What the care home does well

Individual care is provided that results in a supportive and well run service that meets the identified needs and expressed preferences of the residents. There is a stable staff group who are well trained and familiar with the needs of all of the residents. A comfortable, homely and well maintained house is provided that residents say they like and have made their own with personal belongings and photographs that are special to them. There are good relationships demonstrated between residents and staff that contribute to the pleasant atmosphere.

What has improved since the last inspection?

Some of the bedrooms and the bathroom have been redecorated. Care plans and staff records continue to be reviewed and updated. A copy of the up to date gas safety certificate was sent to the CSCI in accordance with the requirement made at the last inspection visit.

What the care home could do better:

The cracked bath is expected to be replaced. There has been an unexpected delay in the registered managers` completion of the registered managers award, this should be completed in the forthcoming year. Staff records must contain all of the required information.

The Home is registered for four residents with learning disabilities, who have lived there for at least the past 10 years. A stable staff group provides the care. The Home comprises a large domestic house, with an extension. It is spacious and is maintained to a high standard of repair. Décor and furnishings are also very well maintained. This inspection visit took place over one day in January 2007 and took place over the afternoon and tea time. It was necessary to pre-arrange this visit a few days beforehand so that the residents could be present during the visit. Discussions were held with all 4 of the residents who were proud to show the Inspector around the home. The Inspector also talked to the registered manager and the staff on duty. Other inspection methods used included the examination of care plans, staff training records, maintenance records and the medication system. Four completed CSCI resident surveys were returned that had been completed with the help of Derbyshire Advocacy Service. One requirement was made at the last inspection of this service and this requirement has been met. What the service does well: What has improved since the last inspection? A process for the assessment of residents need is in place, this assures residents that the home can meet their needs. EVIDENCE: The 4 residents accommodated in this home have lived here for at least the last 10 years and moved into the home together. No new residents have come to live here since then. There is a process in place for the assessment of a new residents needs should this be necessary. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are supported to take control of their daily lives in accordance with their expressed preferences and the care plan. EVIDENCE: Each resident has their own care plan that is colour coded so that the resident knows which is their plan. Two residents gave permission for the Inspector to look at their care plans, each person was aware that the plan contained information about them and they were involved in the development of as many aspects of the care plan as possible. The care plans took into account any changes that may have occurred in terms of the residents needs and were updated regularly. Reviews involved social services and health care professionals and the residents family. The service is tailored to meet the needs of each person, there have been few changes of staff since the home opened and consequently staff know the residents well. Personal and environmental risk assessments were in place that covered transport, safety, hot water and using household appliances. Advocacy services were encouraged to visit the home and had assisted residents in the completion of CSCI resident questionnaires. Residents had also been supported in attending advocacy sessions at a local venue. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 15, 16 and 17. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Appropriate individual support is given that enables the residents to enjoy activities and recreation that is suited to their needs and preferences. Good quality meals are provided in a relaxed and informal atmosphere that residents clearly enjoy. EVIDENCE: The residents in this home attend day centres in the community where they participate in various activities such as pottery, painting and baking and see friends. Residents spoke to the Inspector about having gone to the bank today, doing some shopping and going for lunch. This had clearly been an enjoyable morning with residents being supported and encouraged to take part in the daily routines of the home. Walton Road (195) DS0000020116.V325577.R01.S.doc Version 5.2 Page 12 Residents talked about their involvement in the housekeeping aspects of living in the home such as ironing their bedding, keeping bedrooms clean and tidy and the jobs they particularly liked to do and had chosen to take responsibility for. Staff supported residents in all of these areas and it appeared that everyone worked well together for the benefit of residents. Family and friends are encouraged to visit the home, residents spoke of the visits that they had enjoyed and showed the Inspector many photographs of the people that were important to them. Two residents had families who lived a distance away and staff regularly took the residents to visit and have holidays there. This visit took place over tea time, this was an enjoyable and relaxed meal where residents and staff were provided with an opportunity to share the days events in an informal way. Meals are organised around a balanced and varied menu with the opportunity for residents to choose what they want and be involved in the planning of meals. The home has sought the views of the residents who are fully involved in all aspects of the planning of daily routines and activities in the home and in the community. The home therefore provides an excellent service. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Personal support and healthcare needs are met in the manner in which residents prefer and in accordance with assessed need. Medication systems are in place that serve to protect residents. EVIDENCE: The care plan underpins the personal support that is given. Regular health care check ups are planned and staff provide the support necessary for each person when they attend any appointments. Changes in residents needs are documented and communicated to staff via handovers and daily records. Healthcare and social services professionals are involved in reviews and advice is sought where a need arises. Access is provided for the dentist, optician, and chiropodist. Residents are reliant upon staff to assist with the administration of medication. Training has been completed for staff regarding the safe handling and administration of medicines in order to assist in safeguarding the residents. Policies and procedures are in place to underpin the systems. A local pharmacist visits regularly to provide advice about the medicines used. This judgement has been made using available evidence including a visit to this service. Staff training, policies and procedures assist in the promotion and protection of residents’ rights. EVIDENCE: There are no concerns or complaints having been received by the home or by the local office of the CSCI. A clear complaints procedure is in place that is in an appropriate format for the residents and is present on each persons care plan. Advocacy services are encouraged to be active in the home and residents attend an advocacy group in the community. Care review notes indicate that the residents representatives are encouraged and able to advocate on behalf of the residents. Policies and procedures in the home regarding safeguarding adults assist in the protection of residents. Staff have attended training regarding abuse awareness and the procedures to follow if they are concerned about the welfare of a resident. The registered manager has attended a 2 day course regarding Derbyshire’s local safeguarding adults procedures. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 30. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. This service provides a lovely, homely environment that residents say they like and have made their own. EVIDENCE: The house is situated on an estate that is suited to the needs of residents, close to a bus stop and local shops and facilities. This service provides a well decorated, comfortable and well maintained home for the residents who live there. Each resident was pleased to show the Inspector their bedroom and were very proud of the way in which they had arranged their personal belongings. Three residents chose to spend time in their bedrooms listening to music or doing craft work, the rooms were arranged so that they could do this. All of the bedrooms were single and spacious. For these reasons the home exceeds standards in this area. Every where was clean and hygienic. There is a crack in the bath and a recommendation has been made for the bath to be replaced. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34, 35. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff are appropriately trained in order to meet the needs of residents. EVIDENCE: Staff have accessed a range of training including moving and handling, fire safety, food hygiene and the safe administration of medicines. A programme of NVQ training is in place that staff are gradually completing. Staff communicate well with residents using signs and non verbal communication methods that are individual to each resident. The registered manager has an overall training plan that takes into account each member of staffs learning needs. The staff group is stable with one new member of staff having joined the team in the last year. This member of staff has completed an induction programme and is due to commence NVQ training. Staff files are generally in good order and follow the homes established recruitment procedures. This includes obtaining a completed CRB and POVA First check and an interview process. Two written references have been obtained except for one member of staff where one written and one verbal reference was on file. All of the other required checks were completed. This judgement has been made using available evidence including a visit to this service. Systems are in place that contribute to the home fulfilling its stated purpose and objectives and meeting the needs of the residents. EVIDENCE: The registered manager has worked in this home for a number of years and knows the residents well. The relationships demonstrated between staff and residents were warm and positive with residents being animated and feeling free to join in and instigate conversations and jokes. There is an established quality assurance system in place with monthly recorded unannounced visits from the service manager. Resident questionnaires are also completed annually. Certificates of maintenance were kept in good order and up to date including those for wiring and insurance purposes.
