This inspection was carried out on 15th September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

All residents are issued with a contract stating the terms and conditions of their residency. These contracts are signed by a representative of the home and by the resident or their representative if appropriate. All files tracked contained comprehensive information regarding residents` healthcare needs. Staff are required to attend to residents personal hygiene needs as and when they arise. Residents are enabled to make choices and decisions within their risk assessments. Residents feel able to complain and feel confident that they will be listened to. The home is clean and tidy and measures are in place, including staff training, policies and procedures, to prevent the spread of infection. There are sufficient staff within the home and they hare the skills necessary to meet residents` needs. Rotas are used creatively to respond to changing needs throughout the day.

What has improved since the last inspection?

At the last inspection a recommendation was made that the home does not accept medication without a pharmacy label. None of the containers of medication examined was without such a label.

Room G9 to be used only as specified in detailed letter from the National Care Standards Commission dated 22 April 2002 10th May 2005 Date of last inspection Brief Description of the Service: The Meadows Residential and Nursing Home is a care home providing accommodation for up to thirty-four service users with Nursing needs. It is located in the village of Thringstone near Coalville, Leicestershire. The home can be accessed by car from Loughborough, Coalville and Ashby de la Zouch. Bus services run regularly through the village. Local beauty spots include Charnwood Forest and Bradgate park. The home is within easy access of the M1 and M42 motorways.The building itself is a modern purpose built property set in its own grounds and is located in a residential area. Accommodation is available on ground and first floors. A passenger lift provides access to the first floor. All areas of the home and grounds are accessible for people with limited mobility. This was the second statutory unannounced inspection this year and took place over four hours. Four files were tracked and the inspector spoke with two residents. Various other documents were also examined. Feedback from the residents was positive; staff were described as helpful and the food was described as good. They stated that they are able to make choices within limits and that they understand the reasons for such limits. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2 All residents have a contract stating the terms and conditions of their residency. EVIDENCE: The inspector was shown an example of a contract between the home and a resident. The contract contained comprehensive information about the rights and responsibilities of both the home and the resident. It was stated in the contract that residents have to pay fees to the home during a stay in hospital and that fees will be collected for a period of approximately four days following the death of a resident. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8,9 Residents’ health care needs are met. Residents are protected by the home’s medication policy. EVIDENCE: All files tracked contained comprehensive information regarding residents’ healthcare needs including appointments with GPs and healthcare specialists. Risk assessments were also present for areas such as pressure area care and diabetes. All files contained detailed instructions regarding cleanliness. They all stated that residents who use incontinence pads are to be changed as soon as it is indicated that they need to be and that they are to be washed at every change. Residents who spoke with the inspector stated that they receive the support they need when they need it. The inspector examined the medication cupboard to follow up on the recommendation made at the last inspection regarding the absence of pharmacy labels on medication. All medication examined had a suitable label on it. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Residents are enabled to exercise choice over their lives. EVIDENCE: Two residents spoke with the inspector and they stated that they are enabled to make choices about their lives. They did understand that such choices had to be made within their risk assessments and stated that they find this a little frustrating at times but do understand the safety reasons for it. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Residents feel enabled to complain and are confident that their views will be listened to. EVIDENCE: The home has a comprehensive complaints procedure that all residents are made aware of. The inspector was shown the book in which complaints are recorded. All complaints appeared to have been actioned within relevant timescales and to the complainant’s satisfaction. No complaints had been received since March 2005. The residents who spoke with the inspector stated that they had no cause to complain but that they felt confident that they could do so and that they would be listened to. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 The home is clean and tidy and measures are in place to prevent the spread of infection. EVIDENCE: The inspector discussed infection control with the registered manager. All staff undertake training and are made aware of all relevant procedures such as hand washing and use of protective clothing. The registered manager stated that a number of residents have been admitted to the home with infections such as MRSA that they have acquired elsewhere and that they have risk assessments in place to prevent cross infection. Infection control policies and procedures were comprehensive and up to date. Staff were observed using protective clothing such as gloves and aprons. All areas of the home seen by the inspector were clean and tidy. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 There are sufficient staff with appropriate skills to meet residents’ needs. EVIDENCE: The inspector discussed staffing with the registered manager. The home currently has sufficient staff to meet residents’ needs although at the time of the inspection annual leave was being covered. All staff seen by the inspector interacted positively with residents. The residents who spoke with the inspector stated that they like the staff.
