met. The Care plans had been completed with the involvement of the service user, which gave them the opportunity to agree with staff the help that they needed to live as independently as possible. Through discussions and observations it was evident that service users were encouraged and supported to make decisions relating to their own lives. Service users confirmed that they were able to spend their day as they wished. Several service users had chosen to visit the town centre whilst others were attending a local day centre. One service user confirmed that she received her allowance and that “I can spend it as I wish”. Service users files contained detailed risk assessments relating to all aspects of service users lives both inside and outside the home. They clearly identified the individual risks that were presented to service users on a daily basis and the action required to reduce the risk, which enabled service users to live an independent lifestyle. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15,16 and 17. Service users were encouraged to maintain and develop social and independent living skills. Opportunities were provided for service users to engage in activities within the home and maintain links within the local community. Service users were encouraged to eat a healthy and varied diet. EVIDENCE: Service users were encouraged to live their lives as independently as possible. All service users participated in the day-to-day organisation of the home. Service users said, “we all have our own jobs” and “we do our own washing and ironing”. All areas within the home were very clean and tidy and it was evident that this arrangement worked very well. Service users were encouraged to lead a fulfilling lifestyle. All service users spoke positively about the many social groups, day centres and activities that they attended. One service user spoke highly of a local day centre that they attended and confirmed, “I attend yoga and computer classes”. A garden party was planned at the day centre and service users and staff were planning to attend this event. Service users confirmed that “I have been into town this morning” and “I will be going out tomorrow”. One service user spoke enthusiastically about a planned shopping trip, “The staff are taking me shopping to choose a new skirt”. Service users were encouraged and supported to maintain positive relationships with their families and friends. Service users confirmed that their relatives were invited to visit them and “I visit my family at the weekend”. Through discussions and observations it was evident that service users enjoyed a shared living environment. Service users said that they enjoyed living together and said “We help each other”. Service users were offered and encouraged to eat a healthy diet. Service users confirmed, “the staff help” with planning and preparing their meals. Privacy locks were fitted to bathroom doors but there were no locks to bedroom doors. The manager/owner confirmed that she was in the process of providing locks for service users who had chosen to have one fitted. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19 and 20. Service users received personal support, which promoted their privacy, dignity and independence. Service users physical and emotional needs were met. EVIDENCE: The manager had an excellent awareness of service users individual physical and emotional needs and spoke positively about the progress that service users had achieved. Service users received good support from healthcare professionals who visited them. There were records to evidence that service users were receiving regular visits from their general practitioner, dentist and other healthcare professionals. Service users confirmed that their healthcare needs were met and said, “the staff help me make appointments”, “the staff are very helpful” and “my doctor is very good”. There was a medication policy and procedure to ensure that staff adhered to safe practices. Staff had received medication training; all promoting that medication was appropriately administered to residents. Residents had been consulted about staff assisting them with medication, risk assessments had been carried out to identify if service users could administer their own medication. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23. The homes complaints procedure was clear and accessible. Service users were confident that their views would be listened to. There was an adult protection procedure and all staff had received adult protection training. EVIDENCE: The complaints procedure ensured that service users and their relatives were aware of how to make a complaint and who would deal with them. Service users said that they had “no grumbles” and that the “staff are very helpful”. There was an adult protection policy and procedure that promoted the protection of service users from harm or abuse. All staff had received adult protection training, which enabled them to identify and report any allegations or incidents of abuse to residents. Service users spoke positively about the staff and the support that they offered, “I feel safe”. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25 and 30. The home was clean, comfortable and very well maintained. Service users were provided with an environment that met their individual needs and lifestyles. EVIDENCE: The home was warm, well maintained and was furnished in a homely manner. Service users said that they liked the home “I am happy living here” and “you would always be made to feel welcome”. All the bedrooms were clean and brightly decorated. Service users had been encouraged to personalise their bedroom with personal mementoes. One service user confirmed, “ I have just had a new carpet that I chose myself”. All areas throughout the home were very clean and it was evident that the manager and staff team were committed to encouraging service users to maintain a clean and hygienic environment. There was a pleasant enclosed garden, which was well maintained. Service users said that it was “nice to sit in the garden”, when the weather was warm. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,33,34,35 and 36. A caring and committed staff team supported service users. All staff received training and support appropriate to their role. The home operated a thorough recruitment policy that promoted the protection of service users. EVIDENCE: All service users spoke positively about the support that they received and said, “I am happy with the staff” and “They are very friendly and helpful”. All staff had received training that included adult protection, medication, food hygiene and first aid. The manager had good links with local colleges and training agencies, which enabled her to keep up to date with training courses that were available for the staff team. Refresher training was offered to staff on a regular basis, which enabled them to develop their skills and keep up to date with changing legislation. A thorough recruitment policy and procedure was in place that promoted the protection of service users. Two staff files checked contained a range of required information including two references, declaration of health and qualifications/training. All staff employed had undertaken a Criminal Records Bureau Check at the enhanced level. One to one support and supervision was offered to new staff until they had a good knowledge of service users individual needs and were confident to work unsupervised. JUDGEMENT – we looked at outcomes for standard(s) 37,39,41 and 42. Service users and staff benefited from the leadership, experience and management style of the homeowner. Staff received excellent support and was appropriately supervised. Policies and procedures were in place that promoted and protected the health, safety and welfare of service users and staff. EVIDENCE: The registered manager/owner had many years experience within the caring profession which, enabled her to contribute to the care of service users and communicate a clear sense of leadership to staff. The manager had recently completed the NVQ assessor’s award to support the staff in achieving a NVQ qualification. The manager had an excellent knowledge of service users individual needs. There was a relaxed and friendly atmosphere within the home and positive relationships were observed between the manager and service users. The manager visited the home on a regular basis to support the staff and to keep up to date with service users progress and to support the staff team. She was contactable at all times and one service user said, “I can always ring”. Polices and procedures were in place that covered all topics including confidentiality, abuse, staff receiving gifts and complaints. The manager/owner had reviewed the homes policies on a regular basis to ensure that they complied with current legislation. The records sampled were very well organised, up to date and securely stored in accordance with the data protection act. All documents checked were thorough and information was easy to track. The staff had received regular training to promote the health, safety and welfare of service users and their colleagues.
