What the care home does well

Service users informed the inspector that they have good social contacts, that they regularly visits the theatre, go on outings have lunch out, and make the decision on what they want to do for their birthdays. One service user told the inspector that at her birthday party she had up to fifty people as she invited her friends from outside the home, and her relatives.

What has improved since the last inspection?

A new ventilation unit has been installed in the bathroom on the ground floor. A number of parasols have been bought for the service users as they like to sit out in the garden during the hot weather. Eight tabletop fans were bought for the service users bedrooms. New carpet has been laid in the dining room, and a new dryer has been bought to replace the old dryer. A new bed has been bought for the sleep-in staff.

What the care home could do better:

Employ a full time permanent manager for the home. Reduce the number of bank staff used at the home. Improve record keeping Seek advice from the dietician to reduce service users cholesterol levels; the acting manager is doing this.

Date of last inspection 24 January 2005 Brief Description of the Service: The home is registered to provide accommodation and care to eight individuals with a learning disability. Accommodation is provided in a detached property situated on a main road, with limited parking facilities to the front of the property. Bedrooms are situated on the ground and first floors of the home. There is a bathroom on the ground floor with an assisted bath for the use of service users. The home has a large spacious lounge which is divided into three areas, with a conservatory with air conditioning installed, which is used as a dining room. The back garden is well maintained. This is the first inspection to be undertaken in the Commission for Social Care Inspection year April 2005 to March 2996. This unannounced inspection took place on the 15th August 2005. The first part of the inspection was spent talking with service users as they were being made ready and some were waiting for their transport to take them on their various days’ activities. This was followed by discussion with the temporary acting manager regarding the new style of the inspection and report. The second part was spent reviewing service users care files. A good standard of assessment and care plans were observed. Time was spent talking to the care workers, observing service users and care workers interaction, which was friendly but respectful. A tour of the home followed with more time spent with the service users who were very informative about the care they receive in their home and the social life they enjoy outside their home. The grounds of the home and the outside of the house are in food condition. The home has been without a Registered manager for some time and this is reflected in the haphazard record keeping in connection to the health and safety of the service users. Requirement has been issued on this standard. The inspector spoke with five of the eight service users at the home. They all seemed happy and was free to speak with the inspector openly in the office, in the lounge and privately in their bedrooms. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The home was found to be operating effectively in respect of the standard inspected. EVIDENCE: A random sample of service users files was reviewed, and all contained care needs assessment prior to being admitted into the home. A number of the assessments were reassessed as the service users needs changed. Service users were also risk assessed with solutions to the assessed risk documented. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 9 The health and personal care needs of the service users are met with evidence of good care planning and documented delivery. EVIDENCE: Review of random service users files demonstrated that a comprehensive plan of care was drawn up for each service user. The inspector noted that care plans and risk assessments although very thoroughly prepared were not always signed by the service user or relatives on behalf of the service user. A requirement was issued on this standard. Care workers spoken to on the day of inspection informed the inspector that they worked from service users care plans as agreed with the service users The acting manager informed the inspector that none of the service users at this home are risk assessed as capable to self-administer medication. This was supported by a review of the medication records, which showed that medication administered was signed and witnessed by care workers trained to administer medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 13. Service users have good links with the community, church, their families and friends, which help to enrich their social and spiritual needs. EVIDENCE: One service user spoken to said ”I love it here, I go to the day centre Mondays, Tuesdays and Wednesdays where I get to meet my special friend”. She further stated “I go home regularly for long weekend with my family”. The acting manager informed the inspector that day services visits the home on Thursdays when service users are able to choose their activities for the day. Another service user told the inspector “I do my laundry and bedroom on Mondays. I like my room clean and tidy”. When asked about her spiritual needs she informed the inspector she visits the synagogue on special Jewish festivals and the home caters well for her special religious needs. The inspector was informed by the acting manager that the Church of England holds services at the home on a weekly basis, and one service user in particular visits her own Church of England services once monthly to keep up to date with events in her old community and to see her friends. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 The welfare of the service users is of high importance to the home. EVIDENCE: Training records reviewed and care workers spoken to confirm that the home train staff to recognise, and report incidents of suspected or actual abuse of service users. Of the five service users spoken to on the day of the inspection none of them reported that they have ever made a complaint. They were all very complimentary to the staff, and to one staff member in particular who had moved from the Manor hospital with them and is still at the home. They all reassured the inspector that if they had a problem they would immediately go and sort it out in the office. Review of the complaints folder showed the home had a complaints policy and procedure. The acting manager who has been in post since April 2005 informed the inspector that as far as she is aware the home has not received any complaints since the last inspection. The Inspector was unable to confirm this statement, as there was no complaints record file kept at the home. A requirement was issued on this standard. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 The standard of décor within and outside the home is good with evidence of continued improvement through maintenance and future planning. EVIDENCE: Service users live in a clean well maintained home. All the bedrooms inspected were clean and in good decorative order. The outside of the home was in good decorative order, and the grounds and garden were very tidy. The home presents as a comfortable and homely environment for the service users. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 30 The assessed health and social care needs of the service users are met with evidence of good documentation of actions taken by the care workers. EVIDENCE: From discussions with care workers and the rota documentation of the number and skill mix of staff on duty over a twenty-four hour period, it appeared adequate staff numbers were present to meet the assessed needs of the service users. The home relies on bank staff to supplement the staffing at the home. The inspector was informed that usually the home requests the same bank staff to aid in maintaining the consistency of staff. Discussions with care workers and review of the homes’ training records revealed that care workers are trained to do the work they are asked to perform. Regular updating of staff knowledge was provided and documented. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 38 The home has been without a registered manager for some time. Successions of acting managers have been employed by the home, which results in some instability at the management level. The care workers at the home has been more stable and this has reflected in the interactions between service users and care workers as observed by the inspector EVIDENCE: The lack of a registered manager for the home has resulted in various records relating to the smooth running of the home has not been kept up to date. Service users spoken to were aware of the lack of permanent manager and voiced their opinion that a permanent manager would be welcome. They felt very supported by the current acting manager but knew she was not going to be at the home on a permanent basis. Requirements were issued on this standard. Policies within the home such as recruitment, recognising and reporting of abuse, updating of knowledge and moving and handling all provide evidence that service users and care workers safety and welfare are crucial to the smooth running of the home. All the service users are registered with a General Practitioner, and they access the wider primary health care team expertise as and when required.
