What the care home does well The service has responded promptly and comprehensively to the requirements and recommendations made at the key inspection. All staff administering medication have received refresher training and supervision to ensure their competency in this area. The two staff members directly observed were competent in their administration and recording. Staff members only administer medication for 1 of the 3 Units in the home. This enables them to become familiar with people and their individual medication requirements. We examined all the Medication Administration Records (MAR) including the Controlled Drugs Register. These were completed accurately and correct codes had been used on MAR to indicate when a person, for example, had refused medication. An explanation of codes to be used is clearly displayed on the medication room wall and staff members spoken with said that they felt more confident about this aspect of recording. Clear risk assessments are in place for all people living in the home who administer their own medication. Procedures are in place to monitor their ongoing safety in doing this. All medication stored in the medication room is now stored in locked cabinets and the key to the controlled drugs cabinet is now stored securely. Dates when creams and lotions have been opened are now recorded and their administration is recorded on the MAR. Each person`s `as required` medication has been reviewed and excess stocks of this medication have been returned to the pharmacy. This medication is now stored appropriately and not in the Controlled Drugs Cabinet.All aspects of medication practice have been reviewed and a new medication policy has been formulated. At the time of the inspection some aspects of this did not directly apply to Acorn Close but this has now been rectified by the registered manager. The manager has also liaised with the pharmacist to ensure that administration information which appears on the reverse of the `Nomad` dispensing boxes, corresponds exactly to that which appears on the MAR. This is to ensure that there is no confusion about the medication that any individual is taking. What the care home could do better: The service has met all the requirements and recommendations made at the key inspection in relation to medication storage, administration and recording. The home is purpose built. Bedrooms are located on the ground and first floors and first floor bedrooms can be accessed by passenger lift or stairs. The home is divided into three units, two for men and one for women, and these are separated by electronic locks. Each unit is self-contained with its own kitchen and living room area. This random inspection took place on 9th July 2009 between 13:15 and 14:45. The purpose of the inspection was to monitor the services response to requirements made at the key inspection in relation to medication storage, administration and recording. Before this inspection the service had investigated a potential error in administration identified at the key inspection and ascertained that it was an error in recording. Copies of risk assessments relating to people living at Acorn Close who administer their own medication were forwarded to the Commission, together with minutes of staff meetings which recorded that additional training and supervision in medication administration would be arranged for staff. Information concerning change of practice was also sent to the Commission prior to our visit. This covered a range of issues such as security of storage, recording, and procedures for auditing. At the inspection visit we reviewed all aspects of medication storage, administration and recording and spoke to the registered manager at length about the changes in practice put in place.
