Grantham is a market town, and contains a wide range of amenities. The home is close to a bus route; local shops and taxis are used to access the town centre. The home is a single story facility, arranged in four wings. Each wing has its own lounge and dining area, and there is a communal room at one end of the home, which is used for activities and entertainment. The home is built around a courtyard, which is fully enclosed, and laid to lawns, shrubs and patios with garden furniture. The home is registered for forty-six residents. Lifeline Nursing Services Ltd operates the home, and it is one of three homes operated by the company. There are adjoining warden - controlled bungalows operated by the same company. This is an overview of what the inspector found during the inspection. The inspection took place over 5.5 hours. A tour of the premises was undertaken. The registered manager and proprietor were provided with feedback at the end of the inspection. The main method of inspection used was called ‘case tracking’ which involved selecting three residents and tracking the care they receive through the checking of their records, discussion with them and the staff, and where more appropriate observation of interaction between staff and residents. A sample of care records, were inspected. At the site visit two members of staff were interviewed and the opinions of four residents were sought. The registered manager makes available to all potential new residents a copy of the statement of purpose for the care home when they visit for the first time. A copy is displayed in the care home reception area. The home charges a range of fees from £475 to £650 per week for residential care. Residents who require nursing care pay within the same range of fees but an additional nursing care fee is claimed by the care home from the government. The care home supplied to the Commission for Social Care Inspection preinspection information. This included a Pre- inspection questionnaire completed by the registered manager and 12 “have your say documents” completed by residents. The “have your say document” is a quality assurance document completed by residents. It asks the residents to answer 12 questions and to give their opinion on the service being provided to them by the care home. It offers them the opportunity to state if they wish to meet the Inspector during the site visit. Resident’s opinions and comments stated in “the have your say” documents are included in the main inspection report. What the service does well: What has improved since the last inspection?
The registered manager stated that she continues to work with the staff to improve the quality of services offered by the care home. The activity

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 3 & 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are satisfactory procedures for the introduction and assessment of people to the service, ensuring that all of their personal care needs, health care and social needs are met. The home does not offer at this time a dedicated intermediate care service. EVIDENCE: The care records for three individual residents were inspected as part of the case tracking process. They all contained a detailed assessment completed before the resident admission to the home. The assessment set out the social care and health care needs of each resident. They contained the wishes and choices of residents in respect of the way services should be provided. The residents have signed the initial assessment forms.

The care records for all three resident substantiated this statement. The three residents being case tracked confirmed that they have been assessed before they were admitted to the care home. They confirmed that staff had recorded their opinions, wishes and choices in relation to the way care should be provided to them. The registered manager confirmed that specialised intermediate care facilities are not offered by the care home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7,8,9, & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home identifies the health, personal and social cares needs of each resident and records them on their care plan. This enables staff to meet their needs in a manner, which is described within the individual’s care plan. The medication policy of the home is being followed. Staff respect the dignity and privacy of residents. EVIDENCE: Three individual resident’s care plans were inspected. They all contained the information obtained at the initial assessment .The care plans sets out the methods of providing safe care to each resident. Care files contained the resident’s choices of activities, health care needs, personal care needs and dietary needs. Where a risk had been identified the management of the risk had been balanced against the choices and wishes of the individual resident.

Medical history was recorded. The input from community health care services, chiropodist, dental care and eye care were recorded on each care plan. The registered manager stated only trained nursing staff give out medication. Nurses and carers spoken to during the site visit confirmed this statement to be correct. The registered manager stated that she was satisfied that all staff are competent in the management and administration of medication. She stated all the nurses were being provided with an additional training course called the management and safe handling of medication between November 2006 and February 2007. The nursing staff spoken to as part of the site visit confirmed that they were following the medication policy of the home which meets the National Minimum Standard. Resident’s comments obtained during the site visit provided evidence that staff do respect the privacy and dignity of the resident. One Resident stated “staff are very kind and they respect our privacy. They are sensitive in the way they provider us with care. ” A second resident stated “ this care home is 100 you will not find any fault with it. The staff are excellent. The food is wonderful it is a 100 home, you could not find better.” A third resident stated staff are good here they dont rush their work they treat us very well. All three residents interviewed stated staff do not enter our rooms without knocking they are kind and helpful”. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 12,13,14 & 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are encouraged to keep in contact with their family and friends. All visitors are made welcome by staff at the care home. Catering arrangements for the home reflects the service users choices, preferences and personal dietary needs. EVIDENCE: Care files contained details of the choice and wishes of the activities for individual residents. Observations were made during the site visit of activities taking place. The registered manager and proprietor stated that a regular activity programme is organised by the care home. Residents stated they were happy with the activities being provided by the care home. The registered manager stated that she had held discussions with staff and residents to ensure the activities were meeting the needs of all individual residents. It is her opinion that improvements have been made in this area since the last inspection. Details of activities, were displayed in the care

A written plan of the activities was given to the Inspector during the site visit. Residents decide their own routine each day including when they get up and go to bed. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 16 & 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home listens to resident’s views and wishes and acts on them. They are procedures in place to protect residents from any possible abuse. Staff have received appropriate training to protect residents from being harmed EVIDENCE: The registered manager stated that all staff have received training in the management and recognition of abuse. Staff confirmed in formal discussions that this statement was correct. Care records containing body maps, which are used to record any injury or bruise found on a resident. The manager uses these and accident records to monitored incidents and accidents. The care home has an abuse policy in place which staff were able to discuss. There is also a copy of the Lincolnshire county council vulnerable abuse procedures in the care home. Staff interviewed confirmed that they had read both documents. The “have your say” documents completed by residents provided evidence that they were aware of the homes complaints procedure. Complaints records were seen and discussed with the registered manager. All complaints are being investigated in line with the care homes complaints policy. Complainants have

Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 & 26 Quality in this outcome area is good This judgement has been made using available evidence including a visit to this service. Residents live in a comfortable, homely, clean environment with a choice of communal areas and personalised bedrooms. The infection control policy of the home is being followed. EVIDENCE: A tour was made of the care home. All areas were clean and smelt fresh. Ongoing maintenance has been carried out in the care home since the last inspection. Residence stated their satisfaction with the environment of the care home. A resident commented “ the home itself is nice I like my bedroom the lounges are comfortable.” There were no health and safety issues identified at this inspection. Staff stated that they felt safe working in the care home.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27,28.29 & 30 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home is staffed with appropriate numbers of care staff. Staff are trained to be able to answer the needs of residents who fit within the registration category of the care home. The managers of the care home have not consistently followed the home’s recruitment policy. This places residents at risk. EVIDENCE: Residents stated in the “have your say” documents that they are satisfied with the numbers of staff on duty. They confirmed that staff provide them with help if they press the call bell. Staff stated that they felt they could meet the needs of residents with present staffing levels. The registered manager stated that present staffing levels enable the needs of all residents to be met. There have been no complaints received by the care home or the Commission for Social Care Inspection in relation to staffing levels. The pre-inspection information provided evidence that staff have received training to ensure they can meet the needs of residents who fall within the registration category of the care home.

They raised no concerns about the ability of staff to meet their individual needs. The registered manager stated that she was following the care homes recruitment policy, which meets the National Minimum Standard. The individual file of a new member of staff was inspected. The registered manager had not obtained all the appropriate documents set out in the Care Home Regulations before employing this member of staff. It is a requirement that two written references be obtained. The individual file inspected to check recruitment standards contained only one written reference. The registered manager stated she had obtained a verbal reference from the past employer, but no written references had been sent. The registered manager obtained a second written reference within 24 hours of the site inspection for this member of staff. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31,33,33 & 38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There is good leadership and guidance for staff, ensuring that residents receive consistent quality care. The environment is safe. The home is run in the best interest of residents. EVIDENCE: The care home has an experienced registered manager in post, who is a qualified nurse. She has worked for many years in providing community care. She demonstrated throughout the site visit a detailed understanding of the Care Home Regulations. This knowledge enables the care home to meet both the needs of residents and the care home regulations.

Staff spoken to during the inspection stated the management are very supportive. The registered manager stated that she speaks to residents almost daily to ensure they are satisfied with the services provided by the care home. Resident spoken to during the site visit confirmed that the manager is approachable and will listen to their views regarding the care home. The registered manager stated the care home has a policy of not taking responsibility in any circumstances for residents finances. Residents living in the home manage their own finances including their personal allowance. If they are not able to do this family, advocates or social services department manage the money on their behalf. The care home enables them to find someone to carry out this task. The registered manager stated that staff are supervised in accordance with the National Minimum Standards. Staff interviewed during the site inspection confirmed this statement to be correct. The “have your say” documents completed by residents, discussions with residents and observations provided evidence that the home is run in the best interest of the residents. There were no health and safety issues identified in care records or during the site visit. Residents stated they feel safe living in the care home. Staff stated that the care home is a safe place in which to work. Training records demonstrated that infection control training is being provided.
