Duxbury House provides care to six adults who have a learning disability and/or mental health problems. The property is a terraced house situated in the North Shore area of Blackpool, close to the sea front and local amenities. All service-users are provided with single accommodation, There are six single bedrooms. These are not ensuite. There is a communal lounge and dining area and a large paved area at the rear of the property. There is a Statement of Purpose/Service User Guide, which is given to all prospective residents. This written information explains the care service that is offered, who the owners and staff are, and what the resident can expect if he or she decides to live at the home. Information received on this inspection showed that the fees for care at the home are usually from three hundred and twenty pounds to four hundred and fifty pounds per week. However the fees may be more where a person needs a lot of extra one to one care. There are added expenses for hairdressing, chiropody, newspapers, outings and holidays.

What the care home does well The residents in this home are very well cared for. We observed staff chatting to residents frequently. Staff were cheerful and friendly as they went about their duties. Staff meet residents to assess their health and personal care and support needs before deciding if the home can meet their needs effectively. Staff are knowledgeable about residents needs and residents said they are very happy in the home. One resident said, "They look after me well here. They always check I am ok." Another resident said "I am very happy here, very settled." Care plans are in place,residents health needs are well met and medication is managed well. Routines in the home are flexible and residents spoken to said that they can choose when they get up and go to bed and what they want to do during the day. Meals are varied and well presented. Residents feel they are listened to and any concerns acted upon. The home is clean, tidy and well maintained making Duxbury House a pleasant place to live. One resident said, "I like it here and I like my own room." Staff training is good. This keeps staff skills and knowledge up to date. The manager is approachable and residents and relatives said he is always willing to help. Systems are in place for quality assurance to ensure care practice is up to date and effective. The manager regularly talks to residents and relatives to check they are happy with the care provided. What the care home could do better: All relevant health information should be recorded in residents care records so that all involved have access to detailed information. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: The reason for this inspection was to check that the excellent 3 star rating given at the last key inspection is still valid and that nothing has changed. We looked at all the key standards. The outcome is as follows and shows that the outcome for residents is still excellent and the 3 star rating is still valid. We case tracked one of the residents. This included looking at the case tracked residents care records and the environment they live in, talking to the resident and the staff supporting them and observing staff interactions with them. This is not to the exclusion of other residents who were also spoken to. Prospective residents are assessed, before admission so that staff are aware of their specific care needs and know whether they can meet these. The resident had an assessment of care needs, which was in place before the resident was admitted to the home. The home is jointly owned by Mr Fitzgerald and his partner. He manages the home on a daily basis. Care plans and risk assessments were in place. They had been regularly reviewed and the information in place was up to date. This includes each residents care needs, leisure choices and likes and dislikes, as well as information on professional visits. There were regular individual records showing any changes in the resident and any issues that may have arisen. One resident has a recent health issue. This has not yet been written in the residents records. We discussed the need for this information to be in the records. The manager agreed that he would provide basic information in the care records with more information available as required by the resident or health care professionals. Residents have access to appropriate health services. The residents attend their GP as needed and if necessary they receive GP and District Nurses visits. If residents have a health appointment, the manager or member of the staff team will go with them to support them unless the residents choose for them not to. If a resident has to go into hospital, the manager will spend a lot of time with them at the hospital to ensure their individual needs are met. The manager will advocate on the residents behalf, where he believes their care needs are not being met. One resident said, The staff are all so good. They go with us (to appointments) when we want them. None of the residents had been involved in any accident recently. The manager is aware of the need to audit to see if any lessons could be learnt or accidents reduced if accidents/falls occurred. Medication was checked and was appropriately administered, recorded, stored and disposed of. The home uses a monitored dosage system and is satisfied that it is working well. Residents were very positive about the care they received. One resident said, They look after me well here. They always check I am ok Another resident said I am very happy here, very settled. Residents are encouraged to make choices and routines are flexible with staff encouraging residents to make decisions, with support as needed Care Homes for Adults (18-65 years) Page 4 of 9 about what they want to do. Another resident said, They help me do things I find difficult. Each resident is involved in regular activities of their choosing and said they enjoyed them. Routines in the home are flexible and residents spoken to said that they can choose when they get up and go to bed and what they want to do during the day. Some residents go out on their own, others prefer the support of staff when they go out. All residents regularly have the opportunity to go out alone to an activity of their choice with one of the staff team. This allows them undisturbed time to chat and discuss any concerns they may not want to discuss with other people around. One resident said, Me and Vince (Mr Fitzgerald the owner/manager) go out sometimes. Its good. Another resident said, We went out shopping for new things. I enjoyed it. Meal times are flexible. Staff are familiar with residents likes and dislikes and meals are planned together where possible. We were able to discreetly observe staff and residents during the inspection. There was very frequent, friendly interaction between staff and residents. Residents said they are treated with respect by staff. Residents spoken to said they knew who to complain to if they had any concerns but they were happy in Duxbury House. Residents spoken to said they would tell Vince (the owner/manager) if they had any concerns. There have been no complaints since the last inspection. The staff team chat frequently with residents and check if there are any minor niggles and deals with these quickly so they do not become big problems. The manager said staff have received safeguarding adults training and have an understanding of the action to be taken if they are alerted to a safeguarding issue. We toured the home and found the environment comfortable, homely, clean, tidy and well maintained. Residents said the home was small and nice like any other house and much better than the big wards. One resident added, I like it here and I like my own room. There is a private back yard with patio furniture used in the summer where residents sit and relax. Mr Fitzgerald has continued with a rolling programme of redecoration. The house has been painted externally and the hall and downstairs bedroom has been repainted and a new bedroom carpet. Residents spoken to as we toured the building were complimentary about the staff. One resident said of Duxbury House, It is almost as good as being in your own house, better in some ways because you have help here. Another resident said, Everyone is so kind . I am settled here. I want to stay. There are only three staff including the owners and they have worked together in the home for several years. This ensures that staff are very familiar with residents and their care needs and residents know the people caring for them. Staff rotas showed sufficient staff on duty. The owner and a care staff were in the home during the unannounced inspection. Residents felt that there were sufficient numbers of staff on duty to provide good care. Staff recruitment records were not checked. there had been no new members of staff since the last inspection and records were satisfactory for these staff when last checked. Staff training information was seen and was satisfactory. Care Homes for Adults (18-65 years) Page 5 of 9 The owner/manager, Mr Fitzgerald has completed National Vocational Qualifications (NVQ) in care and management and the registered managers award and one of the staff team has completed NVQ. This is very positive. These are national awards in care that enable staff to gain up to date skills and knowledge about current care practice and management for the higher levels of the qualifications. The fire risk assessment is in place and up to date. There are regular fire alarm and emergency light checks and appropriate health and safety checks are in place. The recent inspection by the fire service had made some requirements about repainting doors with fireproof paint and repairing one door closure. This has now been done.
