Crossways Residential Home provides accommodation for twenty- three people over the age of sixty -five, twelve of whom may have dementia. The home does not provide nursing care. The home is approximately two miles from Walsall town centre. There are nineteen single bedrooms and two double bedrooms. Twelve bedrooms have an en suite toilet. The home has a passenger lift, a staff call system throughout the home and aids and adaptations to assist people. There is a pleasant garden at the back of the home. Car parking is available at the front of the home. The fee range was not clear in the service user guide seen. What the care home does well The home has started to address some of the concerns highlighted at the previous inspection, although there remains a considerable amount of work still to be undertaken. The home has a stable and caring staff group. What the care home could do better: Care plans did not adequately detail people`s needs and how they should be met. We also found that when people had behavioural challenging needs, information on what escalated their behaviour was not in their plan of care. Some care plans are preprinted with standard care instructions without including information that relates to the person requiring their care. There is a need for care plans to be individual and include information about the individual their choices, preferences and capabilities. We found that there was not a record of all accidents that people who were living in the home had sustained. People are not weighed regularly meaning that their health is not adequately being monitored. Appropriate equipment should be available to enable people to be weighed and their weight monitored frequently. Incidents such as falls and admissions to hospital or anything that affects people`s health and wellbeing should be notified to the Commission for Social Care Inspection. The home must also make safeguarding referrals when there are allegations of any abuse. The home must undertake a risk assessment to confirm the adequacy of the checks when staff are employed with a Protection of Vulnerable Adults check and before the criminal records check is returned as required. This is in order to protect the people who use the service from potenially unsuitable staff. . There is not sufficient staff to meet peoples needs at the home, keep them occupied and also minimise risks to them including falling. Care Homes for Older People Page 3 of 9 What we found: The last inspection of this service was on the 19th June 2008, at this inspection the home was made no star POOR. The previous inspection report should be read in conjuction with this report which was a follow up visit to assess the homes compliance with requirements made as a result of the inspection. The service is updating their service user guide as recommended at the last inspection. The service user guide that was available at the time of the inspection contained information relating to the inspection in June 2007 and not the inspection in June 2008. This means that people do not have up to date information about the service available. All people had a plan of care but care plans seen did not adequately detail peoples needs and how they should be met. We also found that when people had behavioural challenging needs, information on what escalated their behaviour was not in their plan of care. Some care plans were pre printed with standard care instructions without including information that relates to the person requiring their care. Care plans should be individual and include information about the individual their choices, preferences and capabilities. We found that there had been incidents such as falls and admissions to hospital which we have not been notified of as required. The home had also failed to make safeguarding referrals when there were incidents between people living at the home as they are required to. The home has met a previous requirement that we made in relation to moving and handling equipment. The Manager told us that new slings are available but they no longer have anyone that needs moving in the hoist. It is reassuring that should people need to be hoisted appropriate equipment is available in the home to do this safely. We found that people living at the home are not weighed very often. Records of people weights that was given to us by the Home Manager showed that: two of the nineteen people living at the home were weighed in Septembern2008, no one was weighed in August 2008, five people were weighed in July 2008 and again no people were weighed in June 2008. Staff told us that they are unable to weigh most people as they are unable stand on the scales that the home has. The owner of the home had told us at the previous visit to the home that she had ordered new scales that people can sit on making it easier for them to be weighed. There was a fault on the fire alarm system that was awaiting repair despite this being a fault when the Fire Officer visited in September 2008. This will be checked again by the Fire Officer. The home has met a previous requirement that we made that staff are only employed when all required checks have been returned. The home does not however undertake a risk assessment to confirm the adequacy of the checks when staff are employed with a Protection of Vulnerable Adults check and before the criminal records check is returned as required. Care Homes for Older People Page 4 of 9 During the day the home has three staff and the manager on duty although the Manager is not on duty every day. There are two staff on duty at night to care for the nineteen people living at the home. We do not feel that there are sufficient staff given the dependency of people living at the home to meet their needs, kept them occupied and also minimise the risks such as falling. People should be consulted about their care needs and a record of this should also be recorded. An accurate and up to date plan of care will provide instructions for staff on peoples needs and how they should be met. The registered person must 31/07/2008 ensure that systems are in place to ensure that there are no unnecessary risks to the health or safety of service users are identified and so far as possible eliminated. This will include risks from falls and depression from lack of social stimulation. The availability of staff at the 31/07/2008 home and their skills should be sufficient and appropriate to meet peoples needs.
