This inspection was carried out on 19th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

As at previous inspections the home met or exceeded all of the National Minimum Standards inspected. Residents spoken with were very satisfied with their care and spoke highly of the staff and the management. Staff spoken with demonstrated an understanding of the needs of the elderly and an enthusiasm for their jobs. The manager and staff are to be commended for providing a safe and happy home where residents can maintain as much independence as they are capable of.

The home continues to maintain high standards.

The only recommendation to arise from this inspection is to file receipts for monies spent with the service users balance sheets for ease of auditing.

The two-storey building provides three lounges and two dining areas, one in a pleasant conservatory setting. There are 30 bedrooms, all comfortably furnished and decorated to a high standard. Externally there are extensive well-maintained gardens and, to the front of the building, a good sized carpark. All parts of the home are accessible to service users. This inspection took place over three and a quarter hours and the process was made easy because of the excellent administrative systems at the home. There were no requirements or recommendations made at the previous inspection, on this occasion therefore, the focus was on those standards not assessed last time. The methods used included reading policies and records and discussions with service users and staff. A limited tour of the building also took place. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 2. Prospective service users will have excellent information on the home prior to admission and know exactly what services are included in the cost. EVIDENCE: The statement of purpose is produced in written format, on cassette tape and on CD. The terms and conditions agreement contains all the necessary elements to meet the standard. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10 Medication is administered in an appropriate manner and records completed. Service users are treated with respect and their privacy is upheld. EVIDENCE: The administration of medication was observed to carried out in a safe manner and records were completed at the time. Medication was stored in a proper lockable trolley. Service users spoken with said that staff were respectful and knocked on doors before entering rooms. Observations of interactions between staff and residents on the day confirmed this. Privacy curtains had been fitted inside bathroom doors so that should it be essential to open the door privacy would be maintained. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 and 15 Service users are positively encouraged to maintain links with families and friends. The dining areas were light, spacious and well laid out for ease of movement. The food was well presented. EVIDENCE: Service users spoken with said visitors were always made welcome at the home. Outings to places of local interest were arranged in consultation with service users; on the day of inspection several people were taken by mini-bus to the Peak Shopping Village. The mid-day meal served during the inspection looked appetising and service users confirmed that the standard of meals is consistently high. They also said that there were always alternatives to the menu, meals could be eaten at a place and time of their choice. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Service users are well protected from abuse. EVIDENCE: The home had satisfactory policies on the protection of vulnerable adults including a whistle blowers policy. These were available in the office and were also included in the staff handbook given to every employee. The home only handled small amounts of monies for service users and kept up to date accurate accounts. One service user was able to describe how she would make a complaint and said she was sure it would be taken seriously. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21,23,24 and 25. Wren Hall offers a high standard of safe accommodation with sufficient bathing facilities to meet service user need. The home was well decorated and clean and fresh without being clinical. EVIDENCE: The home had a variety of bathing facilities suitable to meet residents’ needs. Ceiling tracks were fitted in some bathrooms for the use of those who were not mobile. Fourteen single rooms and five double rooms had ensuite washing and toilet facilities. The bedrooms seen were well furnished to the service users taste and contained small items of personal furniture and ornaments. Service users expressed satisfaction with their rooms and communal areas. No health and safety hazards were observed during the tour of the building. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28,29 and 30. The service users are supported and protected by the staff recruitment policy, which is strictly adhered to. The home provides excellent training for staff. EVIDENCE: Those staff spoken with demonstrated a clear understanding of their roles and of the tasks they needed to perform. One service user said “ the staff know what they are doing and use the hoist so I’m not hurt”. 78 of the staff team hold an NVQ level 2 or above. The yearly training programme provided relevant courses for staff development. The staff personnel files examined contained all the information required by statute and all legal checks had been carried out. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,34 and 36. The manager is a well qualified competent person who takes pride in ensuring that the home continues to exceed the minimum standards. Staff are properly supervised and appraised yearly. EVIDENCE: The manager is a non-executive member of a primary healthcare trust, a panel member for East Midlands Quality Centre, which accredits organisations with the “investor in people “ award and is a member of the Practice Learning team for community nursing at University of Nottingham. This enables her to keep up to date with current practice issues, which in turn can be passed onto the staff team to the benefit of the residents. The home’s aim of maintaining a 95 occupancy rate is met and provides financial security.
