This inspection was carried out on 19th May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The inspector received comments from residents saying that there were `excellent staff who go the extra mile`. `The food is excellent, beautifully cooked`. `All the staff are very, very, kind and I will be coming back`. `This is a fantastic place and I am very happy and content`. The home benefits from a stable caring staff group who work as a good team. The proprietors and group matron consistently work hard to meet the National Minimum Standards and regularly request feedback from their residents and relatives to look at areas where services could be improved. The home is welcoming, comfortable and well maintained.

What has improved since the last inspection?

Since the last inspection the home has organised with their pharmacist to have residents` medicines individually pre packed before it arrives at the home. This has made it easier for staff to administer medicines to the residents. The corridors in the home have been decorated. A new care team leader has been appointed as well as a new acting manager.

What the care home could do better:

More activities should be organised within the home to occupy residents.

The home offers care for service users who require personal care; a total of 30 beds. The home has a Statement of Purpose that outlines it’s aims and objectives for the home and gives full details of the services provided; the document has been revised.Accommodation is provided in single rooms. This unannounced inspection took place with the assistance of the group matron in the absence of a manager in the home. One inspector undertook the inspection spending five hours in the home. The inspector toured the building, spoke with one member of staff and spoke with seven residents. The main method of inspection used was called ‘case tracking’ which involved selecting three residents and tracking the care that they receive and checking of their records. Five service user comment cards were received by the Commission, all confirming that their privacy and dignity was respected, three saying that they like the food, two saying that they liked it sometimes. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 4, and 5 The home provides good written information, which enables new residents to be able to make a choice about the home. Needs of residents are being met with residents commenting on the excellent staff working in the home. EVIDENCE: A resident told the inspector that he had received written information about the home which was sent to him before he moved into the home. He also said he had been able to visit for a day before coming for a trial stay. The administrator, responding to an enquiry about a room invited the new relative and their relatives to come and look around the home at any time. It was evident that residents’ needs were considered by staff to be important. One new resident said, ‘I get all the assistance I need, there is nothing else that I need or want’. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 , 10, and 11 Residents can be sure that their health care needs will be responded to. Privacy and dignity offered to residents was seen to be important by staff who gave a sensitive account of how they cared for residents at the end of their lives. EVIDENCE: A visiting GP said that staff in the home were good and offered him assistance when necessary and that they respected residents’ privacy and dignity. Each resident has a care plan which included a lot of detail and was reviewed regularly. A visiting community nurse said that staff also incorporated her plan of care into their own which benefited residents. There are safekeeping arrangements for medications in the home, with any resident who wished to look after their own medication being provided with lockable storage to keep it safe. A member of staff said that he treated residents nearing the end of their life, ‘as I would like to be treated’. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 14 and 15 More activities could be made available to occupy residents during the day. The home provides nourishing food, with a changing menu with residents saying that they were given plenty of choice. EVIDENCE: The home is currently without an entertainments officer, but the group matron was interviewing a candidate on the day of the inspection. One resident was playing cards at a table with large print playing cards and a jigsaw was partially completed. Large print books were also available. One resident said that they ‘would like activities in the afternoon’. Church visitors regularly attend the home with some residents preferring to take communion in the privacy of their own rooms. One resident expressed his gratitude to the group matron for her support, saying that she ‘was wonderful’ and had assisted him to retain control over his life. Residents said they had enjoyed their food and fresh fruit and snacks were provided at coffee time. One resident said he had had a cooked breakfast, going on to say, ‘you can have as much as you want and have what you want with plenty of choice’. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17 and 18 Complaints and adult protection issues are treated seriously by the home and steps are taken to ensure that residents know how to complain and that staff know how to respond effectively to protect residents from abuse. EVIDENCE: The home has a complaints procedure, but has received no complaints since the last inspection, nor did the inspector receive any complaints. The complaints procedure is given to all residents. A member of staff confirmed that they had received training on how to prevent adult abuse and how to report it. Residents were taken to the Polling Station to vote at the recent inspection. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 22, 24, 25 and 26 Residents’ benefit from a well maintained home, which is cleaned to a high standard and is comfortable and welcoming. EVIDENCE: The home was found to be well maintained, cleaned to a high standard and was odour free throughout. Residents are able to bring their own possessions into their rooms to make them more personal, and both communal and individual’s rooms were comfortable and pleasant. During the summer months residents are able to sit in the garden which is well maintained. There are arrangements in place to prevent the spread of infection in the home. A member of staff confirmed that there is enough equipment in the home and on the day of the inspection, the hoists were being serviced to ensure they were safe following an alert on particular hoists. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28, 29 and 30 There is a good staff team in the home, many of them having worked in the home for many years which provides consistency of care for residents living in the home. Staff are well trained and recruited safely ensuring a more confident and competent workforce. EVIDENCE: There is a stable workforce in the home who were seen to work together as part of a team. One member of staff said that they ‘enjoy working as part of a team’. Staff recruitment files were checked and showed that staff had been recruited safely. A list of training was posted on the wall for the forthcoming year which was comprehensive and included manual handling, health and safety and first aid. 50 of the care staff in the home have achieved National Vocational Training Awards at level 2. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,35 and 38 Staff and residents work and live in a safe environment. EVIDENCE: The group matron, who has changed some work practices in the home, is currently running the home in the absence of a manager. The pre-inspection information provided by the home shows that safety checks have been undertaken on equipment in the home and that fire drills are undertaken regularly and fire bells checked on a weekly basis. There are safe arrangements regarding service users’ financial interests.
