Conditions of registration: NONE Date of last inspection 23/02/05 Brief Description of the Service: Southfields home is registered to care for four adults with Learning Disabilities, one of them over 65 years old.The home, in a converted private bungalow set in a large garden, is situated in the Trimley villages next to the coastal town of Felixstowe. All four bedrooms are single.There are shops and other facilities in the villages as well as access by bus and train to both Felixstowe and Ipswich. This is an overview of what the inspector found during the inspection. This inspection was an announced inspection and was the first of the current inspection year. The manager was in attendance. Two parents came to talk to the inspector. They were very appreciative of the care given by the home, but were concerned about the state of the garden. This is covered under the Environment section. The oldest resident was at home all day, and 2 others arrived from day services in the afternoon. All 4 had lived in the home since it opened in 1995. Comment cards were returned by 3 relatives. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4. Prospective residents can expect to be given information and opportunities to assure themselves that the home will be right for them. EVIDENCE: All residents had lived in the home since it opened in 1995. One service user had lived there previously when it was his family’s home. One resident was due to transfer to a care home providing nursing care a few days after the inspection. There had been several reviews of this resident’s needs over 12 months with the resident, the family and the funder. The home had arranged for staff to accompany and stay with this person to make sure they were settled. The home would be using Mencap admission procedures to fill the vacancy. These procedures and the information available to prospective residents met the standards required. The home was well equipped with specialist equipment and furniture to meet the needs of the residents. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9. Residents are supported to lead as independent a life as possible, with regular reviews of their care needs ensuring that appropriate plans for their development are agreed with them. EVIDENCE: The home conducted regular reviews of residents’ care needs. The parents of one resident described the most recent review meeting with their relative. They were pleased that the resident was able to take part and was showing interest in the review. Some residents had limited ability to make decisions but were supported to do so as far as they were able. Risk assessments had been completed for all aspects of hazards inside and outside the home, and for all activities. 2 service users visited local day centres each day during the week. 1 visited twice a week with day care provided in the home by a designated worker. One resident had been unhappy with their day service arrangements, but the opening of a new facility at the centre had changed their mind. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,15,16. Residents can expect to be supported in accessing any activities they wish. EVIDENCE: Some parents described how pleased they were that their relative had attended their wedding anniversary party recently. One resident was worried that they were living too far from their relative for easy visiting. This worry had manifested itself in behavioural changes. Appropriate health professionals had been involved to provide advice on how to manage and reduce these episodes. There had been lengthy discussions with the funder about one resident receiving direct payments, but there was still no outcome. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20. Residents are assured that their health needs are monitored and action taken to meet them. EVIDENCE: The manager discussed in detail the management of the health care of one resident. It was clear from this, and from the care plan, that staff were able to closely monitor the behaviour, bowel movements, mood and mobility of this person to maintain their health, and to spot changes which required medical intervention, before it became an emergency hospital admission. Although this person had been admitted to hospital several times last year, no further admission had been necessary since January 2005. Medication training had been given to all staff, once in-house by the manager, then by the supplier. Certificates were inspected. A sample of MAR charts were inspected and showed that all administrations had been signed for. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23. Residents are protected from abuse and harm, by the proper procedures and by the open ethos of the home. EVIDENCE: No complaints had been made to the home since the last inspection. Relatives confirmed they could talk to staff about any concerns without difficulty. The home was fully up-to-date with the POVA Regulations. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,29. Residents can enjoy comfortable and safe facilities, personalised as they wish, to suit their needs. EVIDENCE: Two relatives were concerned that the extensive garden was getting somewhat out of hand round the edges. They were also concerned with the closeness of a willow tree to the house. The manager was able to report that an inspection by a tree expert had said that the tree was not a danger. The manager had also made arrangements for the tall, broad hedge on one side to be cut by a contractor. The home kept grass pathways clear for the residents to move round the garden at all times. A bathroom accessed by 2 residents from their rooms was in need of some redecoration. The side panel on the bath had been split to access the plumbing for regular checks, and this had also raised the vinyl flooring. Residents’ rooms were well decorated and personalised. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,33,34,35,36. Residents can expect a stable and well-trained staff, at appropriate levels, who can communicate with them and understand their needs. EVIDENCE: 4 staff had completed NVQ Level 2, and one had started on Level 3. This gave a 50 coverage of the staff. The recruitment of the newest staff member had been carried out according to the Regulations, with the required identification documents and application information. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,40,41,42,43. The management of the home, and its systems, supports and protects the residents. EVIDENCE: The manager is registered with the CSCI. He has one unit to finish, before completing his NVQ Level 4 in Care. The atmosphere in the home was cheerful and positive with staff showing good working relationships and motivation, and good communication with residents. The home followed Mencap’s comprehensive policies and procedures. All records were completed and stored appropriately. The Home is visited monthly as part of Mencap’s quality assurance programme, and the manager can access specialist advice and support. Staff training programmes cover key areas of health and safety, including food hygiene, fire training, and COSHH.
