CSCI found this care home to be providing an Adequate service.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

People are being provided with care plans and risk assessments that are up to date and reviewed when needed.

What the care home could do better:

The care plans should be more person centred care in their approach. Codes used to describe why medication administration has not taken place should be clearly defined so detailed information on a person`s medical treatment can be obtained.

Not to exceed 3 service users in total for the categories DE and DE(E) Date of last inspection Brief description of the care home The Gables Nursing Home is an extended converted building that provides personal care with nursing for both men and women over 65 years. It is a three-storey building, but people living at the home have access to only two of these, the third storey is used for storage and office space. The home is situated on a busy main road with good Care Homes for Older People Page 2 of 9 access to public transport for Leeds and Bradford. Facilities nearby include a public house, shops, a post office, a cricket ground and a GP surgery in the grounds of the home. The majority of bedrooms are single but there are some shared rooms. Some rooms have en-suite facilities, the majority of which have limited access. Lounges and the dining room are on the ground floor; the garden can be accessed from one lounge by a portable ramp. On the 22nd April 2008 the manager said that the weekly fees ranged from #440 - #545 per week. Additional charges are made for newspapers, hairdressing, chiropody and personal toiletries. Care Homes for Older People Page 3 of 9 What we found: We looked at three care plans. These showed evidence that there has been improvement with the provision of care planning in the home. All three had up to date care plans that were reviewed monthly. Evidence was seen in the care plan documentation that outside professionals are consulted regarding care need issues. This was particularly relevant to wound assessment and planning. The dietitian were also involved when needed. Only one care plan seen showed evidence that the person receiving the care package was involved with the consent process. No care plans seen showed evidence of a person centred care approach being adopted. This is good practice for people with specialist dementia needs and provides staff with a better understanding of individuals needs. Improvements have been made to the recording, administration and storage of medicines. The current and previous months Medication Administration Records (MARs) were looked at. Evidence from the current MARs showed that medication is being given as prescribed. Antibiotic courses on the new MAR had signatures of administration that matched the quantity supplied. There was evidence on the current MARs that medication was in stock and available to administer from. One person has paracetamol prescribed as when required for pain relief. Records of administration made on the previous months MAR demonstrated that the person was given the paracetamol at times when needed rather than at the usual administration times. This is good practice as it means that staff have identified the persons needs,however the current MAR only had records made against the usual medication rounds. The code O was used on a number of occasions to record no administration. However there was no definition on the chart to explain why the person had not received their medication. This means that there is not a clear reason available to know how a person is taking their medication. The prescriber, who may wish to review the medication, may also use this information. A cross check of medication administration against medication remaining showed that medication was being given as prescribed. The fridge for storing medication has been moved to the medicines room and was locked on the day of the inspection. This will make sure that people are aware of the services that the home provides. These must be focused on the specialist dementia needs of those people living there. This will help protect people from harm. 5 19 23 The home must continue with the refurbishment programme.
