Conditions of registration: 1) The home is registered for one person in the Category PD as named in the Notice of Proposal to Register dated 20 October 2004 2) The home is registered for one person in the Category PD as named in the Notice of Proposal to Register dated 16 March 2005 Date of last inspection 02 March 2005 Brief Description of the Service: The Fountains Care Home is registered with the Commission for Social Care Inspection to provide nursing care and personal care to people over the age of 65 years and an individual with physical disabilities. The property is located in the village of Tetney in North East Lincolnshire, lying between Louth and Grimsby. It is a large detached building, standing in its own grounds and gardens. Car parking is to the side of the house. Accommodation is provided in mainly single rooms on the ground and first floor. Although there are two double rooms, these are kept for married couples or service users who specifically wish to share. There is a passenger lift to access the first floor. The homes brochure states that ‘we are dedicated to providing you with the best care along with courteous, understanding and friendly attention to enable you to enjoy a high quality of life’. This is an overview of what the inspector found during the inspection. The inspection was undertaken by two inspectors and took place on one day in May 2005. The inspection included interviews with the current manager; service users; two visitors; members of staff; a tour of the premises; examination of care records and incorporated an investigation into a complaint regarding the care of service users at the home that had been received by the Commission. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,5 Service users and their relatives are enabled to make an informed choice about where they live. EVIDENCE: The home has a detailed Service User Guide and both service users and relatives confirmed that they had received information about the home both from brochures and word of mouth; which had aided their choice. Three relatives confirmed that they had had opportunity to visit the home prior to the service users admission. The manager confirmed that service users and their relatives are encouraged to visit prior to taking up residence at the home. The home does not provide intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The home ensures that all service users are treated with dignity and respect. Service users health care needs are fully met. EVIDENCE: Care records examined were comprehensive and included individual care plans. Staff said that service users are informed that they can access their care records at any time and that relatives can access the records with the service users permission. Service users are involved in the updating and review of their care records. Service users comments, about the care they receive is very positive; and members of staff were aware of each individual’s care needs. Members of staff were seen knocking on doors before entering and delivering personal care with courtesy and sensitivity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 Service users benefit from a variety of activities and are enabled to exercise choice and control over their lives and receive a wholesome appealing balanced diet. EVIDENCE: The home has an activities coordinator, who provides a variety of activities for residents; singalongs, carpet bowls, outings to the Wintergardens. Clergy visit the home and offer services to those who wish to attend. The activities programme is depicted in pictorial form on the notice board, to ensure that all residents are kept informed. Service users said how they get up and go to bed when they want to, “there’s no feeling you are a criminal, if you are late for breakfast” Members of staff confirmed that service users are always asked individually to choose the gender of the member of staff, providing their personal care. Members of staff and relatives confirmed that they are able to visit when they like to and eat with the residents if they wish. The manager did point out, that visitors, particularly professional visitors are encouraged to avoid meal times to minimise interruptions to the social part of the meal time.

The home’s menus identified choices and special or soft diets were available for those unable to eat a regular diet. Service users are encouraged to eat independently and socialise at meal times. Members of staff assisted those service users, who were unable to eat independently. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18 Service users are confident that their concerns will be listened to, taken seriously and acted upon. Service users are protected from abuse and exploitation by staff who have received appropriate training. EVIDENCE: The home has a complaints policy and procedure that was available for examination. Members of staff were clear as to whom they should speak if a resident or a relative raised concern. Service users and relatives spoken to on the day of the inspection had no complaints or concerns regarding the care they received, members of staff or the running of the home. One service user said that they “would go to the matron if upset, or mention to the nurse, matron meets everyone at breakfast; everyone keeps coming back, as good a recommendation as you need” The commission had received one complaint since the last inspection; an investigation took place as part of the inspection. The manager and staff members assisted the investigation. On conclusion of the investigation the concerns were not upheld. Service users are encouraged and assisted to exercise their right to vote during local and national elections. Members of staff have attended adult protection training. Members of staff said that they would have no hesitation in reporting any allegation of abuse of residents or any observations of poor practice. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 22,24,26 Service users live in a safe, well- maintained, clean and comfortable environment. EVIDENCE: The inspection included a tour of the premises. Adaptations and aids were in use within the home; service documentation was up to date. Individual rooms were comfortably furnished, well-decorated, with individual service users’ possessions around them. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,30 Service users needs are met by sufficient staff, who are trained and competent to do their jobs. EVIDENCE: The home is currently fully staffed, the manager said that there is very little turn over of staff within the home; this was confirmed by members of staff and by examination of the staff rosters. The home does not use agency staff. The dependency levels, is worked out regularly, by the matron to ensure that staffing levels are adequate to meet the changing needs of the residents. Members of staff have access to regular training, and those members of care staff who were spoken to, had gained NVQ qualifications. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,36 The home is well run by a manager who is well liked and respected by service users and members of her staff team EVIDENCE: Members of staff were very positive about the manager, some of whom, moved with her to The Fountains from her previous post. “Matron is a lovely boss, absolutely brilliant; pushes us to do training. Best boss I have ever had, fair, firm when needed; good teamwork”. Another commented: “like a second family, good team, nice atmosphere, support each other” “matron will always make herself available to talk to you, will listen and put things into action if needed”. Members of staff said that they receive supervision every two months. The manager was seen to speak to service users, relatives and members of staff around the home; relationships were good, humour was positive. The manager is due to retire in the next few weeks; several of the service users and members of staff discussed the manager’s imminent retirement with significant regret whilst wishing her well.
