What the care home does well

People considering moving into the home receive a full assessment and are provided with the opportunity to visit and spend time at the home to make sure that it is able to meet their needs. The home is a well-appointed, suitably equipped and comfortable environment for elderly people, including those with dementia. The home practices excellent standards of dementia care, incorporating many innovative techniques designed to support, assist, orientate and relax residents. On the day of inspection the home was very clean and adequately staffed. A varied programme of social and recreational activities is provided. Meals are nutritious and appetising and the choice and variety is very good.

New service Date of last inspection Brief Description of the Service: The first Sunrise Community opened in the USA in 1981 and the Company has expanded both in the USA and, more recently in Europe. Sunrise provides care services to older people and has developed in the UK along the lines of the model operated in the USA. The first Sunrise community opened in South East London in 1998. Sunrise Operations Southbourne Ltd is a large purpose-built building set in its own grounds in a residential area of Southbourne, relatively close to a wide range of local amenities and the sea front. There is extensive vehicle parking to the side and a large landscaped garden with patio areas to the front and rear. Access to all parts of the building is via the reception area on the ground floor. The Reminiscence Neighbourhood, (the subject of this report) specialises in the care of elderly people with dementia and comprises the second (top) floor of the building. On the ground and first floor levels is the Assisted Living Neighbourhood being a separately registered service operated by the same provider organisation. Private accommodation is provided by a variety of suites for single and shared occupancy: singles, studios, “open” and “closed” doubles. In total there are 18 rooms/suites for single occupancy and 8 suites potentially for shared occupancy (“closed” doubles). Each of the suites has a large en-suite shower/toilet/wash-hand basin; for a few suites the en-suite facility includes a bath with shower attachment. The home provides all meals, laundry and domestic services. A frequent, flexible and varied programme of social and recreational activities is offered. Laundering of clothing and household linen is carried out within the home at no additional cost to residents; items requiring dry cleaning are charged additionally because it is necessary to send them out of the home for this service. There is a well-appointed hairdressing salon; a hairdresser regularly visits the home - there is an additional charge for this service. Fees are charged weekly; the fee range quoted in the service user guide at the time of inspection was (per person) from £700 to £1400. Fees for the Reminiscence Neighbourhood are based on the ‘room charge’ and include food, laundry (by care staff, or by residents using the residents laundry room), activities, Wellness (health promotion), and up to 3 hours per day care provision. Fees do not include the separately negotiated ‘care package’ i.e. when care is required for more than 3 hours per day. The quality rating for this service is 2 stars. This means the people who use this service experience good quality outcomes. This was a statutory inspection required in accordance with the Care Standards Act 2000 and was the first inspection of this new service. The inspection was unannounced; the inspector arrived at 12.15 on 13 August 2008, toured the premises and spoke to residents, staff, observed staff interaction with residents and the carrying out of routine tasks and together with registered manager Mrs Cockrell and other staff discussed and examined documents regarding care provision and management of the home. The duration of the inspection was 2 hours. The length of the inspection was relatively brief because during the previous month the Assisted Living Neighbourhood had been inspected and much of the information and evidence considered at that time (e.g. for premises management, administration) is relevant to both services. During the inspection, particular residents were ‘case tracked’; for example, for evidence regarding Standards 3, 7 and 8, records relating to the same residents were examined and the residents spoken with. During this inspection compliance with all key standards of the National Minimum Standards was assessed. EVIDENCE: The records of 3 recently admitted residents included details of pre-admission assessments carried out by senior staff visiting the prospective residents at their previous addresses. In advance of making the decision to enter the home the prospective residents or their representatives visited the home to view the premises and meet residents and staff. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. This judgement has been made using available evidence including a visit to this service. The health and social care needs of residents are met by suitably trained staff; doctors and other professionals visit as necessary and residents receive the medicines they have been prescribed. EVIDENCE: The care records of 3 people who live at the home were examined and found to contain risk assessments forming the basis for care plans and daily records describing the care of each person. There was evidence that individual residents or their representatives had been involved in the development and review of planned care provision. Medicine handling is carried out by staff trained in this work. Medication records were properly kept indicating that residents receive prescribed medicines at the correct times and in correct amounts. Residents are treated with respect and their privacy and dignity is protected at all times. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. This judgement has been made using available evidence including a visit to this service. Residents have opportunities to engage in social and recreational activities and are encouraged and supported to pass the time according to individual preference. A choice of menu is provided and meals are nutritional and appetising. EVIDENCE: A varied programme of social and recreational activities is arranged during every weekday; to ensure the satisfaction of residents the activities are flexible and usually carried out on a one-to-one or small group basis. Many examples of excellent practice were seen during the inspection e.g. some staff were playing card games with residents, some were walking around the home with those who were unsettled/anxious, others were patiently conversing with residents with whom they were seated in a sociable way, while the residents enjoyed refreshments. Sunrise Ops Southbourne Ltd (Reminiscence Neighbourhood) DS0000071303.V369630.R01.S.doc Version 5.2 Page 14 There are many opportunities for residents to engage in activities; around the home are a variety of items including percussion instruments, baskets of household items including dusters which residents can use if they wish, brightly coloured craft equipment, items of reminiscence, and a comfortable and well equipped sensory/quiet room containing specialist lighting and music. Residents can participate in the regular excursions arranged by the home, and can readily gain access to fresh air in the safe environment of the terrace which overlooks the landscaped gardens of the home. Visitors are welcome at any time and those the inspector spoke to said they are always made to feel welcome and placed at ease by the staff. Residents believe they are shown respect and properly treated; comments made during the inspection about the staff included “they are all very kind and considerate”. EVIDENCE: Residents know how to complain and feel confident that if they had concerns or complaints they will be listened to and taken seriously. The home has a complaints policy and procedure; no complaints have been received and there have been no allegations or investigations regarding the ‘safeguarding of vulnerable adults’. To ensure residents and their representatives have access to the complaints procedure it is included in the service user guide to the home and a copy is provided to each resident. All staff receive training on the safeguarding of vulnerable persons and the home has implemented associated policies and procedures. This judgement has been made using available evidence including a visit to this service. The home is very comfortable, clean, well equipped and suited to the needs of residents. EVIDENCE: Access to all areas is gained through the reception area on the ground floor of the premises. Private accommodation is selected from a variety of suites for single or shared occupancy. Each of the suites has a large en-suite shower/toilet/wash-hand basin; for a few suites the en-suite includes a bath with shower attachment. Sunrise Ops Southbourne Ltd (Reminiscence Neighbourhood) DS0000071303.V369630.R01.S.doc Version 5.2 Page 17 Each suite has additional sitting/recreational space including a “kitchenette” unit (without cooker) for the preparation of drinks and snacks. All double suites are designed so that each of the two residents can have his/her own bedroom or share the bedroom and have a lounge. Residents have the option of furnishing their own room; most of the currently accommodated residents have chosen to do and have brought into the home many personal effects to individualise their rooms and maximise their comfort. Each bedroom includes at least three double electric sockets, television and telephone point. There is a personal alarm call system with handsets and/or cords in each room and en suite facility. Each bedroom door is fitted with a key operated lock with master key override facility. There are a variety of communal rooms and areas, including the dining area, a variety of lounge areas and an open air terrace (fitted with safety glass). Decoration is in colours that are “sympathetic” for people with dementia. Each door has a name plate and a ‘memory box, as an identifying feature for the resident accommodated in that room. With the exception of the sensory/quiet room all communal areas are essentially “open-plan”, although divided up into different sections, including a kitchen/dining area, lounges and activity areas. In addition there are alcoves off the corridors which have features of interest, e.g. costumes and memorabilia from “by-gone” eras. To minimise risks of accident, all windows above ground floor level have limiters to restrict window openings to approx 6 inches and radiators are fitted with covers. All areas of the building have level floors or are accessed via two large passenger lifts or, for use by staff and in emergency, a number of stairways. There are toilets that are suitable for wheelchair users. Corridors are wide and all doors in resident areas are at least 800 mms wide, allowing ease of access to wheelchair users. There is a laundry room for use by residents assisted by staff; all laundering is carried out separately i.e. the items used by one resident are laundered separately from those used by other residents. This judgement has been made using available evidence including a visit to this service. The home employs enough staff to meet the needs of residents and to ensure their safety and comfort and maintenance of the good condition of the premises. Recruitment practices ensure the protection of residents from potentially unsuitable staff. The home promotes the achievement of nationally recognised care qualifications. EVIDENCE: The home is at all times in the charge of an experienced person and staffing levels are provided in accordance with the assessed needs of residents to ensure that at all times sufficient staff are available to properly meet their needs. All staff spoken with during the inspection were enthusiastic about their work and felt that they provided a good standard of care to residents and are properly supported by the management and training provision. Criminal Records Bureau (CRB) disclosures are obtained for all staff. The home has developed and implemented a comprehensive induction process for all new staff, designed to ensure their familiarity with all aspects of the home and a clear understanding of their responsibilities. There is an enthusiastic approach to staff training; clear records of staff training, supervision and appraisal are kept, indicating that all staff receive training appropriate to their needs. Staff are required to undertake (and as necessary update) training in core subjects including dementia care, fire safety, moving and handling, food hygiene, adult protection and emergency aid. All care staff not already in possession of a National Vocational Qualification (NVQ) in care will be enrolled on associated training within a year of employment. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. The home is properly managed, operates in the best interests of service users and protects them from risks of harm. EVIDENCE: Mrs Eileen Cockrell is the Registered Manager; she is supervised by Executive Director Ms Martin and supported by a team of care and household staff. Mrs Cockrell holds the National Vocational Qualification Level 4 in care, and has relevant experience in management. Sunrise Ops Southbourne Ltd (Reminiscence Neighbourhood) DS0000071303.V369630.R01.S.doc Version 5.2 Page 21 The provider organisation has extensive processes for quality assurance; satisfaction surveys are periodically issued and the home has developed and implemented processes of internal audit reflecting aims and outcomes for service users. With the exception of safekeeping cash for personal expenditure the home does not manage the finances of residents; residents who are unable to undertake this responsibility personally have nominated relatives, friends or other representatives to do this on their behalf. Records are kept of accidents, details of investigation and periodic audit, to ensure that risks can be identified and minimised. Staff trained in First Aid and health care are on duty in the home at all times. The use of bedrails by some residents is in accordance with assessment, care planning and relevant Health & Safety guidance. Portable bedrails are not used; only rails permanently fixed by the manufacturer to profiling beds are used. There are good processes for staff recruitment, induction, training and formal supervision. All staff are supervised and each has a personal profile containing records of appraisal ensuring that performance standards are monitored and training needs are identified, in the interests of providing good care to residents. The home has a written assessment of the ‘Health & Safety’ of the premises and working practices and keeps records of fire safety checks and tests, including drills and staff training, and has developed a fire safety assessment and escape plan making specific reference to the currently accommodated residents. To support the initial registration of the service documentation relating to the safety of the premises and equipment was submitted and included planning permission, Building Control approval of the plans, Building Control completion certificate, reports from the Environmental Health and fire officers, certificate for the electrical installations, emergency lighting, gas safety, passenger lift commissioning certificates, fire risk assessment. During the inspection a sample of records regarding equipment servicing and maintenance were examined and found to be in good order.
