Addenbrooke is a registered care home providing personal care to 35 service users who have dementia, who can be admitted over or under the age of 65. The home is situated close to the town of Gosport, the water front and some local amenities. Accommodation is arranged in four units, each with a number of bedrooms, a bathroom and toilet facilities, lounge, dining area and kitchenette. A passenger lift and stairs allow access between the two floors. All service users have single rooms that are fitted with wash hand basins. We looked at all the information that we have received, or asked for, since the last annual service review that took place on 31/10/2008. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a selfassessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people using the service and from other people with an interest in the service. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The results of the previous key inspection of the service on 12/10/2007 and of the last annual service review. What has this told us about the service? The homes annual quality assurance assessment (AQAA) was clear and gave us all the information we asked for. We looked at the information in the AQAA and our judgement is that the home is still providing a good service and that they know some of the further improvements they need to make. For example, the AQAA told us that the service could monitor discharge questionnaires more effectively and so a review of all questionnaires is taking place. The home has introduced new care plans which reflect the current needs of residents and incorporate all information in a single document. It has also introduced a Palliative Care training pack to enable staff to improve their knowledge and understanding of Palliative care. The AQAA told us that the home now has two full-time Social Care Support Workers in post. However the home sometimes have to use the Social Care Support Workers to cover Care Assistants hours where gaps are created by annual leave/ sickness/vacancies. This can limit the individual person centred activities which the home would like to achieve. The AQAA states that the service is looking to fill all vacant posts and plans to continue actively to encourage people who use the service to request activities at meetings, such as sweet tasting, pub nights and football. Changes have been made to the environment to ensure it continues to meet peoples needs. A large lounge has been separated into two smaller lounges, a stairlift has been fitted and new flooring laid in two of the units. We sent out survey questionnaires and received comments from five of the people who use the service, who were supported by a staff member to complete the questionnaire. We also received comments from four social and health care professionals and eight staff members. Three of the people who use the service indicated that they usually receive the care and support they need. The other two told us that they always do. Two said that the staff are always available when they need them. The other three said they usually are. Four people told us that the home usually arranges activities that they can take part in if they want. The other person said the home always does. All said that the home is Annual Service Review Page 4 of 7 always fresh and clean. Two of the social and health care professionals indicated that peoples social and health care needs are always properly monitored, reviewed and met by the service. The other two said they usually are. One told us that the homes managers and staff always have the right skills and experience to support peoples social and health care needs. Two said they usually do. One indicated that they sometimes do. Asked what does the service do well, one health and social care professional told us They ensure that they have all the necessary information prior to admission so that they can be sure that they can meet a residents needs. One told us the home Contact health care professionals if they have any concerns. Another health and social care professional said The service provides a good standard of care for people with dementia and the staff are all trained to look after people with difficult needs. The other wrote Good care. Getting better at end of life care and added that They need more trained staff. Asked what could the service do better, one health and social care professional told us Treat patients as individuals, rather than all the same. The other three indicated that the service always respond to the diverse needs of individual people. The AQAA told us that the service need to improve accessibility to the staff induction programme. This was also stated in the previous AQAA. Five of the staff who completed questionnaires said that their induction covered everything they needed to know to do the job before they started very well. Two others indicated that it mostlydid and one said it partly did. All told us that they are being given training that is relevant to their role and helps them understand and meet the individual needs of people. All said the training keeps them up to date with new ways of working and six said that it gives them enough knowledge about health and medication. Four told us they thought that staffing could be more consistent and the staff to resident ratio could be better. Three staff members commented that the training is good. All confirmed that their employer carried out checks, such as their CRB and references, before they started work. The AQAA states there have been two safeguarding referrals and investigations. The home has kept us informed when there are incidents affecting the welfare of people who use the service. These notifications indicate that the service liaises with external social and health care agencies and has taken appropriate actions to ensure the wellbeing of people who use the service. People who use the service and who took part in our survey all said there is someone they can speak to informally if they are not happy. One told us thay know how to make a formal complaint. The other four indicated that they did not and said that they would talk to someone, such as a staff member. All staff who completed our questionnaire said that they know what to do if someone has concerns about the home. Two of the social and health care professionals indicated that the service always respond appropriately if they or anotherperson have raised any concerns. One said it usually did. The other person indicated not applicable. We asked people who use the service what does the home do well. One person told us Everything. No complaintsand said they are Very happy. Another person said Look after me well. Try to feed me well. Another commented Anything I want done they consider it and that the home is a very nice place. Carers are very kind and understanding. One individual told us Everything really. Another person said staff Annual Service Review Page 5 of 7 pay attention to what I say. We asked what could the home do better. One person told us that they are satisfied with the service. Two said Nothing with one of them adding As long as there is someone around. Another person told us that staff do as much as they can. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 12th October 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
