This inspection was carried out on 25th January 2006.

What the care home does well

St. Andrews provides a comfortable and safe environment where residents are encouraged and supported to "grow", increase independence and feel enabled to live independently. This is achieved by having highly motivated management and staff who undergo continual training to meet the needs of residents. The manager listens to the staff and gives them autonomy. Staff have excellent relationships with the residents. Residents are not intimidated by the staff and are comfortable making any issues known to them.

What has improved since the last inspection?

No requirements or recommendations were made at the last inspection.

What the care home could do better:

Minor adjustments are needed to ensure that residents are protected by the home`s medication policy. It is recommended that when prescriptions are hand written on medication administration records two members of staff should be involved, one to write and one to check. Both members of staff should then sign the medication record.

This sometimes necessitates going on to residential care but some can manage areas where they are supported to be independent. The property is a converted former church in a central residential area of Exmouth. The residential home is situated on the ground floor of the building. All rooms are for single occupancy. There is a communal smoking room, lounge, kitchen and dining room. There is also a small laundry for residents use. There are carparking facilities on site. Also managed on the same site, but physically separated, are two sheltered housing units - one on the first floor of the main building and one in an adjacent bungalow. This announced inspection took place over two hours. The manager was present throughout the inspection. Not all of the standards were assessed at the time of this visit. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): YA Standard 6. Residents’ support needs are assessed and they are encouraged and supported to identify the goals they need to achieve. EVIDENCE: The care plans provide information about assessed needs and sets individual goals that encourages independent living skills with clearly written actions for staff to follow to address each goal. The care plans include any associated risks highlighted from the care plans. Any restrictions or limitations on choice or freedom to residents are recorded and risk assessed in individual plans. The individual care plans include behavioural management guidelines for residents who are likely to become aggressive or self-harm. These have been compiled with consultation and advice from other professionals, such as psychologists and nurse specialists. Daily recordings and regular reviews showed that goals were being met. Residents said that regular meetings are held with their link worker and that they are involved in the drawing up and reviewing of their plans of care. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): YA Standard 16 & 17 Residents’ rights are respected by staff at the home. Residents are encouraged to recognise, and supported to accept, responsibilities in their daily lives. EVIDENCE: Five residents said that they are treated as individuals, their wellbeing and independence is supported and their dignity and privacy is respected. All residents have keys to their rooms, and doors are kept locked affording them privacy and “ownership”. Residents said that staff respect their privacy at all times and do not enter their rooms without permission. Residents’ plans of care include agreements which have been reached over daily household tasks. Residents confirmed they have unrestricted access to the home and the grounds. Residents are encouraged to shop, plan and prepare their own breakfast and lunch. Regular residents’ meetings are held during which menus are discussed amongst other things. Records showed that residents are weighed regularly, and their nutritional needs are carefully monitored. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): YA Standard 20 Residents’ medication is well managed. Some improvement is needed when recording medicines received into the home. Clear, comprehensive arrangements are in place to ensure residents’ medication needs are met. EVIDENCE: Medication records and storage was found to be good. Records are kept of all medications received, administered and leaving the home. The administration records were up to date and generally in good order as was the medication cupboard. Some medication is written on to the medication administration records. When this is carried out it is recommended that two people sign to agree that the prescription has been checked and that the written directions are correct to maintain residents safety. Designated, appropriately trained staff administer medications. The pharmacy carries out 6-monthly checks and regular training for staff. The home has an excellent questionnaire for staff to complete as part of assessing their competencies in relation to the safe handling and administration of medicines. Some residents currently look after their own medication. The decision as to whether a resident looks after their own medication is based on individual needs and abilities to undertake the task. Residents are always involved in this decision-making as well as their care manager and link worker.
