The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The Lodge is decorated and furnished to a very high standard throughout, enabling residents to relax in a range of communal areas including their individual bedrooms. The garden is accessible to all, and is well maintained. Care plans and records pertaining to residents are of a high standard, with the views of residents incorporated. The views of residents are paramount to the running of the home, and the delivery of care. Records pertaining to staff, including training and recruitment are of a very high standard, as are the records, which detail the maintenance of the home. Residents are in receipt of a variety of recreational and leisure activities both within and outside of the home. Residents maintain a high level of contact with residents and friends.

What has improved since the last inspection?

A regime for the cleaning of the home consistent with infection control has been implemented, with records kept; staff have received training in Infection Control to support this. Additional staff have been recruited to ensure senior staff are on duty in the evenings.

What the care home could do better:

The involvement of residents in activities, leisure and recreational pursuits could be recorded to evidence more easily their involvement, as part of their care plan.

Date of last inspection 15th November 2004 Brief Description of the Service: The Lodge is a care home providing personal care and accommodation for thirty-two older persons. The Lodge provides thirty two single rooms, some of which have en-suite facilities. The Lodge provides communal rooms, which includes a lounge, dining room, library and a snug. The Lodge is owned and managed by Bliss Family Care Limited and is situated in the village of Thurnby, close to the main A47 to Leicester. There are local facilities close by, church, shop, pub and regular bus routes. The bedrooms are accommodated on the ground floor, and first floor, access to which is via the stairs or by 2 passenger shaft lifts. The Home has a large and attractive garden, which can be seen from the sun lounge and many of the bedrooms. This Inspection took place between 12.30pm and 3.30pm. Opportunity was taken to look around parts of the home, talk with residents, view records and policies which included viewing the care plans of four residents residing at the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, standard 6 is not applicable. The admission process is well managed with residents needs being assessed ensuring staff have the appropriate information to meet individual need. EVIDENCE: The admission process is well managed, the Registered Manager or Deputy Manager carry out an assessment of need, to ensure that the needs of the individual can be met. Individuals admitted from hospital have an assessment of need undertaken at the hospital by a Social Worker, the Registered or Deputy Manager also undertakes an assessment, visiting the individual in hospital. The Inspector viewed the assessments undertaken, all were detailed and incorporated relevant medical and social history. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 and 11. Residents are looked after well in respect of their health and personal care needs and are involved in the drawing of their care plans. EVIDENCE: The care plans of four residents were viewed, all contained detailed information as to the individuals needs. Daily reports are written as appropriate, which reflect the care of the individual, these are summarised on a weekly basis, with a weekly report produced. Risk assessments and the wishes and views of residents, including their views of illness and dying are incorporated into resident’s records; this enables staff to deliver care which reflects individual belief, with any risks being identified consistent with the promotion of independence and resident rights. Contact with medical personnel is recorded within resident records, records viewed evidenced that residents are supported to access health care. Resident records reflect as to whether the resident wishes to be responsible for the administration of their own medication, this practice is encouraged and risk assessed. Residents are also risk assessed as to the level of support they require with regards to the taking of medication. Residents when asked as to the care they received, indicated that they were all very happy, and could not see how it could be improved. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15. Residents experience a homely life style and visitors are encouraged to visit. Various formal and informal activities are made available to residents. EVIDENCE: The management team organises a variety of social, recreational and leisure pursuits for residents. Singers visit the home twice a month, a music and movement session takes place weekly, volunteers organise card, bingo or games of scrabble. Quarterly a ‘Storyteller’ visits, who in costume and through acting tells a story. Externally residents in July 2005, have attended at the Demontfort Theatre in Leicester ‘High Society’ with future planned theatre trips to see the ‘King and I’ and ‘Annie’. Two residents are to go on holiday, with two other residents participating in relative’s holidays. The activities board identified that there was to be a Garden Party, which would incorporate a cream tea at the end of July 2005. Holy Communion takes place within the home every month. Residents go out independently to the local village, whilst some go out with relatives and friends. The Inspector observed residents sitting in the dining area for their lunch; this was a three-course meal, with choices being offered for all courses. Residents said that the food at The Lodge was of a very high standard, and could not think of how it could be improved. In the afternoon, the tea trolley came round, residents had a choice of a cup of tea, or home made fruit punch with a slice of cake, and residents were also offered an ice cream. A member of staff in the afternoon speaks with all residents on an individual basis, to ascertain their choice for tea and for their lunch the following day. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 24, 25 and 26. A comfortable and safe standard of accommodation is provided for the resident’s including a garden area, which currently meets the individual and collective needs of residents. EVIDENCE: The Lodge is decorated and furnished to a very high standard, communal areas include a library, sun lounge, dining room and snug. The sun lounge overlooks the garden and has access to the patio and garden. Resident bedrooms are decorated and furnished to a very high standard, with twenty-five of the thirty-two bedrooms having an en-suite facility consisting of a toilet and wash hand basin. Residents who so choose have a telephone installed in their room, to enable them to maintain independent contact with relatives and friends, alternatively there is a pay phone located in the library. The Lodge Residential Home C51 C01 S45008 The Lodge V237750 140705 STAGE 4.doc Version 1.40 Page 15 The Inspector viewed one bedroom, which overlooks the garden on the ground floor; the resident said that she loved the room, as it was very comfortable. The garden to the rear of the home is well maintained, with a lawn area, mature trees and shrubs. Hanging baskets and flowerbeds along with seating areas in the garden and patio enable residents to relax in the garden. Since the last Inspection, a regime for the cleaning of the home has been developed, consistent with infection control. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30. Staff at the home are well trained and supported, and employed in sufficient numbers to meet the needs of residents. Recruiting procedures are robust, with checks in place for the protection of residents. EVIDENCE: The staff roster was viewed, in addition to a Manager in the morning there are five care staff on duty and 4 care staff on the afternoon and evening, with two care staff on duty during the night, and a Manager on-call. Currently there are twenty-four care staff employed, of which two have completed a level 2 National Vocational Qualification, with two having completed level 3. There are eight staff members currently working towards level 2, and three members of staff currently working towards level 3 to attain a National Vocational Qualification. The recruitment records of staff employed since the last inspection were viewed; all contained the necessary documentation consistent with recruitment practice to ensure the protection of residents. The Registered Manager for each member of staff keeps a detailed record of documents, the date they were requested and received. Records are also kept as to care staff receiving a copy of the Codes of Conduct, for social care staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 35, 36, 37 and 38. Records within the home are organised and well maintained, and reflect the maintenance of the environment to ensure the welfare of residents. EVIDENCE: The Registered Manager has attained a level 4 National Vocational Qualification in Care and the Registered Managers Award; a Deputy and an Assistant Manager support the Registered Manager. The management team are designated areas of responsibility, which includes the day-to-day and formal supervision of staff. Residents are consulted on all aspects of the running of the home, and their views are acted upon on a daily basis. The Responsible Individual and the Registered Manager maintain a very high level of record keeping, for all aspects relating to the home, this ensures the health and safety of residents and staff. Information pertaining to residents and personnel are stored consistent with Data Protection.
