Conditions of registration: None Date of last inspection 15/12/04 Brief Description of the Service: The Willows is small home in Morpeth that provides care and support for a mix of younger adults and older people who have mental health problems. There are three separate bungalows within the attractive and well-maintained grounds. This home is placed close to a number of good local amenities, and is close to local transport services. This is an overview of what the inspector found during the inspection. This was an announced inspection and lasted for 5 hours. 6 residents offered their views on what it is like to live at The Willows. Questionnaires were sent to residents and relatives and these comments are included in the report. 3 members of staff contributed to the inspection, and the Manager was available throughout. Kitchens and bathrooms were inspected while touring the building and 1 person requested that their room was inspected. Documents relating to people’s care and support were inspected and discussed with the Manager. Matters relating to the Health and Safety of the people who live at the home were also inspected. What the service does well: The Willows has very good systems for assessing people and planning for their care and support; this planning is done with the people concerned. The delivery of care and support is carried out with sensitivity. Each person has a suitable range of leisure, education and therapeutic activities they can take part in. This home is very good at supporting people when they are unwell and encourages people to manage their own conditions. Staff are well trained and are enthusiastic about what they achieve with people. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. JUDGEMENT – we looked at outcomes for standard(s) 2 Each person has had their needs and ambitions properly assessed EVIDENCE: Of the 3 files inspected, each had a comprehensive range of assessments carried out Health and Social Care professionals. The home is currently carrying out their own assessments, with the involvement and co-operation of residents, using an evidence-based tool that is suitable for the people who live there. Where changing needs have been identified, referrals have been made to the appropriate agency, resulting in reassessments. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 9 People are involved in their own care planning. The home encourages and supports people to make their own decisions. People’s preferred activities and lifestyles are supported by good risk assessments. EVIDENCE: The existing arrangements for care planning have been commended and each plan reflects what is important to people. A new system is currently being introduced that is based on evidence-based practice with the involvement of residents. One person was especially enthusiastic about this new system and was finding the process very helpful to them. The Manager explained that this work is in depth and that it will take time to complete the plans with all residents. People are encouraged to make their own decisions whenever possible; where there may be an element of risk this is recognised and a risk assessment is carried out; these risk assessments emphasise people’s own strengths and individual vulnerabilities. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 16, 17 People are encouraged to be part of the local community; their rights and responsibilities are recognised and respected. Each person is offered a healthy diet. EVIDENCE: Several people said that they regularly use local shops, pubs and other places in the local community that interest them. There is an interesting range of activities that people can join if they wish, but one person who needs extra support said that there could be more staff – this was discussed with the Manager who said that interviews for new staff were due to take place the same week. Everyone spoken to said that the meals prepared for them were good; menus are varied and people know that they can request an alternative meal if they do not wish to eat what is offered from the menu. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 People receive personal support in the way that suits them. People’s physical and emotional health needs are being met. The work carried out in these standards is commendable. EVIDENCE: Three people were especially enthusiastic about the support they received from staff; one described how they are given “fantastic” support when they are beginning to feel unwell, while another said that staff were “very supportive”. One visiting relative was full of praise for the way staff carried out their duties. There was a good sense of people being given the right kind of information and support to manage their own conditions. All of the home’s systems ensure that people’s physical and mental health is properly addressed and that any early warning signs are quickly acted upon. EVIDENCE: Each person who lives at The Willows is aware of the complaints procedure; an advocacy agency is used to ensure people’s independent views will be heard regarding any matters of quality or concern. JUDGEMENT – we looked at outcomes for standard(s) 24, 30 The Willows provides a comfortable and safe place for people to live in. Everywhere inspected was clean and hygienic. EVIDENCE: Each of the three bungalows is well presented and accessible for the people who live there. Each bungalow has a programme of ongoing repairs and decoration. There were a few matters that needed to be attended to and these were discussed with the Manager; no requirements or recommendations have been made as this home has a history of sorting things out quickly. JUDGEMENT – we looked at outcomes for standard(s) 32, 35 People are supported by a competent and qualified staff team. There is a good, ongoing training programme for staff. EVIDENCE: Residents were very complimentary about the staff’s skills and qualities. Staff know about what is important to the people who live at The Willows and were observed treating people with respect and dignity. Training for staff includes the subjects required by law such as Food Hygiene, Fire Safety, and Infection Control. Further training is provided that relates to the individual needs of people. JUDGEMENT – we looked at outcomes for standard(s) 38, 42 The management approach of the home leads to good outcomes for the people living there. Good consideration is given to the Health and Safety of people. EVIDENCE: The management approach of the home makes it easy for the people who live there to air their views. The Manager is competent, enthusiastic, reflective and experienced. A new system for Quality Assurance is to be introduced in October.
