Conditions of registration: 1. The Home is registered to provide personal care for service users of both sexes whose primary needs fall within the following categories:Old age, not falling within any other category (OP) (11) 18th May 2005 Date of last inspection Brief Description of the Service: St Oswalds is situated close to the centre of the market town of Alford. Service users confirmed that all the towns amenities are within walking distance of the home. The property is on a recognised public transport route. The building is a detached two-storey property that is sited in a predominantly residential part of the town. The front garden is paved to make it accessible to service users. There is a garden to the rear with car parking spaces for two cars. Accommodation for service users is on two floors. Access to the first floor is by stair lift. There is a lounge and a lounge-cum-dining room on the ground floor. The home is registered to accommodate 11 older people in 9 single and 1 double room. This is an overview of what the inspector found during the inspection. The inspection took place over 1 morning and involved speaking with all the residents, the manager and the staff as well as a tour of the home and a review of a sample of the records. What the service does well: What has improved since the last inspection?
The records kept at the home have continued to improve. The upstairs bathroom has been upgraded to a high standard and two new residents bedrooms are decorated and furnished. Staff are now formally supervised and these sessions are recorded. The home now has a protocol for staff when they are giving medication ‘as required’. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 5 The home is welcoming for prospective residents and enables them to make an informed choice. EVIDENCE: One resident moved to the home in the last few weeks and they and their family were able to visit before the move. The manager and staff are happy to show prospective residents around the home and to answer their questions about the facilities and routines. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The home does administer medication effectively and safely, taking into account the needs of each resident. EVIDENCE: 3 residents are responsible for administering their own medication and they have secure facilities for doing this safely. The pharmacist recorded that they were satisfied with the procedure at their last inspection. Risk assessments have been completed. The manager felt that this was an improvement in the resident’s independence and all 3 of the residents agreed that they valued this opportunity. This is kept under review and recorded. The staff now have clear guidelines in the medication file for administering medication ‘as required’. The file also contained information sheets for every medication kept at the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): All of the standards were assessed at the previous inspection and there have not been any significant changes. The residents continue to be extremely satisfied with the care at the home and with the food that is provided. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17 The home protects the resident’s legal rights and has all the procedures in place to deal with any complaints. EVIDENCE: The home has a complaints policy and the 4 residents asked were confident that they could raise any issues with the staff and manager. The complaints policy is included in the service user guide. Minutes form the residents meetings show that issues are raised and dealt with satisfactorily. There is a document where any complaints could be recorded but none had been received. The manager is aware of the advocacy service and they have previously contacted a solicitor for 1 resident. Residents can meet their legal representatives in private. The home offers all the residents the opportunity to vote and they are willing to accompany residents to the polling stations or to arrange postal voting. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,21,23 The home provides suitable toilet and washing facilities and the residents bedrooms are comfortable, warm and individual. The upstairs windows require restrictors to limit their opening. EVIDENCE: During a tour of the home the inspector saw that there were adequate toilet and washing facilities for all the residents and these are of a clean and comfortable standard. The upstairs bathroom has been redeveloped since the last inspection and it now offers a comfortable and pleasant area for bathing. The toilets and bathrooms are clean and fresh with no odour. A sample of the residents bedrooms were seen and they are personalised and comfortable. The manager stated that there are 5 left to decorate but the residents are happy with them at the moment and considering the number of improvements already made they will wait until next year. It was clear that residents had brought their own belongings to the home and they said that they like their own rooms.

There is a risk assessment in place and although risk assessed as low risk the inspector discussed with the manager the need for restrictors whilst still allowing adequate ventilation. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 The manager ensures staff are competent and that the recruitment procedures are robust. EVIDENCE: There are staff in sufficient numbers to meet the needs of the current residents and the inspector saw the staff rota and that there were enough staff on duty. The manager has a robust recruitment procedure, which involves the checking of application forms and references and a formal interview. Staff files were completed and contained police checks. The staff are undertaking training and the manager is on call when not at the home. Recent training has included Fire training, Health and safety and first aid. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37 The home does protect the resident best interests and safeguards the information they hold about each person. EVIDENCE: The records that were seen were well organised and they included the maintenance of the home and risk assessments for each area that the residents have access to. The records are stored securely and the residents know they can see their records. This is stated in the service users guide but the manager also said they would discuss this with each new resident.
