Date of last inspection 2nd February 2005 The Limes C51 S32890 The Limes V233227 150605.doc Version 1.30 Page 5 Brief Description of the Service: The Limes is a care home providing personal care and accommodation for forty older persons, incorported within the home are eight beds for the purpose of respite care for individuals with Dementia. The forty single bedrooms are without ensuite facilities. The home has a central coutyard which is well maintained and which is accessible to all residents residing in the home. The premise is owned by Leicestershire County Council Social Services Department and is situated close to Hinckley town centre where residents have access to shops, pubs, post office and other amenities. The home is easily accessible by private or public transport. Accommodaiton is provided over two floors with access between the floors being via stairs or a passenger lift. The Limes is fully accessible, and communal areas are provided on both floors of the home. The Limes C51 S32890 The Limes V233227 150605.doc Version 1.30 Page 6 SUMMARY
This is an overview of what the inspector found during the inspection. This Inspection took place between 10.00am and 4.00pm. Opportunity was taken to look around the home, view records and policies which included viewing the care plans of four residents residing at the home and two individuals accessing the specialist respite service. The Inspector spoke with residents and staff. The Registered Manager facilitated the Inspection, spending time with the Inspector and discussing the management of the home. What the service does well: What has improved since the last inspection?
The Limes has appointed an Activity Organiser who has arranged a number of internal and external activities, and is recording the views of residents as to activities to which they would like to participate. The system for care plans and communication records has been revised, ensuring accessibility of information and direct links of responsibility with the individual members of care staff and the management team. The lounge and dining area is in the process of being reburbished. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 4, and 5. Standard 6 is not applicable. The admission process is well managed with residents needs being assessed ensuring staff have the appropriate information to meet individual needs. EVIDENCE: There is a comprehensive Statement of Purpose produced as booklets, one for those residents accessing long-term care, and one for individuals accessing the respite facility for those with Dementia. The admission procedure is adequate in that assessments of individuals are carried out by a Social Worker as part of the referral process. Prospective Residents and relatives are encouraged to visit to view the home prior to admission. As part of the assessment process residents and relatives are asked to provide a history of an individual’s life prior to admission, these were in place but could be more detailed, which would help in the delivery of care and activities provided.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10 and 11. Residents are looked after well in respect of their health and personal care needs and are involved in the drawing of their care plans. EVIDENCE: The care plans of four residents were viewed, along with the care plans of two individuals accessing respite, these reflected residents’ specific needs and contact with other health care professionals. Residents and or their relatives have signed their care plans, in all but one of the files viewed. Records recording residents daily lives reflected their plan of care, contact with health care professionals and relatives and friends. Care plans detailed mental health needs where appropriate, consideration could be given to elaborating on how a persons needs are to be met, consistent with training staff have received on mental health and dementia. A newly appointed Activity Organiser is currently collating the views of residents to establish their social needs, reference to peoples social events are briefly detailed in some care plans.
The Limes C51 S32890 The Limes V233227 150605.doc Version 1.30 Page 11 Residents were asked as to their views as to the care they receive, the following comments were made. “Nothing is too much trouble.” “I like living here.” “The staff are really nice.” “It’s like a four star hotel.” Four relative and twelve resident comment cards were completed, responses to questions were positive, with regards to care, staff and living in the home. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14. Residents experience a homely life style and visitors are encouraged to visit. Various formal and informal activities are made available to residents. EVIDENCE: The home has recently appointed an Activities Organiser, who is employed to work four hours, five days a week. Currently the Activity Organiser on an individual basis is discussing the views of residents as to activities they wish to participate in, their views are being recorded. An activity programme has now been formulated based on the individual and collective needs of residents. Activities within the home are varied and include movement and music, quizzes, jigsaws and board games. A trip to Coombe Abbey has been planned for June along with pub lunch in Osbaston. A family evening has been planned for July and a Summer Fayre and a trip to Twycross Zoo organised for August. Resident comments cards, completed in April 2005, indicated that some residents found there were insufficient activities, a majority however indicated they were happy with activities within the home. The Activity Organiser is recording the participation of residents in activities in order to evaluate the success of organised events.

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17 and 18. Complaints are handled objectively and residents are confident that their concerns would be listed to, taken seriously and acted upon. The home has a copy of the vulnerable adults procedure to ensure a proper response to any suspicion or allegation of abuse. EVIDENCE: The home has a detailed complaints procedure and complaint record. Neither the home nor the Commission for Social Care Inspection has received any complaints since the previous inspection. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25 and 26. On the whole a comfortable and safe standard of accommodation is provided for the resident’s, some areas do require attention. An outside courtyard is accessible to residents. EVIDENCE: The Limes is generally well maintained and is suitable to the needs of residents, it is decorated and furnished to a standard that creates a comfortable and homely environment with some areas however requiring attention. Communal areas are located on the ground and first floor; the home provides sufficient lavatories and bathing/shower facilities to meet the needs of residents. Residents have access to equipment such as hoists to assist them and staff in the delivery of personal care. The Limes C51 S32890 The Limes V233227 150605.doc Version 1.30 Page 15 The bedrooms of seven residents were viewed and in general rooms were in good decorative order, one bedroom does require decorating. , Residents were asked as to their view of their individual bedrooms and the communal areas; all expressed satisfaction with the accommodation provided, but were keen for the refurbishment of the lounge and dining area to be completed. The Limes has a central courtyard with seating, potted plants and a gazebo, the home is surrounded by an open planned grass area, in the opinion of the Inspector, residents and individuals accessing the respite facility would benefit from having an grass area which they could access safely and independently. The pre-inspection questionnaire completed by the Registered Manager details the regular maintenance checks undertaken within the home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30. Staff at the home are well trained and supported, and employed in sufficient numbers to meet the needs of residents. Recruiting procedures are robust, with checks in place for the protection of residents. EVIDENCE: Staff rotas indicated good levels of staffing which were sufficient to meet the needs of residents. The Limes senior staff team consists of the Registered Manager, Deputy Manager and two Assistant Managers. The home employs twenty-three care staff that are supported by thirteen ancillary staff. The preinspection questionnaire completed by the Registered Managers states that thirteen staff have attained a National Vocational Qualification at level 2. Staff training records reflected a variety of topics of training accessed, which include health and safety, adult protection, care practices and specific training in mental health. The Registered Manager confirmed that the recording of attendance of staff on training was currently being reviewed. One member of staff spoken to said that the one-day training in mental health/dementia was very enjoyable, and directly corresponded with the care needs of residents in her care. Records of two members of staff recruited since the most recent Inspection were viewed and found to contain all relevant documentation including Criminal Record Bureau checks.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36, 37 and 38. The Registered Manager offers a clear sense of leadership, which reflects on the day-to-day delivery of care practices of residents and running of the home. EVIDENCE: The Registered Manager has attained a National Vocational Qualification in Care at level 4, and is currently working towards the Registered Managers Award. A member of staff spoken to stated she receives one to one supervision session with a member of the management team, these take place on a regular basis and are used to discuss training needs, changing needs of residents and any areas of concern. Records of tests to fire safety equipment were in good order and health and safety issues were well documented. A tour of the environment noted that banisters of stairs were low in height, and in the opinion of the Inspector represent a potential risk to the safety of residents. A risk assessment should be undertaken to establish the risk, if any, this may pose to residents, and any risks identified acted upon.
