This inspection was carried out on 11th October 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The administrative systems and structure are generally good and information can be accessed easily. Staffing levels provided are appropriate and staff training opportunities are good.

What has improved since the last inspection?

The home has had new carpets downstairs and in some of the residents` bedrooms. The front entrance has been completely revamped as have the two office spaces. New light fittings have been fitted. The Service User Information Guide has been further developed to include information that was required in the last inspection report dated 07/02/05.

What the care home could do better:

The needs assessments need a little more attention to detail. The registered person must enable the deputy manager and senior care workers on each shift period to access information that they may be required to provide for inspection.

The property was originally a farm, which has been converted into a care home. Residents’ bedrooms are located on the ground and first floor and are accessed by a shaft passenger lift. There are 26 single bedrooms and 16 have en-suite facilities and one double bedroom that is currently used as a single. There is an attractive garden area with garden furniture. A wide range of health services are available including a choice of General Practitioner, district nurse, chiropody, dentist and optician. Staff training takes place to inform and enable staff to care for service users appropriately. A programme of leisure activities and social events is in place. This was an unannounced visit that took place at the home over a four hour period additional time was spent in preparation for the visit, looking at previous reports and other documents. During the inspection process a number of documents were examined, including residents’ care files, staff files and records, time was spent looking around the building and speaking to a number of residents. The inspector spent a specific amount of the inspection concentrating on the care arrangements in place for two residents, the recruitment of staff process and the areas identified in the last inspection report dated 07/02/05 as in need of development or implementation. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1.2.3.5. The home has a Statement of Purpose and Residents’ information Guide in place. The home offers trial periods to potential residents. EVIDENCE: The Statement of Propose and residents’ Information Guide’ were examined and found to be detailed and informative. Needs assessments had been completed in place prior to the resident being offered permanent residency. The deputy manager and senior care worker carry out all new assessments. The Terms and Conditions of residency were examined and found to be satisfactory. Two residents’ files were examined and both had areas in the need assessments that had not been completed one of these areas was to identify previous interests and hobbies. The home maintains a respite care book, trial periods of residency offered to potential residents are recorded in this book, and several instances of residents having respite care have also led to permanent places being offered at a later date. Intermediate care is not offered at this home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7.8.11. Care plans are in place and a wide range of health services are available to residents. EVIDENCE: Two residents’ files were examined the Individual Care Plans were comprehensive and informative; the manager stated that residents/relatives are involved in the care plan process when appropriate. Residents are able to access dental care, eye and hearing tests as and when required. The home has regular visits from district nurses. All the residents have a choice of general practitioner. There is a policy in place addressing the needs of a resident who id dying. Friends and relatives can visit at any time, and if a room was available they could remain at the home. Appropriate nursing care would be sought if needed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16.17.18. There is a Complaints policy displayed, residents can access advocacy or legal assistance, and a policy for ‘The Protection of Vulnerable Adults’ is in place. EVIDENCE: The Complaints policy was seen and found to be satisfactory, however the complaints/compliments record book was not available for inspection. Residents retain their right to vote, and advocates from ‘Aware’ are used when residents need some independent assistance. The home has a policy in place for ‘The Protection of Vulnerable Adults’ it made no reference to Derby and Derbyshire Local Authority Social Services procedures. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27.28.29.30. Staffing levels are appropriate to meet the assessed needs of residents; staff are given good training opportunities. The home adheres to its own recruitment policy. EVIDENCE: Staff rotas were examined and found to be satisfactory; the staffing levels provided were appropriate for the identified needs of residents. Staff have the opportunity to attend a range of training including National Vocational Qualification (NVQ) six care staff have already achieved NVQ level 2 a further three members of staff have achieved NVQ level 3 the proprietor and another member of staff have achieved NVQ level 4. A further eight members of staff are commencing NVQ training in Jan 2006. Two staff files were examined both were found to contain the required documents as indicated in Schedule 2 of the National Minimum Standards. Staff are supplied with contracts of employment.
