What has improved since the last inspection?

Following the last inspection 2 requirements were made. Firstly to have all static electrical fixings checked and secondly to display menus. Both of these requirements have been complied with.

Date of last inspection 02 March 2005 Brief Description of the Service: Minstrels Residential Home is a home for up to 38 people who are admitted in the categotry OP - Older People. The home is located at the back of a plot which also accommodates Minstrels Nursing Home and sheltered accommodation flats. The home is set in landscaped gardens with car parking facilities. The home is placed in a residential area close to the amenities of Boston Town centre. Accommodation is provided in a mixture of single and double occupancy bedrooms with many having en suite facilities. The home’s manager assisted the inspector during the unannounced inspection. One inspector undertook the inspection spending four and a half hours in the home .The inspector toured the building and spoke with the homes manager and deputy manager. There were 35 residents living in the home and 8 were spoken with. The main method of inspection used was called “case tracking” which involved selecting clients and tracking the care they receive through checking of their records, discussion with them, the care staff and observation of care practices. What they could do better: The home needs to confirm in writing that the residents needs can be met following a pre admission assessment. Photographs of residents and staff need to be in place for all. Progress notes need to contain sufficient detail so that it can be clear what action has been taken if there is a change in a residents welfare of health. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4 The home provides information so that residents can make an informed choice about living at the home and contracts provide information about the service the resident can expect. Residents need confirmation that their needs can be met to give them confidence in being admitted to the home. EVIDENCE: The home has an updated service user guide in place, which contains the necessary information. Residents spoken with were clear of the staff structure in the home and what facilities were on offer. Through case tracking it was evidenced that residents had been given contracts which contained terms and conditions. Residents are assessed prior to admission to the home and a written assessment is placed on their files. Residents are not written to, to confirm that their needs can be met. The manager has confirmed that this will now happen. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9, Care plans provide sufficient information to ensure that residents needs can be met. Risk assessments protect residents from being placed at unnecessary risk. Sufficient details need to be contained in progress notes. Access to health care professionals is satisfactory which ensures service users health needs are met. Medication systems ensure medication is administered in a safe manner. EVIDENCE: 2 residents care files were inspected. Care plans were clear and had been reviewed monthly. Progress notes are written in when there is a change in welfare or health needs. It is recommended that an entry be made at least weekly. Entries in progress notes did not always have further information to reflect if a change is need had been resolved. Residents spoken with confirmed that their needs were met and that they were aware that there were information records held by the home. Three residents spoke about having access to dentists, opticians and chiropodists. All residents are registered with one of 4 local surgeries and can meet with the doctor in private. A risk assessment is in place for this practice. All staff who administer medication have received training. Medication administration records were inspected and these had been completed satisfactorily. Photographs are used to help with identifying all residents. A couple of medication sheets did not have photographs in place. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,15 Service users are provided with a varied diet with preferences and specialised diets catered for. Activities are provided but residents would like to have more opportunity to access outside of the home. EVIDENCE: The home has a four week menu in place and this is displayed daily. The home provides a hot main meal and if residents don’t like this then an alternative is provided. Specialised diets such as diabetic, soft and celiac are provided for. All of the residents spoken with said they enjoyed the food and there was always plenty. The kitchen was clean and tidy and the home employs a supper cook to cover teatime up to 6:30pm. Residents said that the home provided activities and that these tended to be quiz afternoons and card games and outside entertainers. 3 residents said that they would like to go out of the home more but that this was not offered to them. An example was that one resident was unable to get into the homes minibus as they used a wheelchair. The manager discussed this issue with the inspector and confirmed that trips out of the home had been provided and 3 residents had gone to a farm centre the week previously. The manager also said that when a trip was advertised not many service users said they wanted to attend. It is recommended that a record is maintained of all activities offered and who attended. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, Residents have systems available to them to raise concerns and complaints. Training provided to staff ensures residents are not put at risk. EVIDENCE: The home has a complaints procedure in place and this forms part of the service user guide. Those residents spoken to said they were clear who to speak to if they had any concerns. From inspection of a care staffs file it was evidenced that adult protection training had taken place recently. Induction training also covers adult protection issues. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23,26 Residents live in a safe environment. The standards of decoration and furnishing are high, providing residents with a comfortable and homely place to live. There are adequate communal areas for residents and bathrooms are homely and fitted with equipment to meet resident’s needs. EVIDENCE: A tour of the home evidenced that the home was clean and tidy. Residents gave permission for their rooms to be inspected. All residents spoken with expressed satisfaction with their rooms, which were well decorated. The corridors in the home are wide and have handrails along the sides to assist residents. The home is set in landscaped gardens and residents said they thoroughly enjoyed sitting out on the patio. There were lavatories located by the communal areas and residents have the use of commodes in their bedrooms. The bathrooms inspected were homely and one had a hoist fitted, which had been recently serviced. The home has a lift, which access both floors of the home. There are quiet areas at the top of the stairs for resident to meet with visitors in private. A full time cleaner is employed and resident said that their rooms were kept clean and tidy. No adverse odours were noted on the tour of the home. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 Staff are receiving training and supervision to ensure they are competent to do their job. All Staff files need to contain photographs. An enthusiastic care team cares for residents. EVIDENCE: The duty rota demonstrated that minimum staffing levels were being met. Residents said that they felt their needs were being met and that in the afternoon care staff did have time to sit and talk with them. The manager works in a supernummary capacity and extra staff will work if there is a change in residents needs. Staff were seen to speak with residents in a valuing and appropriate manner. Residents said that they found staff approachable and kind. Two staff files were inspected. Application forms and induction packs were in place. It was evidenced that staff were receiving training in care practices. No photos were in place for either of the files inspected. It was evidenced that one member of staff had received a supervision session. This took place in October 2004. The manager is aware that supervision sessions need to take place 6 times a year and this target is being worked towards. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,37,38 The home is well run, with good guidance and leadership for staff. Record keeping is of a good standard and the health and safety and welfare of residents is promoted. EVIDENCE: The registered manager Mrs Sandra Copeland manages the home. Mrs Copeland has been the manager of the home for almost 9 years. Resident spoke highly of the manager and confirmed that she was approachable and often spoke to them on a daily basis. The manager also provided residents meetings. Staff were seen to be spoken to in an appropriate manner by the manager. Polices and procedure are in place and the manager confirmed that these are reviewed and updated regularly. Records for fire safety and hoist servicing were inspected.
