It is located in a suburb to the west of Southampton, Maybush with reasonable local transport links. The home makes provision for three separate services, Orthopaedic Rehabilitation, Social Rehabilitation and Residential/Respite Care. The first two services are classified as intermediate care, whilst the later as stated is residential. All accommodation in the home is single occupancy with many rooms having en-suite facilities, especially those within the newer Orthopaedic Unit. Respite care costs are established following assessment, there is no charge for intermediate care (rehabilitation), services for up to six weeks. What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: The annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some numerical information about the service. Surveys returned to us by people with an interest in the service. What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement. The previous key inspection and the results of any other visits that we have made to the service in the last 12 months. Other information we have received about the service. What has this told us about the service? The home sent us their Annual Quality Assurance Assessment (AQAA) when we asked for it. The AQAA was completed by the service manager as the home does not currently have a registered manager. The AQAA must be completed by the Registered Person; in this case the Responsible Individual. The AQAA provided some but not all of the information that was required. The previous inspection was undertaken in December 2008. One recommendation was made following that inspection. This was in relation to activities. The report identified that the home was not providing many leisure activities for people. The AQAA contained little information about how the home had addressed this requirement and what activities are now provided. The AQAA stated that they could provide a better variety of activities and that they were looking at trying to get more outside entertainment for the home. Five surveys were received from people who live at the home, four stated always and one usually that the home arranges activities that they could take part in. We sent comment cards (surveys) to the home to give to people who live at the home to complete. We received five completed comment cards. These showed that people are happy with the service they receive at the home. Some people added comments including I am happy, I am glad I choose Brownhill. Another stated I cant fault this in any way, its clean and Im looked after always. Service users also commented about the care staff at the home stating that they are caring and considerate, picking up when I need assistance and help, give us every care and attention immediately and willingly. We sent surveys to the home for staff to complete. No completed staff surveys had Annual Service Review Page 4 of 6 been received at the time of completing this ASR. We also sent surveys to the home to give to external health and social care professionals. No completed surveys have been received from them either. The AQAA did not give information about the number of care staff who have an NVQ in care. There was also little information about the training that staff have undertaken. The AQAA identified that the home has a number of staff vacancies and that they are using agency staff as they are unable to recruit to these positions as they will be filled by staff who are currently working in other homes owned by the council that are due to close in the future. The home is also without a registered manager. The previous registered manager left the homes employment in June 2009. The home are also not intending on recruiting a new manager as the manager from a home that is due to close is to become the homes registered manager. The AQAA did not state how long the home will be without a registered manager or relying on agency staff. The previous report identified that there had been a number of medication errors but that the home had taken action and these had reduced. Since the previous inspection the home has continued to notify the commission about medication errors. In April 2009 a commission pharmacy inspector visited the home to undertake a random inspection focusing on the homes medication management systems. This concluded that the home was managing medication appropriately and had taken action to ensure that people receive their medication as prescribed. The random report also identified that the home did not have any procedures in place for people who may require medication for minor ailments such as headaches. There was no information in the AQAA about the action the home had taken to address this issue. Since the pharmacy inspection in April there have been further notifications of medication errors at the home. No concerns or complaints were raised in any of the surveys received by the people who live at the home. Four of the five people who completed comment cards stated that they were aware of the homes complaints procedure. What are we going to do as a result of this annual service review? There is no evidence to indicate that the service is not continuing to provide good outcomes for people who live there however we will continue to monitor the home as it does not have a registered manager and medication errors continue to occur despite the measures that have been put in place to prevent these. We are not going to change our inspection schedule and will undertake a key inspection before 16th November 2010, however we may inspect the service at any time should we decide we need to.
