Conditions of registration: 1. The service shall employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection as manager of Vine House only. Date of last inspection Brief Description of the Service: Vine House Rest Home registered with the Commission for Social Care Inspection to provide personal care and accommodation to 14 older people. The home is located on the main road of Oswaldtwistle and is within easy access of public transport services, shops and local amenities. Accommodation is provided on two levels and access to the first floor is via a stair lift. There is one double and twelve single bedrooms and there are sufficient toilet washing and bathing facilities provided to meet the needs of the service users. There is a small private car park and garden area at the front of the building. All meals are prepared on the premises. This is an overview of what the inspector found during the inspection. This unannounced inspection was the second in the inspection year and took place over one day on Monday 6th March 2006. The service was inspected against the National Minimum Standards for Older People. Case tracking of service users also took place along with the rooms they occupy in the home. Observations were made of the care provided and the service users were invited to have a discussion with the inspector to discuss their experiences of life in the home. The inspection spoke to 6 service users, 2 care staff, 1 cook, 2 students on work placements, 1 relative and the registered manager. Breaches in regulations and standards that pose an immediate risk to service users have been highlighted for urgent action. What the service does well: What has improved since the last inspection?
Care plans examined had been reviewed to include risk assessments relating to falls and bed rails. A requirement relating to service user daily records made at the previous inspection was addressed satisfactorily. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): OP 7, 8, 9 Service user health care needs were identified and met through care plans. The control of medication was well-managed promoting good health. EVIDENCE: Case tracking of service user who moved into the home in November showed the care plan had been drawn up from the initial service user assessment. The plan addressed areas of care and detailed the care to be given. Choices in dayto-day living, assistance, dietary needs, intellectual cultural needs, interests hobbies, background, life history, and lifestyle choices enabled staff to meet service users needs better. The care plan also contained a service user personal profile and reason for admission to the home that included a medical history, known allergies and last wishes. It also highlighted individual use of specialist equipment to ensure service user safety and comfort in the home and a risk assessment around cigarette smoking was included in the care plan. The care plan examined was recently reviewed and care plan review sheets had been completed and signed by staff. Service user daily reports had been written daily When asked one service user commented positively on the care received and said, “It’s alright here, my bedroom is very nice the staff are lovely, really

During a tour of the building the inspector noted equipment necessary for the promotion of tissue viability and prevention of pressure sores was provided. Case tracking and examination of two service user care plans highlighted this and discussion with one service user confirmed this. There was a medication policy and procedure at the home. Written guidance was available for staff to follow when administering medication. Policies and procedures examined ensured service user safety. All medicines were stored securely ensuring service users were kept safe from harm. Medication Record Sheets were completed accurately. A majority number of care staff had undergone training in the safe handling of medicines and a care staff was observed competently administering medication to service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): OP 12, 14 Daily routines were flexible and varied. Service user autonomy and choice was maximised. EVIDENCE: Case tracking and discussions with service users confirmed that routines and daily living were mostly based on service user wishes. This information was highlighted in care plans. Past interests, hobbies, present needs and wishes were also included in care plans. When asked one service user confirmed they had regular contact with their relatives and visiting times were flexible. The inspector observed another service user having her hair washed and styled by her granddaughter who was visiting that afternoon. Relatives were observed visiting service users throughout the afternoon and were made welcome by staff on entering the home. The registered manager confirmed this was a regular occurrence for them. It was apparent that service users who did not receive visitors that day were included in conversations with other visitors and enjoyed the different company. Case tracking confirmed that the home did not handle service user financial affairs and relatives did this. Bedrooms examined were personalised with service users own photographs, furniture and other personal possessions. JUDGEMENT – we looked at outcomes for the following standard(s): OP 19, 22, 26 All areas of the home were safe and well maintained. Specialist equipment, furnishings, toilet and washing facilities met service user needs. EVIDENCE: The location and layout of the home was suitable for it’s stated purpose. It was accessible safe and well maintained meeting service users individual and collective needs. There were ample accessible toilets for service users close to lounge and dining areas. An assisted baths was available in the home. Where rooms weren’t en suite toilets were within close proximity of service users bedrooms. Call systems were provided in all bedrooms. Grab rails were provided in bathrooms, toilets and corridors. Aids, hoists and assisted toilets and baths were installed and capable of meeting the assessed service user needs. JUDGEMENT – we looked at outcomes for the following standard(s): OP 29 Service users were not fully protected by the home’s recruitment procedures EVIDENCE: Employee case tracking confirmed that the recruitment process recently used by the home did not ensure the protection of service users. Checks that should have been made prior to appointing a new employee had not been done. An immediate requirement was issued for the registered manager to ensure that all relevant checks relating to the new employee including 2 written references and an up to date CRB and POVA 1st check are carried out.
