This inspection was carried out on 8th March 2006.

What the care home does well

Service users were clearly the focus for the two staff on duty; they both demonstrated good interpersonal skills. Their interactions with service users were conducted both skilfully and professionally

What has improved since the last inspection?

The homes owner has submitted an action plan for works to redecorate and upgrade the facilities at Walton Grange. She has made good progress in keeping within her timescales to achieving the changes she has made. Staff continues to work towards their NVQ awards and undertake personal training and development opportunities.

What the care home could do better:

The maintenance, refurbishment and redecoration has been prioritised and demonstrated within an annual action plan. These works are ongoing.

The Christian approach is offered to those residents for whom Christianity is important. It does not intrude into the lives of others who attach no importance to religion. The home does not discriminate against potential residents on the grounds of non-belief. The building is a large Victorian property situated on a wide tree lined avenue with similar properties on both sides. Walton Grange is registered to up to six residents with Dementia. They are provided with a dedicated lounge and dining area. Staff has received additional training to equip staff to meet their care needs. Walton grange offers single room accommodation for all residents. Ten rooms have en-suite facilities. All bedrooms are fitted with locks to enable residents to carry a key and lock their individual bedrooms. Staff have a master key to facilitate entry in case of emergency. Lounge areas and other communal spaces include a room where smoking is allowed with other areas providing television and quiet areas. The Inspection took place over 6.5 hours and was the second of the cycle of inspections for year 2005/6 and followed a risk assessment carried out with the CSCI risk assessment tool It was the Inspectors first visit to the home; consequently an emphasis was placed upon meeting residents, relatives, visitors and the staff team. The Officer toured the whole site to understand the range of services provided there. The officer case tracked 3 resident’s files, and the records maintained. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,6 Records demonstrate that Residents and their advocates are involved in choosing their home. The management consider the needs of others residing at the home when making this assessment. EVIDENCE: Case records examined contained copies of individual service user care assessments, plans and written contracts stating terms and conditions. Intermediate Care is not provided at Walton Grange; respite care is provided following assessment and by arrangement with the Manager. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Staff was focussed upon meeting residents needs. Their interactions with service users were skilful, professional and with obvious empathy for the individual concerned. Records indicate that residents and their families are involved in the social, physical and psychological care planning. Access to health care professionals and health care services is provided. EVIDENCE: Care records of 3 service users were inspected; they contained individual “needs” assessments and plans for staff to follow to meet the identified needs. The records were adequately maintained. Changes to plans and reassessment of physical and psychological needs were ongoing. None of the service users were responsible for their own medication although the facility to enable this is in place. Staff had received additional accredited training for the administration of medicines. Staff was observed dispensing medications and assisting service users to take them. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 The resident group have a range of needs and abilities. Activities are mainly conducted on a one to one or small group basis. Records demonstrate the plans/systems to facilitate personal choice and maintain links with the community. EVIDENCE: Planned service user activities are usually conducted on a one to one or small group basis to meet individual’s needs and abilities. There were numerous visitors to the home on the day of inspection. Visitors spoken to confirmed their satisfaction with the quality of care and service provided at Walton Grange. A number of service users leave the home in the company of relatives, friends or staff for short periods. They visit places of local interest or a relative’s home. Service users were observed to be smart and dressed appropriately. Staff guide and assist service users with their choice of clothing and personal appearance as required. Staff was observed to assist and encourage residents appropriately. The menu appeared to offer choice and adequate nutritional content. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Staff were confident and competent able to respond to concerns or complaints effectively. EVIDENCE: Staff has been undertaking additional training to both recognise and prevent abuse. Staff spoken with was confident in their ability to respond to any concerns or complaints that may be expressed. No complaints had been received by the CSCI. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26 The home’s owner is working in accordance with her action plan, timescale, and schedule of works to redecorate and refurbish the home. Health & Safety remedial work had commenced. EVIDENCE: The home was clean. There were no odours. Many individual bedrooms appeared comfortable and personalised. Service users are encouraged to bring personal items and memorabilia with them to achieve a personal and homely private area. Bedrooms had been redecorated and refurbished. This included soft furnishings and light shade replacement. The main dining area had been redecorated and was awaiting delivery of the blinds/curtains. Lighting throughout the home has been changed and is domestic in nature. The home’s owner continues to redecorate and refurbish the home in accordance with her action plan. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,30 The numbers of staff on duty on the day of inspection was in accordance with the duty rota. Ancillary staff was employed to enable care staff to focus on care duties and activities. Visitors to the home confirmed their satisfaction with the interpersonal skills and abilities. Staff appeared both knowledgeable and skilled when undertaking their duties. EVIDENCE: Examination of the duty rota confirmed that the numbers of staff on duty was in accordance the needs of the service user group. Ancillary staff was employed to enable care staff to focus on care duties and activities. Discussion with visitors to the home confirmed their satisfaction with the staff’s interpersonal skills and abilities. Staff interviewed appeared both knowledgeable and skilled when undertaking their duties. They confirmed the benefits and skills they gained from ongoing training and updates provided by the home’s owner Mrs Walton. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 38 The home’s owner and manager have appropriate knowledge and experience. Health & Safety, redecoration and refurbishment of the home is being addressed in accordance with the home’s action plan. EVIDENCE: Mrs Walton has owned the home for 25 years and is experienced in Residential Care. The building redecoration and refurbishment is being managed in accordance with the written action plan and schedule of work.
