Each resident accesses a day service, college placement or work experience. The home offers opportunities for residents to develop their independent living skills, in promoting community participation and involvement. The home is furnished and decorated to a high standard and offers good quality and comfortable accommodation. Communal areas are substantial and homely and there is a large garden at the back of the house. All residents have good size single bedrooms and there are two fully equipped bathrooms and a shower room. This unannounced inspection took place between 2:15pm and 6:30pm and was carried out as part of the annual plan of inspection. Planning for this inspection included reviewing the previous inspection report, the pre-inspection questionnaire and the comment cards from residents and relatives. During the inspection, residents showed the inspector their bedrooms and care records were inspected. Three care files were viewed. The inspector spoke to and observed the practice of staff and spoke to most of the residents living in the home. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. JUDGEMENT – we looked at outcomes for standard(s) 2, 4 Prospective residents’ needs and aspirations are fully assessed therefore their care needs can be met once they move into the home. EVIDENCE: • • • • • Three residents’ files were viewed and all had a full assessment completed by a social worker in consultation with relatives and carers. Care plans meet the needs and aspirations assessed. Records indicate that a range of professionals are involved in the care of individual residents. Residents sign agreements for assessments and care plans. A member of staff and the resident concerned spoke about the phased introduction he was given as a prospective resident. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9 Good systems are in place to ensure residents are consulted in all aspects of their lives in order for them to achieve independent lifestyles. EVIDENCE: • • • • • • • Staff the inspector spoke to were knowledgeable about the care and support each resident required. Staff were competent in their knowledge and understanding about how each person communicates. Care plans were comprehensive but needed evaluating. Staff were observed offering choices to residents, discussing events of the day and planning activities. Residents had comprehensive risk assessments that are regularly reviewed. Residents told the inspector that they have regular house meetings where they discuss holidays, trips, meals, household routines etc. A resident spoke about a recent event in their life where the staff in the home supported them to use an advocate to enable them to arrive at a positive conclusion. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15, 16, 17 Social activities and meals are both well managed, creative and provide daily variation and interest for people living in the home. EVIDENCE: • • • • • • Residents attend day services and colleges. Staff provide numerous, well organised formal and informal leisure activities. During the inspection, most residents took part in a planned game of bingo. Staff were observed interacting with residents positively, with respect and upholding their dignity. A mealtime was observed. Residents were offered a choice of meals. The mealtime was relaxed and informal. A resident told the inspector about the holiday they have recently been on and another resident spoke about a trip to Disneyland, Paris. Photos on the walls showed the residents at Disneyland. Staff encourage and facilitate family/friends contact. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20 There are good levels of personal and healthcare support to residents. However, there were minor shortfalls in some areas relating to the safe handling of medication and these have the potential to place residents at risk. EVIDENCE: • • • Personal support is clearly detailed in the care plans. All residents are registered with a GP and have access to the appropriate healthcare professionals as required. Generally, medication was received, stored, handled and disposed of appropriately. However, the home uses a number of homely remedies and should seek the advice of a GP or have the medication prescribed. The home keep a record of medication returned to the pharmacist but this has not been signed by staff or a pharmacist. A list of staff who can give medication and their signatures needs updating. Syrups and creams should be dated when opened. The home should consider ways of eliminating the need for sub-dispensing. The times medication is given should be clearly stated on the medication administration record. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23 The inspector was satisfied that the home responds to complaints and concerns effectively and that residents are safe from abuse or neglect. EVIDENCE: • • The residents living at The Briars are supported to make their views known and to raise issues as and when they happen. This was evident from discussions with residents. The Registered Manager/Owner stated that she has received the new guidance for vulnerable adults procedures and is planning a training session for staff. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25, 26, 27, 28, 29, 30 The Briars is homely, clean, safe and comfortable. EVIDENCE: • • • • • The home was clean and hygienic on the day of this unannounced inspection. All rooms are single. Four residents showed the inspector their bedrooms, which were highly personalised. The inspector observed a number of adaptations to promote independence such as a bathing seat and grab rails. The home has an extremely comfortable lounge that has large windows and a high ceiling, and the service users enjoy gathering in this room, on return from their daily activities. The lounge is fully carpeted, contains large comfortable armchairs and sofas, and is clearly a central point for the residents to meet up and socialise. There is a light and pleasant conservatory area where service users can read, use computers, and listen to music. There is a good patio area around the back of the house and a well maintained garden with a lawn and a garden shed. JUDGEMENT – we looked at outcomes for standard(s) 33 The deployment and number of staff is sufficient to meet the needs of the residents. EVIDENCE: • • At the time of this unannounced inspection, there were four staff on duty when all the residents were at home. This included a tutor, who was running a bingo session and the Registered Manager/Owner. The Registered Manager/Owner stated that staff are deployed to suit the lives of the residents. JUDGEMENT – we looked at outcomes for standard(s) 39, 42 The staff team benefits from good management, leadership and direction from the Registered Person, which ensures residents receive consistent, quality care. EVIDENCE: • • It was evident at this Inspection, through conversations with residents, staff and the Manager that feedback about how the home is operating is sought and discussed on a regular basis.
