What did we do for this annual service review? We looked at all the information we have received or asked for, since the last key inspection. This included: - The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment document that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. - Surveys returned to us by people who live at the home, and staff who work for the service. - Information we have about how the service has managed complaints. - What the service has told us about things that have happened to people living in the home. These are called Regulation 37 notifications and are a legal requirement. - The previous key inspection and the results of any other visits we have made to the service in the last 12 months. - relevant information from other organisations. - What other people have told us about the service. What has this told us about the service? The service sent us the AQAA when we asked for it. It was clear and gave us all the information we asked for. It told us of the improvements the service has made in the last 12 months. It told us about the planning currently being undertaken to improve the environment at Bosworth Court, particularly in improving the environment for people living at the home who have dementia. It told us of the training staff have attended. This has included the Gold Standard Framework, and Liverpool Care Pathway (end of life care). The employment of more Mental Health trained nurses has improved the support given to residents with dementia, and has helped other staff to understand and give effective support to people with mental health and dementia related conditions. The AQAA told us the service has received eight complaints in the last 12 months, of which three have been upheld. There has been one safeguarding referral made, and a safeguarding investigation took place as a result of the referral. All staff have undertaken training in Safeguarding Adults, and most staff have undertaken training in the Mental Capacity Act. A programme of training to inform staff of Deprivation of Liberty Safeguards is being rolled out to staff. The AQAA told us that there are 22 full time care staff/registered nurses working at Bosworth Court, and nine part time care staff/registered nurses. Twenty four care staff are permanent workers. We were told that when the service has to use agency staff, it Annual Service Review Page 4 of 7 asks for staff who have worked regularly at the home to ensure continuity of care for service users. The service currently has a stable staff group. We looked at the Regulation 37 notifications. The service notified us of an error made by an agency member of staff whilst administering medication. We were satisfied that the service took the appropriate action to ensure the resident was safe, and in the subsequent action regarding the member of staff. We have received no complaints about the service, or any negative information from other organisations regarding the care of people at Bosworth Court. We received 10 surveys from people who live at Bosworth Court. Nine residents told us that they always receive the care and support they need, with one saying they usually receive the support they need. Two said that staff were always there when needed, and eight said staff were usually there when needed. All said they always get the medical care they need. The majority of residents said they usually enjoy the meals at the home, and said that they can usually take part in activities arranged by the service. All residents said they felt able to speak informally to someone if they were not happy, although two did not know how to make a formal complaint. All said the home was always fresh and clean. Typical comments about what the service does well were: Im well fed and looked after staff all friendly and helpful they look after me to the best of their ability Two people said the home could do better by helping them go out a little more, and having more social get togethers. We received three surveys from relatives. These were all positive about the care their relative is receiving. One said: The service provided by the staff is excellent, they are cheerful and provide a pleasant atmosphere in the home, are always helpful and polite..the food is excellent and well presented. One relative thought the service could do better by: trying to involve some residents in activities instead of just sitting staring into space We received three responses from staff. All were positive about the service and the support they receive to do their jobs well. They felt they were given the training needed to support people living in the home, and there were enough staff to support residents. What are we going to do as a result of this annual service review? Annual Service Review Page 5 of 7 The next inspection of this service will be based on the Fees and Frequency Regulations 2007, and the assessment of risk of the service. We will be able to give more information about this once the new registration and inspection system under the Health and Social Care Act 2008 is agreed.
