Conditions of registration: 1. Service users shall be within category LD Date of last inspection 8th June 2005 Brief Description of the Service: The Terrace, one of four homes situated within large grounds, runs independently to the others and has its own staff team. There is a large, pleasant rear garden which service users and staff have spent time personalising. The home is a short distance from the village of Bilsthorpe and has transport, which it shares with the other homes. The Terrace is on two floors with all bedrooms on the first floor. This is an overview of what the inspector found during the inspection. This is an unannounced inspection which took place on the 23rd November 2005. The method used was Case Tracking where 3 Service users were spoken with about their experiences and expectations of living at the home, with analysis of the records and talking with members of staff to ensure that those living at the home have their needs met and their health and welfare maintained appropriately. The Terrace care home for adults with a learning disability offers a safe environment in which service users are encouraged to develop their potential. Karen Tingle is the acting manager and is leading a competent and enthusiastic staff team to deliver a high quality of individualised care to people with complex needs. Karen Tingle, acting Manager and staff team members on duty assisted with this inspection, and present during some of the time was Sally Hamilton, acting Regional Manager, who is also the Manager of The Mews, one of the other units within the complex. What the service does well: What has improved since the last inspection?
Meals are prepared in the central kitchen area, but individuals within The Terrace are supported in preparing food within the unit’s own kitchen if they wish. Redecoration and refurbishment are continuing. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,5 Statement of Purpose, detailed care plans and individual assessments of prospective service user’s needs assist in making a choice about where to live. EVIDENCE: The home has a detailed Statement of Purpose, which gives all the information a prospective service user needs to help make a choice. Assessments are undertaken to determine if a trial period is required, also taken into consideration are the needs of the existing service users before any placement is offered. The case tracked service user files all contained a comprehensive care plan and each also had a written, signed contract with the home. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,8,9,10 Service users participate in all aspects of life in the home and know their needs are reflected in their individual plans. They are supported to be as independent as their abilities enable by the staff teams. EVIDENCE: The care plans case tracked are well written and detailed and included a negotiated timetable of personal activities which encourage the service user to take risks as part of an independent lifestyle. The service users spoken with said they were aware of the care plan and the activities planned in the timetables. Individualised choices were evidenced during the inspection with service users involved in activities they have chosen. The staff interactions also observed encouraged this choice. The service users have house chores to carry out and those who are able to do so keep their own rooms tidy. Evidence was seen of actions taken to assist service users to move on if they wish this. Others were seen of holidays organised on an individual, as well as group basis. The service users spoken with were positive about the holidays and informed the inspector how much they enjoyed them. Service users have completed satisfaction questionnaires and there are regular meetings, which do have minutes taken and issues from these meetings are addressed as was evidenced in the activities timetable for service users.

Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,13,14,15,16,17 The service users take part in appropriate leisure and other activities in the home and around the local community leading to personal development. Rights are respected and personal relationships are supported. EVIDENCE: The service offered by the home is of a high standard and encourages personal development of the individual service users. There are activities within the local community for those who choose to use these and within the home for those who choose to stay in. The service users rights were observed, during the inspection, to be respected and support was given to enable all to engage in activities safely. The staff spoken with said that this was one of the most important areas of their work, as this is when they develop the relationship with the service users and encourage them to personally develop their skills and activities. Service user achievements were seen within their own rooms, displayed on walls, but also in some communal areas around the home. Wycar Leys Terrace Care Home DS0000048262.V259377.R01.S.doc Version 5.1 Page 11 The activity centre on the ground floor contains a craft room, pottery room, music and games room where people come to teach from the local community. Aerobics are also taught regularly. The sensory room is not used at present as it is in need of refurbishment. Service users spoken with said they liked being in the games room, others said they preferred the privacy of their own room. Visits to relatives are encouraged and service users confirm that they do visit relatives, or have relatives visit them. There are, at present, no open relationships of a sexual nature within the home. However, the care plans were shown to identify this area of need and the privacy required by individuals. The service users spoken with were happy with the food which is prepared in the central kitchen. Since the last inspection, service users who want to, have the opportunity to prepare some food and drinks within the unit’s kitchen, with staff assistance. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 The staff team support the service users as they prefer to meet their physical and emotional health needs. The medication policy and procedures are of a high standard. EVIDENCE: Service users spoken with said they feel the staff help them in the way they want them to and daily records reflect how this is done and how the support helps to meet the physical and emotional needs identified in the care plan. As the service users have varying degrees of learning disability, the medication is given by the staff, who are trained to do this. Certificates in case tracked staff files evidence this. The Medication Administration Record (MAR) of the service users case tracked, were correctly completed. The medication was all stored correctly in the team leaders office, locked in the appropriate cupboard. The temperature of the room is monitored to ensure it does not exceed the 25 degrees Celsius. Individual service user MAR sheets are identified with a photograph, specific instructions regarding the needs of the service user and how they like to take their medication is included. All this information is laminated and kept with the MAR sheet.

JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Service users’ views are acted upon and they are protected from abuse. EVIDENCE: Wycar Leys have a well structured complaints procedure as well as having Service User meetings, which have minutes. The service users spoken with said they can tell Karen if they have a problem and that she listens to them. The staff team are aware of the Nottinghamshire Protection of Vulnerable Adults procedure and this was evidenced when talking with the staff as well as in staff records. The intended outcomes for Standards 24 – 30 are: 24. 25. 26. 27. 28. 29. 30. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,28,29,30 The majority of the rooms are homely, comfortable and safe with bedrooms suitable for the service user’s lifestyle. The home is also clean and hygienic and specialist equipment is available to assist with independence. EVIDENCE: The Terrace has had some redecoration and the unit is homely, comfortable and safe. The service users’ bedrooms are, mostly, suitable for their needs and lifestyles and the service users case tracked were anxious to show the inspector their individual rooms, which were appropriately furnished, warm and the service users said how satisfied they are with them and they were clean and tidy. One room, however, is in need of refurbishment. The room requires redecoration, and replacement of a dilapidated wardrobe and armchair with furniture of a more modern and “tough” type. The service user in this room when spoken with, said that the chair was very comfortable and “favourite”. This service user preferred to stay in the room. There is an activity centre, music and games room with specialist equipment. The sensory room is not in use at present and is awaiting redecoration. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,33,34,35,36 Staff training and supervision and the support of the acting Manager, benefit the service users. The home’s recruitment practices also support and protect them. EVIDENCE: Karen,the acting Manager, has just completed her National Vocational qualification at Level 4 and staff and service users spoken with are all satisfied with the support given by her. The staff teams are structured in a way to support the service users and regular supervision is evidenced in the staff files and also by the staff confirming this does take place. All the staff files seen during the inspection contain the appropriate Criminal Records Bureau Enhanced checks, two satisfactory references, and identification and the home has a robust and protective recruitment policy seen during the inspection and evidenced within the staff files. Staff spoken with said they felt competent to do their job and that there are opportunities for training within the organisation. The service users spoken with said they were satisfied with the staff teams and during the inspection, the positive rapport between the staff and service users was clearly seen. The banter between was positive and cheerful with staff and service users, in some instances, working together on a task. Service users benefit from the ethos, leadership and management approach of the home. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37,38,39,40,41,42,43, The home is well run, and the leadership approach safeguards the service users rights and best interests as well as health, safety and welfare. EVIDENCE: Karen, acting Manager, is supported in safeguarding the service users’ rights and welfare by the Management structure within Wycar Leys units. There is always an on-call regional manager available for support and advice, and an open and positive environment is promoted. This was observed during the inspection and helps the home to be well run and the service users to feel secure in their surroundings. Records analysed showed all the compulsory health and safety checks and records are kept up to date and maintenance of the home is carried out regularly. The service users spoken with during this inspection are satisfied that their views are taken into consideration “Karen helps me” “I can ask Karen about anything”. “I am being helped to find somewhere to move onto”. Care plans and activity plans are regularly reviewed and service users are aware of these reviews. The home has policies and procedures in place to serve the best interest of the service users. These were seen during the inspection and the staff were observed during the inspection carrying out the procedures in accordance with the policies.
