The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service users spoken with were complimentary about the standards of care provided within the home. Comments included: "You can`t fault them, I`m happy and I`ve settled here". "Yes, I wouldn`t change much". "I have said before that I am very satisfied, with it all, absolutely faultless". "The food is plentiful and well cooked". "The staff are kind, they are a good team and its well run". The home records are clearly written and stored. Staff recruitment procedures are very good overall. The care staff receive an induction and they are well trained. The staff have a good understanding of the needs and preferences of each resident. Throughout the visit staff were observed to be attentive and polite when speaking with residents. The organisation continues to invest in each aspect of the service. The physical environment is clean, tidy, comfortably furnished, well maintained and are liked by service users. The owners and staff work hard to achieve this.

What has improved since the last inspection?

The acting manager is in the process of securing some formal management training. A new care recording system has been implemented and this is working well with only minor adjustments needed. The home has recently received a positive report following a pharmacist inspection of the medication systems. Any requirements have been met.

What the care home could do better:

Some minor adjustments are required regarding the recording of information regarding service users to fully document when each service users care plan has been reviewed and by whom. The homes recruitment procedures are good overall although individual staff records must include a photograph. Minor adjustments are required to minimise risks to service users prone to wandering.

The home is registered to provide personal care for service users of both sexes whose primary needs fall within the following categories 39 OP and 1 named person (SI) as included in the proposal letter dated 26th March 2003. The maximum number of service users to be accommodated is 39. Date of last inspection 14/12/04 Brief Description of the Service: Summerfield Rest Home is a detached two-storey building situated in a quiet residential area in the seaside resort of Sutton-on-Sea and is close to all local amenities. There are thirty-two single bedrooms and three double rooms. The doubles and twenty-eight of the singles have en-suite facilities, although all single bedrooms have a toilet. A stair lift is installed for access to the first floor. There is a choice of six lounges, plus a bar room which is also the area designated for smokers. The dining room, with its annexe, is able to accommodate all service users. The surrounding gardens have been landscaped and are a feature that the proprietors continue to put a considerable amount of work into. Car parking is available to the front and side of the building. The inspection took place over 4.5 hours, it was unannounced and was carried out by two inspectors. A tour of the premises was conducted, service users care records and staff records were inspected. The inspectors spoke with the proprietor, acting manager, six service users and three relatives. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 4, 5 Satisfactory procedures and administrative systems are in place for the introduction of residents to the home. Information is provided to prospective service users to enable individuals to make informed choices about admission to the home. Only minor alterations are needed. EVIDENCE: A service users guide and a statement of purpose have been produced, which detail the services provided by the home and its terms and conditions. These documents are made available to prospective service users prior to admission. Three service users personal files were seen, which contained pre-admission assessments. The home administrator is in the process of devising letters to be sent to each service user prior to admission confirming that the home is able to meet their needs. Individually signed terms and conditions of residence contracts were maintained on each of the service users files inspected. Comments regarding the admission process included: “From what I can remember it was all handled well”. “The staff were very good, kind, helpful, it wasn’t easy to do but they were very supportive”. “They helped me and helped me settle”. Admission policies and procedures are in place giving guidance to staff, which are discussed as part of induction to the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10, 11, The homes care plans and risk assessments are well maintained overall and provide staff with the required information to meet service users care needs. Service users health needs and privacy and dignity is promoted and met. EVIDENCE: Policies and procedures regarding the homes care philosophies and practices are in place, giving guidance to staff. A care plan and risk assessment is completed for every service user upon admission, these clearly detail individuals care needs and document how these are met. Care records are generally updated on a daily basis and reviewed each month or as changes occur although minor alterations are required to document this. The care plan document is signed by service users, or their representatives where possible. The six service users and three representatives spoken with confirmed that staff provide the care needed and that supporting health services are utilised as required. Comments included: “Its always the staff that make a place and here the are very good”. “They get busy but you cant knock them, they’re a marvellous lot”. “Yes, I am satisfied with the treatment here”. The staff were observed carrying out care tasks and they were respectful to the service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 Service users are supported to express their views regarding life within the home, the care they receive and to maintain and develop community links as they prefer. The activities provided by the home are enjoyed by service users. Relatives and friends of service users are made welcome in the home. EVIDENCE: The service users spoken with confirmed that they were no restrictions as to how they spent their time and that they were able to maintain and develop interests or relationships as they preferred. The relatives spoken with said that they were always made welcome to the home. Comments included: “When we visit there is always a welcome, we have drinks and have eaten here a few times too”. “The staff are very good, very welcoming, to us all”. Information regarding the homes care philosophies and the promotion of service users rights are included in the statement of purpose. Service users are consulted with about their likes and dislikes and they have the opportunity to express their views and offer ideas at the residents meetings of which records are maintained. The home also provides information to service users detailing how they may contact external agencies, e.g. advocates. The service users spoken with were satisfied with the variety and frequency of activities provided and said they can choose whether to join in. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The service users spoken with feel safe and confident in approaching care staff regarding any concerns that they might have. Clear guidelines are in place for service users and care staff. EVIDENCE: Complaint policies and procedures are in place and information regarding this is provided to service users. The service users and representatives spoken with said that they felt able to voice any concerns regarding the home and that these would be acted upon. The two care staff spoken with were familiar with complaint, whistle blowing and abuse policies and procedures. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 22, 23, 24, 25, 26 The standard of the physical environment is good, with the proprietors ensuring the maintenance of each area of the home. All private and communal space is suitable for the residents and is homely and comfortable. Service users are able to personalise their own accommodation as they prefer. EVIDENCE: All areas of the home are furnished and decorated to a good standard and the facilities are appropriate to the needs of the service users. Maintenance programmes are in place. Domestic staff keep the home clean and odour free and health and safety policies and procedures give guidance to staff. Each radiator is covered and water temperature restrictors are fitted to water outlets to minimise the risk of burns or scalding. The ten service users rooms viewed identified that rooms are personalised and contained individuals personal effects. The service users and their representatives spoken with were satisfied with the physical environment. Comments included: “The place is spotless, it’s quite lovely, the owners put a lot of work into it”. “Whenever you come its always clean and presented nicely”. “It’s kept very nicely”. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 There are sufficient staff to allow them to care for the residents. Recruitment procedures are in place and the staff are trained, competent and they receive induction when starting at the home. EVIDENCE: The staff records viewed contained the majority of the required documents and records to demonstrate that appropriate recruitment procedures had been followed and were well maintained overall. The home administrator is in the process of including photos within each staff members personal records. The service users spoken with commented that their care needs were met and that there were sufficient numbers of staff deployed in the home. Comments included: “The staff are busy but you don’t have to wait for too long to get help”. “They do a wonderful job, I’ve no complaints”. “I can say that they all work hard, you sometimes have to wait a short while but you expect this”. Newly appointed staff receive an induction and a rolling programme of training is in place. The two staff members spoken with said that they received an induction and training to enable them to carryout their role. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33, 37, 38 In the absence of a registered manager the service users are satisfied with the overall standards of care and that they are supported to contribute to the running of the home. The health and safety of service users is promoted overall and only minor adjustments are required. EVIDENCE: Residents meetings are held and quality satisfaction questionnaires also assist service users and their representatives to express their views regarding the home. Service users said that they felt able to express their views regarding the home and that these generally would be acted upon. Health and safety policies and procedures are in place, giving guidance to staff. Risk assessments have been completed for all service users and of the premises.
