Ashfield House is a purpose built residential home opened in September 2008. Registered as a residential home providing personal care for up to forty older people including people with dementia. Accommodation is spread over three floors. All bedrooms are single with en suite facilities, TV and telephone points, within each group there is a large lounge and dining area. There is a passenger lift, which provides people with access to all floors. Ashfield House is situated in the centre of a housing estate on the outskirtsof the town of Raunds. The home is run in partrnership with Northamptonshire County Council and Shaw Healthcare. Fees are agreed as part of the contract with Northamptonshire County Council and are based upon the assessed level of need and includes the cost of accommodation, meals, laundry and personal care. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection. This included: The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment tool that focuses on how well outcomes are being met for people using the service. It has also provided us with numerical information about the service. Information we have about how the service has managed any concerns or complaints. What the service has told us about things that have happened in the service; these are called notifications and are a legal requirement. The previous key inspection Relevant information from other organisations. What other people have told us about the service. What has this told us about the service? The home sent us their AQAA when we asked for it. It was clear and detailed giving us all the information we asked for. The homes manager continues to let us know about things that have happened since our last key inspection and this shows that they have managed issues well. The manager states within the AQAA that people using the service and their advocates are consulted in many ways to gain their views about the quality of the service. We hold regular residents meetings at least quarterly relatives meetings. We have comment books in each unit and a suggestion box. At least quarterly we ask residents to fill in our Living in the home questionnaires. I also have an open door policy to all of my residents if they need to speak to me and make a point of the first job of the day to walk around the units and speak to the residents. Equality and dignity is interwoven into all the aspects of the service from direct communication, support plans, activities, dietary requirements and staff approaches. The AQAA states that Our care plans are developed to take into account everyones individuality. We encourage relatives and residents participation and contribution to enable us to promote a persons own beliefs and choices, giving a high quality person centered approach to care. Residents are given information to enable them to make choices regarding attending church services, communion or any chosen religious or cultural belief. Staff training regarding policies, promoting equality and diversity promoting person centred care. Policies are made available for staff to read and sign that they understand. The data provided within the AQAA confirms that of the forty three staff working at the home nineteen hold National Vocational Qualifications (NVQ) in care at level 2 or above with others working towards their Qualification. The registered manager is reflective about the service and states the following when asked what the service does well. We offer a good high quality service with staff that are committed to the residents well being and are continuously monitoring and strive to improve and build on the good service that we already provide Annual Service Review Page 4 of 6 CQC has not received any direct complaints since the last inspection. However in reviewing information there has been an increase of notifications in relation to medication errors and safeguarding. The AQAA confirms that two complaints have been made to the service and investigated but not substantiated. Over the past twelve months there have been nine safeguarding investigations. Two of which have been referred to the CQC and are presently under investigation through the local Social Services Safeguarding Adults Team. There has also been three medication errors which has led to an internal enquiry on each incident within the home. What are we going to do as a result of this annual service review? The next inspection of this service will be based on the Fees and Frequency Regulations 2007 and the assessment of risk of the service. Further clarity will become evident as the new registration and inspection system under the Health and Social Care Act 2008 is confirmed. Our opinion of the service has changed and we may undertake random inspections to look at how outcomes are being met for people who use the service.
