Up to nine (9) Service Users who have a learning disability may be accommodated. 25th August 2005 Date of last inspection Brief Description of the Service: St. Germans is a care home providing residential care for nine people who have a learning disability.It is owned by Exceler Healthcare Services Limited.The home is located in the village of St. Germans approximately five miles from Kings Lynn.The home is close to the village shop, post office , church and village pub.The home, previously a pub, was converted into a residential home and opened in 1984.The home comprises of the ground floor accommodation of two reception rooms, dining room, kitchen and office. There is one single bedroom on the ground floor. The first floor has three shared rooms and two further single rooms.St. Germans shares its garden area with Abbotsford another small home for people with learning difficulties, owned by the same Proprietors. This is an overview of what the inspector found during the inspection. This was an unannounced inspection taking place on a weekday over 4 hours. The Manager, Mrs Penny Clare was present throughout the inspection. There were eight residents accommodated on the day. A tour of the building was undertaken and some records were viewed. One resident was spoken to privately and seven were spoken to in a group. Three members of staff were spoken to privately. What the service does well: What has improved since the last inspection? Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 The residents are consulted on, and participate in all aspects of life in the home. EVIDENCE: The Manager and staff regularly consult with the residents informally and they hold monthly meetings. This meeting is recorded in large print and an action plan is drawn up, and feedback is given to the residents. During the day there was plenty of evidence to show that the residents are able to make decisions and participate in all aspects of life in the home. Some examples include, every Sunday the residents decide upon their menu for the following week and if they want to they help to prepare the food. Some enjoy cooking and one resident had made some scones and another some biscuits. On the day of inspection a resident was laying the tables and helping with the tea of cheese and potato pie. Following this they will have the food cooked by the residents. The resident had chosen the colour and style of the hallways and the colour and furniture for their bedrooms, although some were yet to be decorated and refurnished. Service users are offered a healthy diet and enjoy their meals and mealtimes. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. JUDGEMENT – we looked at outcomes for the following standard(s): 22 There is a clear, pictorial complaints procedure, ensuring the residents understand it. The resident felt their views were listened to and acted upon. EVIDENCE: There is a clear complaints procedure in pictorial form and each resident has a copy in his or her bedroom. One resident was very clear what she would do if unhappy about something and also said “the staff always have time to talk to me”. She said she feels happy and safe. EVIDENCE: Many parts of the home are newly decorated, and this has been tastefully done incorporating the ideas and wishes of the residents. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for the following standard(s): 32 Competent and qualified staff support the residents. EVIDENCE: The home benefits from an enthusiastic staff team. One said “I love it……….…. I don’t look on it as a job”………. “I think, good I’m going to work”. The staff spoke of the many training opportunities offered to them, and of the fact that if they asked for some training, the Manager organized this immediately. All training is up to date and regularly updated. Three members of staff have achieved the NVQ level 3 and two will be starting in February. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37 The residents’ benefit from a well run home. EVIDENCE: The Manager has a wide range of knowledge and experience of people with learning disabilities, having worked in this field for fourteen years. She is congratulated for achieving the Registered Managers Award and will be commencing the NVQ level 4 in Care in February. She also said that she would like to do the Diploma in Dementia once the other training is completed. She has completed many other training courses; for example, Train the Trainer in dementia, health & safety, abuse and fire awareness. This is a well run home where the residents are enabled to make many decisions and choices regarding their lives within a robust risk assessment framework. The staff benefit from informal, and regular formal supervision sessions, and the many training opportunities offered. They said that the Manager is very supportive and approachable. The home is continually improving in its environment as well as day-to-day issues. The Manager is commended.
