What the care home does well

Detailed pre-admission assessments are in place and contain full information about the needs of residents. Detailed care plans are also in place and the process for reviewing them has been improved. Further improvements are planned. A comprehensive and interesting range of activities is in place with an event for every day. All of the resident`s bedrooms (except for one) have an en-suite WC and washhand-basin. All areas of the home are well decorated and furnished in a domestic style. The home has a calm and supportive atmosphere and residents are treated with respect.

Both the home`s statement of purpose and its service user guide have been updated to comply with National Minimum Standards. Internal doors on the lift have been replaced. Care plan reviews are more detailed.

The building has four storeys, including a basement that provides offices, kitchen, dining room, laundry and other facilities used by staff. All storeys are served by a passenger lift. Resident’s live in 22 bedrooms, one of which is available for sharing and all but one have en-suite facilities. There are two lounges and a number of assisted baths and showers around the home. Summer Fields has a garden area to the front and a yard area to the rear. Work to update facilities has been completed. The owner has plans to improve all areas of the home and to extend the property thereby providing a number of additional places. This unannounced inspection of Summer Fields was undertaken on Tuesday 8th August 2006 over a period of 4 hours. It involved meeting with the deputy manager, the examination of some records, a tour of the building and discussions with two of the residents. The home’s manager had completed a pre-inspection questionnaire and had returned it to the inspector on 3rd July 2006. The inspection was carried out as part of the Commission’s responsibility to visit and report on all residential care homes. What the service does well: What has improved since the last inspection? No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5 and 6 Prospective residents have the information they need to make an informed choice about where to live. Detailed pre-admission assessments are carried out and admissions to the home are managed appropriately. EVIDENCE: The home’s statement of purpose and service user guide have been amended and now contain all matters identified in Schedule 1 and standard 1.2 of the National Minimum Standards, Care Homes for Older People. Each of the residents has a contract/terms and conditions of residence a copy of which is kept in the home. Four resident’s care files were examined during the inspection. Each contained a detailed pre-admission assessment completed by a senior member of staff at the home. Assessment documents from the placing authority are kept on some of the resident’s files. Routines within the home have been developed to ensure care is provided in a safe and supportive manner. Prospective residents (and their families) are invited to visit the home and to spend some time there before making a decision to move in. They are also given time to settle into the routines of the home before taking a decision to stay. The home is not contracted to provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 and 11 Detailed care plans are in place and the recording of reviews has been improved. The health care needs of residents are given appropriate priority. EVIDENCE: Detailed care plans are kept on residents care files. Daily write-ups refer to particular elements of the plan and thereby confirm they are used by staff on a day-to-day basis for the delivery of care and support. The arrangements for recording reviews have been improved and there is evidence on some of the files that the new process is being used. The home’s deputy manager discussed proposals for developing further the care planning and review arrangements in the home, all of which would give enhanced opportunities for the delivery of care and support. The home is anxious to include relatives in all of the care planning arrangements and will be further considering ways in which this might be achieved. The home has good support form GP’s, district nurses, a visiting optician, a local dentist and a chiropodist. The continence adviser is supportive to the Summerfields Residential Care Home DS0000061741.V299103.R01.S.doc Version 5.2 Page 11 home but it can sometimes take up to eight weeks for appropriate products to be delivered. Arrangements for the storage, management and handling of medicines were examined during the inspection and found to be in good order. The PCT has advised homes of the arrangements to be followed when ordering prescriptions. The deputy manager is to discuss these with the pharmacist and the GP practices to ensure a smooth transition of practice. During the course of the inspection residents were treated with dignity and respect. All personal care tasks are carried out in the privacy of their own room or in one of the bathrooms. Not all of the residents are comfortable with taking their meals in the communal dining room. Staff are therefore supporting them to take their meals in their own room or in one of the communal lounges. Records confirm that staff are discussing the resident’s wishes at the time of death and recording them on the care file. Some of the residents have funeral plans in place and most families have indicated that they would wish to make all of the arrangements themselves. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 A full and varied programme of activities is in place. Relatives are welcomed at any time. Food served in the home is well prepared. EVIDENCE: The deputy manager allocates half of her time to the organisation of activities in the home. A programme that includes visiting entertainers and trips out is arranged on a month-by-month basis. During the course of the inspection residents were asked where they would like to go on trips out from the home. All staff assist with individual and group activities and provide one-to-one support for residents when necessary. A realistic activities budget is provided and the home holds an expanding stock of games, videos and equipment. Information provided by the home identifies that some of the residents are supported with small gardening projects and with baking. The inspector is impressed with the range and frequency of activity at the home. Relatives and friends are encouraged to visit the home at any reasonable time but are asked to avoid mealtimes if possible. Residents are encouraged to exercise personal choice and to remain as independent as possible within the scope of their ability. They confirmed that a wide variety of foods are provided that reflect the cultural and preferred choices of residents. Staff are aware of particular likes and dislikes but residents who do not wish to take the main meal on offer are provided with an alternative. The food being served at the time of the inspection appeared well prepared and presented. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 and 18 Appropriate procedures are in place to deal with complaints and to protect residents from abuse. EVIDENCE: The home has a complaints procedure in place that is available to residents and their families or supporters. The procedure is displayed in the home and a record book is maintained to confirm the details and outcome of any matters raised. No formal complaints have been to the home or to CSCI since the inspection in March 2006. All of the residents are listed on the Electoral Register and have the opportunity to vote in local and national elections. The home has policies and procedures in place to deal with any concerns about the treatment of residents inside or outside of the home. Staff are familiarised with the procedures during their induction to the home and appropriate formal training is to be provided. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25 and 26 Residents live in a safe and well-maintained environment. EVIDENCE: Summer Fields is set in a residential area of Rock Ferry, Birkenhead, close to local bus routes. The building has benefited from a programme of refurbishment and further improvements are planned. The homeowner also has a programme of continuous improvement to the furnishings and decoration of the home. Early discussions are proceeding with the intention of extending the capacity of the home. Since the CSCI inspection of March 2005 the internal lift doors have been replaced and can now be fully closed. All of the resident’s bedrooms (except for one) have an en-suite WC and wash hand-basin. Many of the rooms are quite spacious with sitting areas but all of them are decorated and furnished to a good standard. The home has two lounges, one to the ground and one to the first floors. Both are decorated and furnished in a domestic style. The dining room has been created in the refurbishment programme and is located in the basement next to the kitchen. Assisted bathing facilities are available on the ground and first floors. Additional facilities are planned in any new developments at the home. All of the radiators are fitted with protective covers to prevent residents coming into direct contact with hot surfaces. The home is maintained to a good standard and on the day of this inspection was free from any offensive odours. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Residents are protected by the home’s staff selection procedures, rota working arrangements and staff training programmes. EVIDENCE: The home employs a manager; a deputy manager and 14 care staff. There are also 10 ancillary workers in post. A senior care assistant and the deputy manager were on duty at the time of this inspection and this arrangement is reflected in the sample rotas provided prior to the inspection. Two care staff are on wakeful duty throughout the night. Cooks, domestics and a handyman are also in post. Nine of the current 14 care staff have an award at NVQ level 2 or above in care and the home has a commitment to continue to support staff in gaining a formal qualification. Some overseas staff are employed through an agency that brings eastern European workers into Britain. The home has been pleased with the quality of workers that have been recruited in this way. An examination of a sample of staff files identified that appropriate procedures are being put into place for the recruitment and retention of staff. Overseas workers had all of the required checks and permissions. Files contained an application form, two references, confirmation of identity and confirmation that POVA and CRB clearances had been obtained. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35, 36, 37 and 38 The home is run in the best interests of residents. EVIDENCE: The home’s manager is qualified and experienced in the care of older people and is registered with the Commission. On the day of this inspection the home was busy and staff were getting on with their everyday work without constant reference to the deputy manager. Overall there was a calm and relaxed atmosphere and staff were observed to be supporting residents in a calm and respectful manner. The home’s financial accounts were not examined during the inspection. However, the homeowner has invested a considerable amount of money in the refurbishment programme and is considering further developments on the site Summerfields Residential Care Home DS0000061741.V299103.R01.S.doc Version 5.2 Page 20 and the Commission is not aware of any matters that would affect the continued operation of Summer Fields. The home’s staff manage only small amounts of money on behalf of residents. Detailed records were examined at the CSCI inspection in March 2006 and found to be detailed and up to date. Deposits were recorded and withdrawals supported by receipts. The home has begun the process of staff supervision and is developing the arrangements to ensure staff receive one-to-one time with a senior member of staff on six occasions each year. A record of the sessions already held was kept on the individual files and the inspector discussed ways of improving the system, with the deputy manager. All of the records seen during the inspection were being kept to a good standard, were up to date and were being stored securely. The manager has confirmed that policies and procedures required for the running of a care home for older people are in place and are being reviewed. Records of fire alarm tests and of staff training in fire awareness are being kept. Environmental Health Officers inspected the kitchens in January 2006. The home’s gas and electric wiring safety certificates are up to date. New resident’s call system was installed in January 2006. Checks on the water storage arrangements for Legionella were undertaken in October 2005.
