If yes, what have they been: Date of last key inspection: Date of last annual service review (if applicable): Brief description of the service Eastfields offers planned short breaks for adults with a learning disability aged between 18 to 65 that live with their family or carers. This service enables parents or carers to have a rest or go on holiday. Eastfields has 2 lounges, a games room, a conservatory a kitchen and laundry. There is one bathroom, shower and toilet upstairs and a bathroom, walk in shower and toilet downstairs. There are 3 downstairs bedrooms and 5 upstairs bedrooms.There is a garden at the front and back of the house. The cost to stay at Eastfields depends on your age and your savings if they are over £22,250. At the time of this visit the cost for each night ranged from £7.53 to £53.03. What did we do for this annual service review? We looked at all the information that we have received from Eastfields, or asked them for, since the last key inspection. This included: The Annual Quality Assurance Assessment (AQAA) that was sent to us by the service, and was received in August 2009. The AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service. It also gave us some statistical and numerical information about the service. Information we have about Eastfields on the QA system has been reviewed, including any complaints or concerns that have been raised. The last key inspection report and the inspection record have been reviewed The service folder was reviewed for any other relevant information. We also looked at any relevant information that we have received about Eastfields from other organisations. We looked at what any other people have told us about the service. What has this told us about the service? The AQAA was returned within the time scale asked for. It was completed in a very full, complete and professional manner and provided sufficient depth to allow quality judgements to be made. Among the improvements to the service identified in the AQAA were: We have ensured that we continually network with the other short break services to better utilise the resources we have across the area. We have assisted families with alternatives to short breaks e.g. use of direct payments finding long term placements. The referral pack for Eastfields has been updated to clearly explain the referral process in an accessible format. We have introduced a system of review for individual support plans to ensure they are kept up to date and current. We have been supporting people who are already self medicating to some degree at home to do so whilst staying at the service and have better control and independance with manageing their own monies, we have installed lockable cabinets in bed rooms so medication and perosnal monies can be dealt with in private. Where individuals have been assessed as able to self medicate, self manage their own monies or both they are given the keys to cabinets in the bedrooms. A number of people accessing are self medication or self manging their own monies. Introduced a person centred medication record for people whio are self medicating to use. We have replaced the flooring in the dining room downstairs hallway, bedroom two, lounge one and office. We have had blinds fitted to the office in the bungalow end of the building and bathroom. We have replaced all the curtains upstairs, in the hallway and lounge one. We have replaced the dining room wall unit and replaced the settes in lounge two. We have decorated both lounges and the games room. The Commission has not received any complaints about this service since the last key inspection. In their AQAA the agency stated they had received one complaint in the last twelve months, which had been investigated in line with the agencys complaints procedure, and was upheld. Two referrals have been made under safeguarding adults procedures in the last twelve months. We sent out five questionnaires to people who use this service, and five to relatives, Annual Service Review Page 4 of 6 and another five to members of staff, to gain their views on the service and how well it is meeting peoples needs. Of these three were returned by people who use the service, one by a relative and five by members of staff. Information provided in the questionnaires has helped with the judgements made in this Annual Service Review. Among the comments received from people who use the service were: It provides interesting outings. They feed me well, and Im very comfortable there. i like meeting other service users. I enjoy all my respite time at Eastfields. Its clean, the staff are nice and its warm. Comments received from a relative included: They help my son feel comfortable and happy in a safe environment. Comments received from staff included the following: We all care for the individuals we support in a way they like. We give support to people who use the service i.e. to parents and carers. We have a good relationship with these people (the parents and carers) and also the professionals involved. We are given specific training to meet the needs of the people we support. We care for and about the people using the service. We promote where possible their independence and self esteem. The home is a very welcoming one. The home treats people as individuals. The home and its staff value each person. We have received one notification form during the past twelve months to report a notifiable incident involving a person who receives a service. Discussions with the manager, has identified that this was dealt with appropriately and professionally by the care team. At the last key inspection this service was rated as being a good service. There are no ongoing concerns about the quality of care that Eastfields offers, and a review of the documentation that has been received does not highlight any issues that need to be addressed. As a result of all the information received we have decided not to alter the inspection date for the service. What are we going to do as a result of this annual service review? The next inspection of this service will be based on the Fess and Frequency Regulations 2007 and the assessment of risk of the service.
