The property is situated in the residential area of Netherhall close to shops and other facilities. The home is easily accessible by private and public transport. The home consists of two floors accessible by use of stairs and passenger lift. There are a variety of facilities in the home including dining and lounge space. The majority of bedrooms are single bedrooms, of which two have en-suite facilities and there is one double bedroom. Information with regards to fees is available by contacting Vishram Ghar. The reason for this inspection was to follow up on requirements made at the previous Key Inspection which took place on the 21st and 22nd July 2009 and the outcome is as follows. We looked at three care plans and found that they were detailed and contained a good level of information as to the needs and wishes of people receiving care, and found that care plans were regularly reviewed which included consultation with the person receiving care and/or their representative. Risk assessments were in place where risks had been identified, including where those in receipt of care manage or administer their own medication. Risk assessments were reviewed regularly in consultation with the person receiving care and/or their representative. Discussions were recorded, where risk assessments are carried out, the person carrying out the risk assessment must record the methodology used to determine the outcome of the risk assessment. We looked at medication and found that there is a system in place for the auditing of medication which is received into the home, administered or returned to the Pharmacist. We also found that there is a system for recording the temperature of the fridge which is used to store medication. A new Controlled Drug cupboard has been installed. We spoke with the Area Manager and Manager of Vishram Ghar who told us that staffing levels had been reviewed with reference to the needs of those receiving care, and that staffing numbers during the day had been increased for the morning and afternoon. We were told that the reviewing of staffing levels had included consultation with those receiving care and their representatives. What the care home does well: What they could do better:
Care plans included information where individuals had Dementia, specific care plans need to be develop and should contain greater detail including known triggers for affecting individuals well-being along with measures as to how to deal with or anticipate situations which may cause distress or anxiety. Care plans for those with Dementia need to focus with greater detail on preferred daily routines, and information gathered about their lives prior to admission into Vishram Ghar to be used to provide support to reduce anxiety and distress. Care plans for those whose first language isnt English and who cannot read English, to be offered the choice of having their care plan written in their first language to promote understanding and discussion as to the care and support they need and would wish.

This would enable the Manager to understand and develop the care needs of people with Dementia, and promote individual rights and choices.
