First inspection since new registration on 02.12.05. Date of last inspection Brief Description of the Service: Sunrise is a residential home registered to provide care for twenty older people over the age of 65 including up to ten older people with dementia. Four residents with a physical disability and one with a mental disorder living in the home at the time of registration are able to continue living in the home while their needs can be met. The home is situated in the village of Little Addington in East Northamptonshire. The home is owned by Le Flamboyant Ltd. The premises consist of a detached house which has had an extension built on and accommodation is provided in 18 single rooms and one double bedroom. All bedrooms except one are located on the ground floor and Eight have ensuite toilets. Access to the first floor bedroom is via the stairs and would therefore require good mobility. The home has a well-maintained garden, which is accessible to the service users, with pleasant views over the local countryside. The home is easily accessible by car; however there is no public transport to the village and access, without a car would be by public transport to Irthlingborough followed by a taxi to the village. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out over approximately three and a half hours on the late afternoon of a weekday. Prior to the inspection the inspector spent half an hour reading the homes service history and planning the areas to be inspected. At the time of this inspection the new owners had only been registered for a few weeks. In view of this and the fact that two complaints had been received within three days of registration the focus of this inspection was to check the well being and care of the residents. This was carried out through talking to residents and staff and observing interactions between staff and residents. Residents and staff spoken to during the inspection confirmed that they were feeling more settled as they got used to the change of ownership which had come as a surprise. Three residents care records were reviewed to check how resident’s care and health needs were being assessed and to ensure that relevant health professionals were involved. During the inspection a sample of resident’s records was reviewed to check how their care is planned and supported. Discussions with residents, a relative and staff and observations of the daily routines and interactions between staff and residents were made. Staff support and training was discussed with a member of staff and the responsible individual and staff rotas reviewed. Plans for decoration and refurbishment were discussed with residents and the responsible individual. What the service does well:
After the initial difficulties for residents with the change of ownership a feeling of trust appears to have been established. There was a comfortable atmosphere in the home and conversations between residents and between staff and residents with some light hearted banter. The new owners are treating the assessment and review of residents care and health needs as priority and there was evidence of a thorough approach to this.

Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3, Std 6 is not applicable as the home does not provide intermediate care. The admission process provides assurances that new residents will be admitted only on the basis that their needs can be met within the home. EVIDENCE: The home has a statement of purpose, which is available to prospective residents and their families. The statement of purpose provides information about the home and the services it provides. No new residents have been admitted since the new registration. Discussion with the responsible individual confirms that a thorough admission process, which will include assessment of a prospective resident’s needs, will be carried out prior to admission to ensure that their needs can be met within the home. The responsible individual confirmed that careful consideration will be given to the needs of both existing and prospective residents when considering a new admission. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 Resident’s care and health needs are being appropriately assessed with relevant referrals to health professionals and guidance to staff through care plans. EVIDENCE: A sample check of three residents care records identified that since the new registration residents care needs are being re-assessed and existing care plans are being reviewed and revised as necessary. This process had not been fully completed at the time of this inspection however the responsible individual confirmed that all assessments and care plans would be completed by the end of January 2006. There was evidence that appropriate referrals are being made to health professionals such as the General Practitioner, District Nurse, Community Psychiatric Nurse and the diabetic nurse. New care plans are being implemented as soon as health care issues are identified. Residents spoken to during the inspection were satisfied with the care being provided. Staff on duty had no concerns about the care being provided.

Advice was given to record any medication carried forward to the next cycle to ensure that accurate stock checks can be carried out. The inspector also advised that the date of opening should be noted on eye and eardrops, which have a short shelf life once opened. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Residents and staff know how to make a complaint. EVIDENCE: The Commission for Social Care Inspection (CSCI) received two complaints three days after the home was registered with the new owners. The complaints related to residents being unsettled following the takeover, organisation and staff training and practices. The responsible individual investigated the complaints using the homes complaints procedures, and a response forwarded to CSCI. While there was a general acknowledgment that residents and staff were unsettled due to the changes the specific areas of complaint were either not upheld or a satisfactory explanation given which did not put residents at risk. Residents and staff spoken to during the inspection confirmed that they were feeling more settled as they got used to the change of ownership which had come as a surprise. Residents and staff were aware of how to raise any concerns within the home. The role of The Commission for Social Care Inspection was discussed and contact numbers given. Staff were aware of their responsibilities to report any concerns they had that would affect the welfare of residents. No concerns were identified during the inspection.

The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 Residents are encouraged to see the home as theirs by being involved in choosing the décor and carpeting. EVIDENCE: The new owners have carried out a review of the premises and have started to do some re-decoration and refurbishment. The lounge has been painted in colours chosen by the residents. A resident told the inspector that they had also been involved in commenting on carpet samples in order to choose a new carpet for the lounge. The responsible individual is giving careful consideration to the suitability of the carpet patterns for people who may have dementia or poor sight based on research. This inspection did not involve a full premises check however the inspector saw the communal lounge and a resident’s bedroom at his invitation. The resident had recently moved bedrooms to enable easier access for assistance with movement and handling and was quite happy with the new arrangements.

Residents tend to sit in the same place in the lounge however some changes had been made with some of the more communicative residents now seated together. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 There is a strong commitment to increasing staff training to build on their knowledge and skills in meeting resident’s needs. EVIDENCE: At the time of the inspection there were sufficient staff to meet the needs of residents. Review of the staff rota indicated that there were times when there was only one member of staff on duty however discussion with staff and the responsible individual confirmed that another member of staff had been on duty, which had not been recorded. The responsible individual confirmed that he will ensure that the rota includes everyone working in the home. Staff training and development is considered to be important by the new owners and an assessment of staff training needs commenced immediately following registration. Appraisals are being carried out for all staff with plans to follow up with regular supervision sessions. Training records were not reviewed however staff confirmed that some training has already been provided. At the present time none of the staff hold a National Vocational Qualification however the responsible individual advised that staff are keen to work towards a qualification and arrangements are being made to support them with this.
