We give a quality rating following a full review of the service. We call this review a ‘key’ inspection We do an annual service review when there has been no key inspection of the service in the last 12 months. It does not involve a visit to the service but is a summary of new information given to us, or collected by us, since the last key inspection or annual service review.   Has this annual service review changed our opinion of the service?   Yes You should read the last key inspection report for this service to get a full picture of how well outcomes for the people using the service are being met. The home is registered to provide support and accommodation for up to five adults with a learning disability. There are currently four residents living at the home and one vacancy. There is a large open plan dining room and living room and the kitchen is domestic in scale. There is no lift to the first floor, however there is an adapted bathroom on the ground floor for a wheelchair user and residents with some mobility needs. There is a wheelchair user that has a bedroom on the ground floor. The home would not be suitable for other wheelchair users because Annual Service Review Page 2 of 6 all other bedrooms are on the first floor. The home is not of a size that could accommodate a lift. Nottinghamshire Community Housing Association own and run the care home. At the time of writing this key inspection report the basic fee for living at the home is £361 per week but this varies upon the benefits a resident is entitled to and prices are available when applying for a place. Copies of inspection reports and results of their own quality monitoring are available to residents and other stakeholders by request. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received from 53 Church Street, or asked them for since the last key inspection. This included: The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service, and was received in December 2009. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some statistical and numerical information about the service. Information we have about 53 Church Street on the QA system has been reviewed, including any complaints or concerns that have been raised. The last key inspection report and the inspection record have been reviewed The service folder was checked for any other relevant information. We also looked at any relevant information that we have received about 53 Church Street from other organisations. We looked at what other people have told us about the service. What has this told us about the service? The AQAA was returned within the time scale asked for, and had been completed in a professional manner, providing all of the information asked for, and in sufficient depth to allow judgements to be made. Among the improvements to the service identified in the AQAA were: We have continued to build on the knowledge and needs of service users, with involvement of family members. The staff team are recognising and offering service users more choices and involvement in the day to day running of the home. We have held more 1-1 meetings, for individual choices, addressed decorations and purchasing of new furniture, and crockery. We have offered more activities weather related, e.g. gardening. New staff have inputted to new activities. Built on support plans for family and friends, and actioned/planned outings. Worked well with day services to offer support during changes in the service. Implemented H.A.P. for all service users. Auctioned Quality Tree surveys and introduced Health M.O.Ts, addressing all required support plans, Introduced a 4 week planner to ensure M.O.T.s are carried out. Worked well as a team to introduce new staff members. More 1-1 consultations. We have met all of the goals set in our Quality tree, e.g. changes to lighting, new shed/shelter. Refurbished bathroom with service user involvement, and decorated other areas. Service users have helped to choose plants and helped to lay new flower beds, and grown tomatoes, and maintain the upkeep of the grounds and garden pots. We have purchased some new items of furniture, for communal and personal use. New French doors have much improved the warmth and appearance of communal areas. The Commission has not received any complaints or concerns regarding this service since the last key inspection. In their AQAA they stated that they had received one complaint in the last 12 months, which had been investigated in line with the care homes complaints procedure and was not upheld. No referrals have been made under safeguarding adults procedures in the last twelve months. We have received no notification forms during the past twelve months to report any incidents that have affected the wellbeing of the people who live at the care home or Annual Service Review Page 4 of 6 the staff. At the last key inspection this service was rated as being a good service. We sent out five questionnaires to people who live at the home, and five to staff. of these we received all five back from people who live at 53 Church street, and two from staff. Among the comments received from people who live at the care home were the following: They meet my needs, and assist me on an annual holiday, they keep me safe and well, and assist me in outings at weekends. It would be better if they could provide a vehicle which is more suitable and more accessible to places as this does restrict where I can go. They look after me. The minibus is noisy, and sometimes we cant get in because it is too high, and it keeps rattling, and it gets on my nerves when I sit in it for a long time. They support me to do the things that I want to do. It would be better if there was more time to take me out to see trains (my favourite hobby.) They look after me, and help care for me when I am ill, and have to go to the hospital. They look after me well every day, keep me safe and offer me choices. Staff are friendly, they help me to make decisions, and always support me. A higher staffing ratio at weekends when Im not at the Day Centre would be better. Comments from staff included: The home is very good all service users are well looked after. The staff are trained very well. We could sometimes do with more staff at the weekends. The day to day and long term care of the residents is very good. We put the interests of the residents first. The rota - particularly staffing at weekends could be better. I enjoy my job, but I do struggle with the IT, and I am finding that the job involves more and more work on the computer. There are no ongoing concerns about the quality of care that 53 Church Street offers, and a review of the documentation that has been received does not highlight any issues that need to be addressed. As a result of all the information received we have decided not to alter the inspection date for the service. What are we going to do as a result of this annual service review? The next inspection of this service will be based on the Fess and Frequency Regulations 2007 and the assessment of risk of the service. Further clarity will become evident as the new registration and inspection system under the Health and Social Care Act 2008 is confirmed.
