Coldwells House is a detached late nineteenth century period house on the outskirts of Hereford city. Although the home is close to the main A49 road it is in a small, rural lane and there are good views of the surrounding countryside from many windows. The main building has been adapted for its current use and a purpose-designed extension was built in 1995. The service has twenty eight registered places, although only 22 places were in use at the time of this inspection due to refurbishment work. The service caters for people over the age of sixty five years who have care needs arising from the general ageing process, dementia, physical disability or mental disorder. All the bedrooms are single occupancy and thirteen have en-suite facilities. The schedule of fees is available from the home. What the care home does well The focus of this inspection was to check if requirements had been met. Because of this we did not look at all aspects of the service and may therefore not have identified some positive things about the home. The home has made all the improvements which we identified at the previous inspection. Risk assessments are carried out whenever a risk has been identified, and care plans show the ways in which any risk can be minimised. We saw that care plans are personcentred. This means that they focus on people`s individual needs, wishes and preferences. The home has good procedures for managing medication. People can be confident that they will receive any medication which has been prescribed for them, and that accurate records will be kept. Infection control is managed well, so as to limit the risk of any infection spreading within the home. The manager is making effective use of the staff team, so that people can have their needs met in a timely manner. What the care home could do better: We have not identified any areas at this inspection where the home needs to make improvements. We will carry out a full inspection of the service before 8 December 2009. Care Homes for Older People Page 3 of 8 What we found: We carried out this inspection in order to check whether the home had complied with the requirements which we had set at the key inspection on 8 December 2008. We had required the home to make sure that staff are aware of the outcome of any risk assessment, and use this information to make sure people who use the service are not placed at risk. We looked in detail at the records for two people living at the home. We saw that the home has carried out comprehensive assessments of any risks which have been identified, and records showed that the staff were using this information when providing care. We saw that the new manager has made sure that people living at the home have care plans which clearly state exactly what care needs to be provided, and how this should be done in order to promote peoples dignity and privacy. We saw that one persons care plan for personal hygiene stated encourage A (name of person) to try to wash and dry her hands and face and any other areas she can manage staff not to leave As body unnecessarily exposed and staff to give clear explanations. The home was required to make sure that the use of any specialist moving and handling equipment, such as a hoist, was governed by a full written risk assessment. We saw that one person needs a hoist in order to move. The records showed that a full risk assessment had been carried out, and the care plan explained to staff exactly how to assist the person to move and how to use the equipment. We looked at the way in which the home manages medication, as this had been an area where there was a high level of risk at the previous inspection. We saw that the new manager had put into place all the improvements which we had required the home to make. The home has put into place a new system for ordering medication. This helps to ensure that the home will not run out of peoples medication and that the stock levels of medications are monitored. We saw records which showed that the manager has assessed the competency of all staff who administer medication. We looked at records of medication for three people. These showed that all medication is recorded accurately, which means that people can be confident that they will receive the medication which has been prescribed for them. Where people are taking medication on an as required basis (such as painkillers), the home has recorded full details about how this medication should be used, including information about variable doses, the maximum dosage in 24 hours, the reason for giving the medication, and any special instructions. We saw that the home has new storage for controlled drugs, which meets all the legal requirements. At the previous inspection, we saw that some practices for administering medication were unsafe and could have put people at risk. This inspection showed that the home has made changes to the way they administer medication, and the safe practices in use at the home now mean that people are no longer at risk. Care Homes for Older People Page 4 of 8 At the previous inspection, we had seen that the homes procedures for dealing with laundry meant that clean and dirty laundry could have come into contact with each other. This increases the risk of cross-infection. We saw that the laundry is now kept completely separate so that dirty laundry cannot come into contact with clean laundry. The previous inspection showed that staff were not always meeting peoples needs in a timely manner. The new manager told us that the staffing levels remain the same, but that she has trained the senior staff in more effective use of staff. She also told us that she personally checks that all the required tasks have been done.
