The home was originally a large detached family home, and it retains many of its original features. There is a large lawn at the front of the home which provides a good sitting area for residents, with smaller gardens at the side and rear. There is a car park at the front of the home. If nursing care is required it is provided by members of the District Nursing team. Any medical or specialist services are obtained via the GP. The home is situated on the outskirts of the village of Ashill, on the Swaffham to Watton Road (B1077). The fees charged at this home range from £393.00 - £450.00 per week. We carried out this random inspection after a safeguarding investigation into concerns about staffing levels and lack of appropriate care for a person with diabetes. During this investigation, carried out by Norfolk adult social services, it had been found that there were often only two care staff on duty at weekend, which was insufficient to meet the needs of the people using the service. There was also an outstanding requirement to improve staffing levels that had not been met at the last key inspection. This random inspection was carried out over 2 hours by one inspector. We spoke with staff and residents and looked at records. At the time of the visit there were 18 residents in the home. We looked at four, weekly staffing rosters. We found that there were always three staff on duty at all times during the waking day and two staff at night. In addition, there was a cook and a cleaner, which meant that staff could concentrate on care issues instead of housekeeping. The manager was not included in the numbers and generally worked Monday to Friday. The rotas showed that cover was arranged for sickness and holidays. The manager and staff confirmed that three staff were enough to meet the current needs of the residents. Staff told us that having the extra member of staff at weekend had made a difference and meant they could spend time talking to residents. The manager also told us that she would be able to increase the staffing levels in the future should this be needed. The residents we spoke to were unable to tell us whether they thought there were enough staff, although one said he did not have to wait very long if he needed anything. We looked at two care plans for residents who had specific, ongoing health care needs. A person who had diabetes had a care plan in place. The district nurses had advised that the residents blood sugar levels should be maintained slightly high in order to prevent other complications, but this was not explicit in the care plan. The care plan did not specify what was considered to be an acceptable range for this resident and all the staff varied slightly as to what they thought was too high or too low. Despite this, the staff were very clear about how to recognise if the resident was having a hypo or hyper glycaemic attack and knew what to do to provide emergency aid. All senior staff had certificates following recent training from the district nurse. The training was to administer insulin and had also included some theory and practice in associated topics.
