The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The residents spoken with are very happy with the care they receive and made many positive comments. They said that the staff are very helpful and that they ensure that people`s privacy and dignity is maintained at all times. The observed interaction between residents and staff was of a very good standard. People said that they particularly liked the relaxed environment and Woods Court and the opportunity to make decisions about their day-to-day lives. One person said that she chooses to spend most of her time in her bedroom, except when she goes to participate in the impressive range of activities provided. All of the residents spoken with said that they enjoy the food provided at Woods Court. One person confirmed that she receives a specialist diet. The premises are purpose-built, they are well equipped and maintained to a good standard. People have been encouraged to personalise their bedrooms with pictures ornaments and small items of furniture. There were many aspects of good practice highlighted in the main body of the report.

What has improved since the last inspection?

The manager has fully implemented all but one of requirements made following the last inspection. The inspector was advised that nine bedrooms have been redecorated and five carpets have been replaced. New curtains and duvet covers have also been provided. Staff have also received a considerable amount of training since the last inspection.

What the care home could do better:

The manager must ensure that where possible residents or if appropriate their representatives sign to confirm that they have been involved in planning and reviewing the way in which care is provided. The way in which medication is administered has improved significantly however staff need to ensure that risk assessments for people who manage their own medication are updated on a regular basis, to ensure that they remain safe. Staff need to ensure that residents health and safety is not put at risk by allowing them access to the laundry room when there are no staff in situ. The homes Quality Assurance System needs some minor amendments to fully meet the required standards.

It is owned and run by Nottinghamshire County Council Social Services. The home is located within easy walking distance of Newark town centre where there are a variety of shops and other facilities. The home was opened in 1988 and consists of a two-storey purpose-built property, divided into five units. There is a shaft lift to assist independent access between levels. All of the bedrooms are for single occupancy, with wash hand basins. None of the rooms have ensuite facilities. The gardens are well tended and are accessible by service users. The inspector was advised that there is a secure garden area leading directly of one of the units which has been specifically designed for people who have dementia. This unannounced inspection was completed over one day and lasted for approximately 5 1/2 hours. It included the inspection of care and other records, a discussion with three members of staff, three residents and two visitors to the home. A partial tour of the building was also completed. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,6. The literature supplied prospective residents now contains all the required information. Prospective residents needs are comprehensively assessed prior to their admission to the home. The Intermediate Care Unit is helping people to maximise their independence and where possible return home. EVIDENCE: The literature supplied to prospective residents has now been updated to include a summary of the resident’s views of the home. This document now contains all the required information to enable people to make an informed choice as to whether the home will meet their individual needs. Of the three residents records checked during this inspection, two people had Extended Social Work Assessments, which had been obtained prior to their admission to the home. The other person was admitted to the home some considerable time ago and the senior staff believes that the preadmission Woods Court Care Home C03 C53 S35542 Woods Crt V248045 070905 Stage 4.doc Version 1.40 Page 9 assessment has now been archived. She stated that residents would never be admitted without social work assessments. Intermediate care is provided in a separate unit within the main building of the home. This unit has specialist facilities, equipment and trained staff to provide intensive rehabilitation to enable residents to return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. Residents have an individual plan of care that appropriately identifies their personal, social and health care needs. The homes policies and procedures for the administration of medication are well maintained and appeared to provide adequate protection to residents, but the risk assessments for those people who administer their own medication must be reviewed regularly. Residents are treated with respect and their right to privacy is upheld. EVIDENCE: All of the Care Plans viewed during this inspection contained relevant information and were being reviewed and updated to reflect the individual residents changing needs. It was however noted that not all of the residents or their representatives were signing the care plans to confirm that they had been involved in the planning and review process. It is important that this information is included as it is helpful for staff to be aware of the residents’ lifestyle and interests, prior to their admission to the home. The care records show that peoples health care needs are being appropriately met. The resident’s relatives spoken with during the inspection stated that they believe that staff make appropriate referrals when medical intervention is required. The homes medication is stored safely and the records of receipt and disposal of medication were well maintained. The staff who administer medication have all received appropriate training. (Staff training records were sampled during the inspection). The control medication was checked at random and was well maintained. The senior staff was informed that staff must record the temperature in the room in which medication is stored on a daily basis. The temperature must not exceed 25°C as the medication can deteriorate and become less effective if stored at higher temperatures. The records of one resident, who administers some of her own medication, were checked and an appropriate risk assessment had been completed; however this had not been updated for over two years. It is important that medication risk assessments are reviewed regularly to ensure that the residents remain safe to continue to administer their own medication. All of the residents spoken with during this inspection said that staff are always friendly and respectful and that they ensure that their privacy and dignity is maintained at all times. The observed interaction between staff and residents was of a very good standard. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 Residents are encouraged to maintain contact with family and friends. EVIDENCE: The literature supplied prospective residents and their representatives contains details of the homes policy on maintaining relatives and friends involvement with residents. All of the people spoken with during this inspection, said that they can have visitors at any time and that their visitors are always made very welcome. Both of the visitors spoken with during this inspection said that they could visit at any time; one person said that he visits his wife three times a day. On one occasion, when his wife was ill, he stayed overnight and was provided with meals the following day. (This is good practice). Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18. Woods Court has a simple, clear and accessible complaints procedure. There have been no complaints made and Woods Court since the last inspection. Appropriate procedures are in place to protect residents from abuse. EVIDENCE: A copy of the homes complaints procedure is displayed in the main reception hall and is included in the literature supplied to all prospective residents. The home had not received any complaints since the last inspection. All of the residents spoken with were confident that if they had any complaints senior staff would deal with them appropriately. The home has an appropriate Whistle Blowing Procedure and a copy of the local Protection of Vulnerable Adults Procedure. At the last inspection some of the staff interviewed were unclear which agencies to contact if they needed to go higher than their own management team. Since that inspection every member of staff has been issued with a copy of the Whistle Blowing Procedure and they have signed to say that they have read and understood it. (This is good practice). The inspector was informed that there have been no allegations of abuse, in the home, in the last year. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,26. The purpose-built accommodation is maintained to a good standard. At the time of inspection the home is clean and there were no offensive odours. EVIDENCE: A partial tour of the premises was completed as part of this inspection. The accommodation is comfortably furnished and well decorated. The home has a written programme of decoration and refurbishment. (This is good practice). The residents spoken with during the inspection said that they are all very satisfied with their bedrooms, they confirmed that they were encouraged to bring photographs ornaments and small items of furniture to personalise their individual rooms. People said that they could use their rooms at any time. One person said that she chooses to have all her meals in her bedroom and that she never leaves the room without locking the door. The room is rather small and the staff informed the inspector that there are plans to extend it. At the time of the inspection the laundry door was left open and there were no staff in situ. The inspector expressed concern as residents; some of who may have dementia would have access to the laundry equipment and potentially to soiled or infected linen. The registered person must ensure that residents health and safety is not put at risk. It was also recommended that the relevant sections of the homes Infection Control Policy be displayed in the laundry. All parts of the home viewed during the inspection were appropriately clean and there were no offensive odours. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 Staff do not commenced employment until the appropriate POVA 1st checks have been completed. EVIDENCE: At the last inspection a requirement was made that a Protection of Vulnerable Adults check needs to be in place before staff commenced employment. The records of the most recently recruited members of staff were checked and there were written evidence that POVA1st checks and two written references had been obtained before the staff commenced employment. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,38. Additional work needs to be completed to ensure that the homes quality Assurance System contains all the required information. Appropriate radiator covers have now been fitted to all the radiators in the areas to which residents have access, ensuring that people are not at risk of burning themselves. EVIDENCE: At the last inspection a requirement was made that stakeholders, e.g. GPs District Nurses etc need to be consulted about the homes Quality Assurance System. The inspector was advised that the manager has started to prepare a questionnaire to be completed by visiting professionals. However the questionnaire was not available at the time of inspection. This will need to be completed and circulated to the visiting professionals in order to comply with this requirement.
