The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides a safe, comfortable, clean and homely environment for residents living here. The staff group is stable and residents said they liked the staff; one resident said `They`re very good to me and very patient`. The Manager of the home was a previous owner and is enthusiastic and knowledgeable about all the residents. She devotes much of her time and energies to ensuring that they are well cared for.

What has improved since the last inspection?

The kitchen has been completely re-fitted and two bedrooms have been redecorated. Following storm damage the first floor landing has been repainted and the ceiling renewed. The entrance hall and corridors, the pantry and the dining room have been redecorated and there is a new carpet in the dining room. Care plans have been reviewed to show involvement with service users and quality assurance monitoring has improved.

What the care home could do better:

Now that the home is registered to accommodate people with a dementia some work needs to be carried out to make the garden safe and secure.

Date of last inspection Brief Description of the Service: Saxilby House cares for older people in a family environment in a detached property situated in the village of Saxilby. The home is approximately six miles from the historic city of Lincoln. Facilities include a post office, pubs, shops, a library, chemist and a village hall. The home is a two-storey converted farmhouse standing in its own grounds and gardens with car parking facilities to the rear. There is a stair lift to the bedrooms on the first floor. There are adaptations and aids around the building to allow residents to move around the home more independently. All of the bedrooms are single. There are five communal toilets and two communal bathrooms. This key inspection was unannounced. It was carried out over three hours and took into account any previous information held by CSCI. This included the home’s previous inspection reports, its service history and residents’ questionnaires sent to the home by the Commission prior to the inspection. The site inspection consisted of case tracking a sample of residents’ records and assessing their care. Some policies and procedures were examined and some records concerning the safety of the home were also seen. Three residents, two care and ancillary staff and one visitor were spoken with. The Manager was present throughout the inspection. At the time of the inspection the home confirmed that the weekly fees ranged from £335 - £415, depending on the assessed needs of the resident. Information about these costs as well as the day-to-day operation of the home, including a copy of the last inspection report, is available in the reception area or the manager’s office. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home clearly sets out what it intends to provide for residents and this information is freely available to residents and their visitors. EVIDENCE: The statement of purpose and the service user guide were clear and up to date. The Manager said that it was retained in the office for prospective residents or their relatives to read. A copy was also given to residents or their families on admission. Clear initial assessments were carried out. The contract was clear and was signed 28 days after admission. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this outcome area is good. This judgement has been made using available evidence, including a visit to the service. The home’s records give a clear indication of the needs of residents and enable staff to meet their needs with sensitivity and regard for their privacy and dignity. EVIDENCE: The two care plans looked at in depth contained clear assessments. They were reviewed regularly and were signed where possible by the service user or relatives/advocates. Risk assessments clearly addressed any safety issues for residents. Feedback comments from a district Nurse read ‘medical needs are managed appropriately’. Residents spoken with, made positive comments about the care at the home. One said her daughter helped her with the signing of her care plan. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. A variety of activities is available and the residents are helped to make choices and live their preferred lifestyle. EVIDENCE: Observations indicated that residents have a choice of activities in the home. An activity plan was displayed in the entrance hall. Two carers assisted with activities for two hours each afternoon. Residents spoken with described activities and events they had taken part in and had enjoyed. These included church services, bingo, exercises, flower arranging and entertainers. There was an activities box full of various crafts and a resident described how some residents were making crackers and paper chains for Christmas. One resident said that she never ate fish and an alternative meal was always provided for her. The complaints procedure is clear and gives residents and their families the confidence that concerns will be listened to. Clear adult protection procedures ensure that residents are safeguarded. EVIDENCE: Residents and visitors to the home all said they had nothing they wished to complain about but would speak to the owners if they needed to complain formally. There had been one complaint in the last twelve months, which had been successfully addressed. There was a clear adult protection procedure, which was linked to the Local Authority procedures. Staff members spoken with understood this procedure and had received training in safeguarding adults. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The residents live in a comfortable and pleasant environment with both private and communal space, which is on the whole suitable for their needs; however the garden is not suitable for residents with a dementia. EVIDENCE: The standard of decoration internally was generally good and afforded residents a great degree of comfort. A visitor spoken with complemented the home on its cleanliness and pleasant smell. The home has a rolling maintenance programme and recent refurbishment had included the redecoration of two bedrooms as they were vacated. Storm damage to the roof had necessitated the redecoration of the entrance hall, corridors and Saxilby House Residential Care Home DS0000059111.V317089.R01.S.doc Version 5.2 Page 15 pantry. The dining room had also been redecorated and a new carpet had been laid. The kitchen had been completely upgraded. The garden was not secure and led onto the road. In view of the recent change of registration to include residents with dementia, the garden should be made secure. There were two sets of steps up to the lawned area; these should be made safe with rails. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff numbers are in sufficient quantity for them to be able to care for the residents. Staff members are suitably trained, qualified and competent. EVIDENCE: The staffing rota showed that there were sufficient staff numbers on duty to care for the residents. Both residents and staff confirmed this. Training records showed that three carers had achieved the National Vocational Qualification at Level 2 and two currently undertaking this. Records showed that statutory training was ongoing and staff members had recently undertaken a basic course on dementia. The Manager explained that all staff completed a performance questionnaire and staff members spoken with confirmed this. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The health, safety and welfare of residents are promoted in this home. EVIDENCE: The registered Manager previously owned the home. She is completing the Registered Manager’s Award next year. The present owner visits the home on a regular basis. Staff and residents all commented that the Manager was approachable. A visitor said The staff do a very good job’. Policies and procedures seen were up to date. Records showed that regular quality assurance reviews took place and residents had meetings every 3-4 months. Questionnaires were sent to service users and their families for them to complete together.
