This inspection was carried out on 13th December 2005.

What the care home does well

The home has a stable work force and this benefits the resident`s. Christmas decorations were being put up and staff was busy helping the residents with fluids in the lounge. Visitors were made welcome and one visitor had bought his dog in to visit also.

Accommodation may be provided for one (1) named service user under the age of 60 years. 4th July 2005 Date of last inspection Brief Description of the Service: Surrey Hills is a registered care home, one of a group of similar services run by Care Homes of Distinction, in the area. The home is a large detached premises situated in the village of Wormley, in picturesque Surrey countryside. Nursing care is provided for forty-five older service users with dementia. There are road and train links to the village and the home provides car-parking facilities. Catering is in-house. There is a staff team with nurses and carers who are supported by a wider network of health care professionals offering holistic care. . This unannounced inspection took place over three and a half hours and was the second inspection to be undertaken in the Commission for Social Care Inspection year April 2005 to March 2006. Lesley Garrett Lead Inspector for the service carried out this inspection and Patricia Grimwood Registered Manager represented the establishment. A full tour of the premises took place and the inspector looked at recruitment files, medication documentation and some policies and procedures. The inspector saw most of the residents but was unable to speak with many due to their mental frailty. There was a relative visiting who told the inspector that ‘they look after my brother very well’. The inspector would like to thank the residents, staff and manager at Surrey Hills for their time, assistance and hospitality during this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 & 10 The system for administration of medicines is adequate. The resident’s right to privacy is upheld. EVIDENCE: The inspector looked at the medication system and found the home has a medication policy but there is no local policy that is relevant to their home. The home uses a local pharmacy and the medication is delivered monthly in blister packs. The controlled drugs are checked at every handover morning and evening and the inspector sampled one resident’s and found it to be correct. All rooms have locks and shared rooms have curtains to maintain privacy during personal care. The doctor can see the resident’s in their own room or there is a clinical room with a couch and can see them in there if they are not too ill. The home has a privacy and dignity policy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Resident’s are able to exercise some control over their lives. EVIDENCE: The manager stated that residents are able to exercise control over their lives but due to their mental frailty they will often forget. During the inspection the inspector saw some resident’s in bed because they had chosen to get up later. The manager stated that some really enjoy their bed and therefore do not request to get up early. The resident’s choose which clothes they will wear that day and will sometimes select clothes that are inappropriate for the weather as evidenced in the resident survey a relative stated that a particular resident had selected a thick woolly jumper in the summer. They all have a choice of menu and this is chosen on the day otherwise they will forget what they have ordered. The inspector saw personnel possessions in the room and their doors had photographs to help them identify it. Mainly family and friends act on their behalf and some have a solicitor as power of attorney. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home has an adequate complaints system in place and they have few complaints. EVIDENCE: The manager stated that since the last inspection she had received only one complaint which had been investigated and was now resolved. The complaints policy is available for all residents and is displayed in the reception. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home now needs decorating as the paintwork and general decoration is of a poor standard and there is no evidence of regular maintenance taking place. EVIDENCE: As the inspector walked around the home it was noted that several of the rooms and all the corridors and communal areas now need decorating. A requirement has been made at the end of the report for the registered persons to look at all areas of the home and supply CSCI with an action plan detailing timescales for this refurbishment programme. The laundry room also needs refurbishment as the wall finishes are not easily cleanable and a new floor is required. The sink is also very stained and dirty. The veranda also needs to be cleared of the old furniture and the bar-b-q to allow access for any residents that may wish to use this facility. The lighting in the corridors and some bedrooms is inadequate and this also needs to be reviewed. The inspector also noted that hot water was coming from al hand basins and this is also a requirement to fit thermostatic valves. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 Recruitment practices are adequate but there was some gaps noted. EVIDENCE: The inspector checked some recruitment files and noted that all files checked had something missing. To ensure good practice it has been made a requirement at the end of the report that the registered persons look at all files to ensure they comply with current legislation. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33 35 & 38 The manager has done regular surveys for the residents. It is the homes policy not to keep pocket money accounts. Most health and safety checks have been complied with. EVIDENCE: The manager stated that twice a year surveys are sent to the residents to complete for quality assurance. Due to the mental frailty relatives are also given the surveys to complete. The manager then acts upon the comments and actions those that are necessary. During a recent management meeting the managers have been told to now do these surveys quarterly. It is the homes policy not to keep pocket money accounts. The home does have a safe and anything deposited is listed and then signed for when the items are collected. Health and safety practices are generally good. A pre-inspection questionnaire was completed for the inspection in July6 that the inspector checked.
