Centenary House currenty provides residential care services for up to 13 people over the age of 65. The home faces a busy road but is within walking distance to the centre of Shepton Mallet. There is a convenience shop in a garage next door and a large supermarket nearby. There is car parking in front of the home. The home comprises the original house and a later extension. All service users rooms are on the ground floor. The first floor is currently unoccupied. Many of the bedrooms have individual access to a pleasant garden and two have access to a side patio. There is a double bedroom with en-suite facilities. There are two lounges, a pleasant conservatory overlooking the garden and a dining room. There are two communal bathrooms, one has been redecorated and fitted with a spa bath. The shower room has been refurbished and now has a good sized walk in shower cubicle. The fees at the home are 390 to 430 pounds per week.

What the care home does well Some people who live at the home commented "staff are lovely" and "food is good". we observed some good interactions between some members of staff and people who live at the home. What the care home could do better: The management must ensure that all staff have received all necessary training. In particularly medication training for those staff who work alone on nights and moving and handling training for all staff. The staffing rota`s need to reflect the staff on duty at all times. This should include details of staff on call at night and those "covering" for kitchen and domestic staff. Care Homes for Older People Page 3 of 9 What we found: This inspection was conducted by one inspector over four hours. The purpose of the inspection visit was to assess progress towards meeting the outstanding requirements made at previous inspections. As part of this inspection we spoke with people who live at the home, management and staff. The home sent us an Annual Quality Assurance Assessment (AQAA) in March 2010. Information from the AQAA, was been used throughout the inspection process. Information held in the AQAA was confirmed during the inspection visit. During the inspection visit we reviewed the care and support plans for two people living at the home. On this occasion improvements to the care plans and assessments were found. The manager has developed a new care planning system since the last key inspection. The plans contained a range of information giving guidance to the staff on the needs of the individuals. The plans need to be kept under review however to ensure that the information is not ambiguous and is clear in all areas. We spoke to a number of people living at the home during the inspection visit. All were very complimentary about the care and support that they received. We reviewed the lunch time meal arrangements. No choice was on offer but all people spoken to were happy with the fish which was served as part of the main meal. The inspection visit was completed on a Friday and people told us that they always had fish on a Friday. We observed that prior to the main course soup was served. Staff then asked people if they preferred mashed potato or chips. Beans or peas were also available. A desert of peaches and cream was then served. The tables were nicely set with table cloths and glasses. Water was available during the meal. We completed a tour of the building. All rooms seen were personalised to peoples individuals taste and belongings. The rooms were clean and tidy and free from odours. At the last inspection it was found that the storage of documentation did not comply with the Data Protection Act with personal information of both staff and people living at the home being freely available in an upstairs store room. This had improved on this inspection visit. All information is now securely stored. At the last inspection it was clear that the owner of the home was not recording her visits in line with Regulation 26 of the Care Home Regulations. At this time there was no clear action plans developed for any issues identified. On this inspection visit we observed that the Regulation 26 visit are now conducted and documented. The manager and owner now discuss any actions required as a result of the visit and a clear action plan is developed. The Regulation 26 visit now include discussion with people living at the home as part of the home Quality Assurance systems. During the inspection we reviewed the staff duty rotas. These demonstrated that two care staff are on duty during day time hours. One waking staff member is available over night. There is an additional staff member on call over the night time hours. This staff member lives in separate accommodation. The duty rotas did not show arrangements for the on call when the live in staff member was having a day off. In addition to the care Care Homes for Older People Page 4 of 9 staff the home employs a cook and domestic staff member. The duty rotas showed that these staff have regular days off. The rotas however did not show who undertook the cooking and cleaning when the normal staff members were not available. we reviewed the homes training matrix after the site visit. When compared to the duty rotas not all staff employed were detailed on the training matrix. The training that these staff have undertaken could not be confirmed. In addition not all staff have undertaken moving and handling training and a significant number have not undertaken refresher training in the last year. The training matrix showed that not all staff have undertaken medication training. This included some staff who lone work on nights. It could not be confirmed therefore how these staff could safely administer any medication required at night. The home manager is yet to Register with us. This is required to ensure that the manager has the skills and competencies to fulfill her role. An application to register was received by us however was returned as it was not complete.
