CQC found this care home to be providing an Adequate service.

The inspector found no outstanding requirements from the previous inspection report, but made 5 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

The service now ensures that all care records are stored in a locked cupboard at all times. This ensures people`s privacy is maintained and the records are secure at all times. Complaints that the service receive are now recorded, in a complaints file, which is accessible so we can see what action is taken. Staff have received a briefing session, to ensure they are aware of the procedure and their role in receiving and recording any complaints made about the service. A new manager has now been appointed to the service and is due to start work in a months time. This will provide the staff with the required leadership, and support in their role.

What the care home could do better:

All complaints received by this service need to be addressed within the timescale reflected in there procedure. This is to ensure issues are responded to within a clear timescale and feedback provided to the complainant. Staff need to have training which is specific to their role, to ensure they have the skills and knowledge to support people they are working with. The service needs to purchase a cupboard that meets the legal requirements for the secure storage of controlled drugs.

The home is located on the same site as another home owned by this organisation and the same Registered Manager is responsible for both homes. The home was purpose built to a high standard and registered in July 2005, to provide personal care and accommodation for up to 8 people in the category of learning disability, aged between 18 and 65 years of age. At the present time the group of people living at the home are all aged under 30 years. The home is situated in Chaddesden just outside Derby City centre and is close to local amenities. A car park in available at the front of the premises and a garden at the rear. The home provides a spacious environment with all rooms being of single occupancy with en-suite shower facilities. Care Homes for Adults (18-65 years) Page 2 of 8 What we found: The main purpose of this visit was to check the progress made on addressing the requirements made at the last key inspection which was undertaken on the 6th October 2008. A pharmacy inspector accompanied us on this visit and they looked closely at the medication practices in this service. We also looked at the staff training records, complaints, fire records and the leadership within the service. We looked at the record of complaints and this indicated that two issues had been received since our last visit. One of these complaints was received recently and is currently being addressed. The second complaint was received verbally in December 2008. Although the complainant agreed to put their concerns in writing they did not follow up this action. The complaint was recorded but no action has been taken since it was received. The acting manager has since left the service and the operations manager is currently managing the service. She addressed the complaint during our visit and provided evidence of the outcome the day after our visit. She also provided in-house training to the staff during their team meeting in relation to the complaints procedure and how to complete the required documentation. An updated training matrix which reflected the training undertaken and outstanding for the current staff team was provided to us. This record demonstrated that staff have completed induction training which covers how to safeguard people from risks. The records demonstrated that four staff have not completed moving and handling training but training was now planned. There are ten staff who have not completed training in relation to epilepsy and four staff have not received training on how to support people with autism. This training is required as the people who live in this service have these support needs; therefore the staff should have the skills and knowledge, so they can provide appropriate support to these people. We were told that a new manager has been appointed and he will commence employment in the service in July 2009. When we checked the Fire records they demonstrated that weekly tests were now being undertaken to ensure the system is in good working order. We looked at the homes arrangements for storing and recording medicines, and how two peoples medical needs were met. We found that the home does not have a cupboard for storing controlled drugs, should these be prescribed for a person living in the home. We also found that the refrigerator that is available for storing medicines has no lock or thermometer for recording the fridge temperature over a 24 hour period. Other medicines were safely stored, and we saw that tablets supplied by the pharmacy in their original manufacturers packs were counted twice a day. We noted that records of the receipt and administration of medicines, on the current medication charts were complete. However we found that where changes in the dose of two medicines had been made, the persons chart did not show when or why this had happened. This to minimise the risk of medication not being given as prescribed. 2 20 13 A cabinet that complies with 10/07/2009 the Misuse of Drugs (safe custody) regulations must be obtained and correctly installed. This is to ensure controlled drugs prescribed for people in the home can be safely and legally stored. 3 22 22 Complaints must be responded to within the timescale detailed in the procedure. This is to ensure they know how to support and respond to people with these needs. 31/07/2009 Recommendations These recommendations are taken from the best practice described in the National Minimum Standards and the registered person(s) should consider them as a way of improving their service. It would beneficial if the 10 staff identified at the inspection attend training in relation to autism.
