This inspection was carried out on 9th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This is a care home where residents are well looked after. The Inspector was able to see first hand, the care and attention being give to one resident who required regular visits by the district nursing service and also observed the professional manner in which the owner and staff related to the nurse. The staff team work well together and show a good understanding of the needs of the residents in the home. The homeowner felt that they work hard to ensure that the staff that are employed are suitable for their role and fit into the staff group. The majority of the resident are very elderly and frail, one being over 100years and two coming up for their 100th birthday. Those residents who were able to make their feelings known said that they were happy in the home and the staff looked after them well. The atmosphere in the home is very relaxed and visitors are made welcome. A visitor who was spoken to during the inspection said that they could not speak highly enough of the home. They said that the resident that they were visiting had been in a poor state of health before they came into the home and with the good food and good care they had improved beyond recognition.

What has improved since the last inspection?

Since the last inspection three bedrooms have been redecorated and refurnished completely.During the previous inspection, the homeowner was advised to confirm in writing to residents, before admission, that the home could meet their assessed needs. This advice has been acted upon. The assistant manager has successfully completed one course and is near to completing another, which are recognised qualifications, required for the management of a care home.

What the care home could do better:

The management and staff work very hard to ensure that the needs of the residents are met and are considering applying for the home to become registered for elderly persons who have Dementia, due to the changes in the needs of the residents, to ensure that their needs can be fully met. Staff training should continue and the homeowner should ensure that in the light of their proposals, staff have the opportunity to go on courses that cover care of people with Dementia. The homeowner, who is a qualified nurse, should make arrangements to undertake a management qualification or ensure that a person with the required qualifications is registered in the position of manager.

The accommodation comprises of eleven single bedrooms, two of which have en-suite facilities, two double bedrooms with en-suite facilities, a lounge, a dining room and a sun lounge, which is very popular with the residents. There are sufficient bathing and toilet facilities situated within easy access for all residents. A passenger lift enables residents to access the first floor without assistance. The grounds of the home offer a pleasant and safe area for residents to use when they wish and a ramp at the front of the home enables residents to access the grounds easily. This was an unannounced inspection that commenced at 9.30am and took place over 2.25 hours. At the time of the inspection there were 15 residents in the home. The Inspector spoke to 5 residents, 1 visitor, 4 staff members and the homeowners. A selection of documentation was looked at and a general tour of the home was undertaken. EVIDENCE: At the time of the previous inspection it was noted that whilst the homeowner was confirming verbally that the home could meet residents assessed needs, they were not confirming this in writing. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 &14 Social activities and contact with family and friends are actively encouraged. Residents are helped to exercise choice and control over their lives within their capabilities. EVIDENCE: Examination of residents’ files confirmed that information is recorded about the resident’s social, cultural, religious and recreational interest and needs. The Inspector was informed that in addition to the daily discussions that take place, staff discuss with residents on an individual basis, their expectations and wishes. Relatives are also asked for their input to ensure that the needs of the residents are being met. Every afternoon staff spend time with the residents encouraging them to participate in activities, discussions and what ever they wish to do. Staff spoken to said that this can be difficult at times because of the frailty and capabilities of the majority of the residents. The homeowner told the Inspector that they are making arrangement to have a physiotherapist visiting the home to encourage resident to take gentle exercises. The Owls Rest Home DS0000009812.V274146.R01.S.doc Version 5.1 Page 11 One resident spoken to said that they enjoyed staying in their bedroom but came out to have a cigarette, which the Inspector observed during the inspection. The Inspector was told that only through spending time and talking to this resident had the staff found out that the resident had enjoyed a smoke when they were at home and dearly wanted to continue to have the occasional one. The abilities of the residents to make their own decisions varies greatly throughout the home. Residents who were able to communicate with the Inspectors confirmed that they were able to make their own decisions. Visitors are made welcome at any time and a visitor confirmed this at the time of the inspection. The Inspector was able to observe the warm, friendly manner in which visitors were received. The written information contained in the residents’ handbook includes information about visiting and the open house policy. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Arrangements for complaints are handled well and taken seriously ensuring people feel listened to and protected. EVIDENCE: The home has a complaints policy and procedure that is clear, accessible and meets the requirements of the standards. A copy is displayed in the hall and the procedure is outlined in the written information given to residents in the Residents Handbook. The homeowner is aware that residents and their relatives should feel confident that their complaints are listened to, taken seriously and acted upon. The homeowner informed the Inspector that the home had not received any complaints in the past twelve months and the Inspector confirmed that the Commission had not received any complaints. JUDGEMENT – we looked at outcomes for the following standard(s): 31 & 35 The home is well managed and run in the best interests of the residents. EVIDENCE: The homeowner and assistant manager work very closely together and both are experienced in caring for the elderly. The Inspector was informed that the assistant manager had successfully completed NVQ (National Vocational Qualification) level 4 and had almost completed the Registered Managers Award. The staff spoken to had a clear understanding of their role and what was expected of them. The Inspector was able to observe the relaxed atmosphere in the home and the positive, caring way in which staff and the homeowner interacted with the residents, visitors and each other.
