Conditions of registration: 1. 2 Day Care Date of last inspection 19th July 2005 Brief Description of the Service: The Green Care Home is situated within the village of Hasland, close to the centre of Hasland, which has numerous shops, park and a church. The home provides nursing and residential care for 40 service users. The home is purpose built and is fully accessible. There are two lounges, the smaller of which is designated as a quiet lounge. There is also a separate dining room. Outside there is a well maintained garden with a seating area for service users. This is an overview of what the inspector found during the inspection. Only a limited number of the National Minimum Standards were examined at this inspection (with emphasis on several of the ‘key standards’), and the previous requirements. The person present at the inspection was: Mrs C Innes, Manager. Within this inspection, which occurred over a three hour period, the inspector toured the building relating to the previous requirements, spoke with service users, and staff and examined some documentation. Within the Conditions of Registration, section of the report, it identifies that there are 2 Day Care. These places formed part of the registration under the Registered Homes Act 1984. The Commission for Social Care Inspection does not register Day care. Therefore the condition will be removed. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 3. Accurate assessments will ensure that the home has sufficient information to be aware of the service user’s needs prior to admission. EVIDENCE: The home received service users’ assessments from the Social Services Care managers prior to the admission. The manager informed the inspector that the home only admitted planned admission, not emergency admissions. The manager or senior staff prior to admission to the home assessed all service users. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): EVIDENCE: All key standards were assessed at the previous inspection. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13, and 14. Various activities were organised within the home, which would provide stimulation to service users and enhance their quality of life. EVIDENCE: The manager advised the inspector that the activities were co-ordinated by the care staff. She also informed the inspector that the activities included progressive mobility, which included movement to music, and bingo, which was undertaken by external volunteers. A carol service as well as an evening church service was planned. Service users were able to access the local village, being accompanied by staff or their relative or visitor. There was no restriction on visiting and visitors were able to visit in the communal areas or the service user’s own room, should the service user wish. The inspector discussed the provision of activities with 4 service users. He was informed that they were satisfied with the provision and were looking forward to Christmas. The service users had an alternative choice of meal.

The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 16 and 18. As far as could be established the home took complaints seriously and acted upon the issues. The staff received training, on Adult Protection, which if implemented should make them more aware of Adult Protection issues and how to act should an allegation occur. EVIDENCE: The complaints procedure was displayed, and contained all the relevant information. The complaint book was examined and no complaints were outstanding. No one during the inspection asked to see the inspector to voice any complaints. The inspector received only positive comments regarding the home. Regarding Adult Protection, it was established by the examination of the training records that staff had attended adult protection training. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 19. The environment, monitored at this inspection, had not been maintained to the required standard to provide a safe, well-maintained environment for service users, as the requirement relating to Room 12A still required attention. EVIDENCE: The key standards were assessed on the previous inspection. The previous requirements were assessed. All had received attention except for the redecoration within room 12A, and the provision of door locks. Therefore the requirement relating to décor was repeated. The manager was able to provide evidence that the service users had been offered the choice of a lock. The inspector will monitor this at the next inspection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 29 and 30. The necessary documentation designed to protect the service users were in the staff files. Staff had received moving and handling, fire and other training. An up to date workforce will contribute to the delivery of care. EVIDENCE: On examination of the staff files the inspector established that the files contained the required information as detailed within Schedule 2 and 4. On examination of the training records relating to Moving and Handling and Fire, the records indicated that all staff had received both Moving and Handling and Fire training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31,33,35, and 38. The management of the home does comply with the requirements of the Care Homes Act. Quality assurance systems will assist the managers to measure the home against expected outcomes. EVIDENCE: A registered manager was in post. She had undertaken the necessary training regarding moving and handling, fire etc. The home had basic quality assurance systems, which were implemented by the manager. The home was achieving almost full compliance to the audit tool. The inspector discussed the benefits of producing a more critical analysis tool, which the home could strive to achieve. Green, The Nursing Home (Hasland) DS0000002084.V267754.R01.S.doc Version 5.0 Page 15 On examination of the staff supervision records, the inspector found evidence that supervision had occurred and was on target to achieve the six times a year supervision for each member of staff. Regarding the service users’ personal monies the home operated a basic credit and debit system. Regulation 26 visit occurred and there was documentary evidence to support this. As far as could be established there were no health and safety issues except if any were raised with the previous sections of the report.
