Set in its own grounds Etherley Lodge is a large converted two- storied former residential house without the benefit of a passenger lift. There are 2 double bedrooms; 31 single bedrooms of which 21 have en suite facilities. Throughout the building there are a number of different communal lounges, dining areas, a conservatory, communal toilets and bathrooms. The home has a large well-maintained garden of approximately 2.5 acres, along with a patio and other hard surfaced areas for residents use. Car parking spaces are located to the front of the home for use by relatives and visitors. Etherley Lodge is registered to provide residential care for up to 35 people with a mental disorder (excluding learning disability or dementia). What the care home does well Conclusion Staff and the manager have detailed knowledge of the health and personal care needs of the people who live at the home and how they are to be supported. And there are written instructions / care plans which give a detailed description at the start of each shift the supports / interventions which staff are to undertake. Staff at the home understand the importance of people having their healthcare needsmet. When people are visited by healthcare professionals they make sure that staff are available even if the visit was unexpected. Senior staff have a good understanding of the different types of medication that people have been prescribed and accurate records are kept which show that medication is well organised. There are sufficient, suitably qualified and experienced staff working at the home to meet the needs of people who currently live there. The manager and owner have taken steps to minimise the risks of harm from cigarette smoke to non smokers. They have done this by having areas where smoking is allowed and making sure the air is treated or extracted in these areas. All staff continue to encourage people to comply with the homes smoking policy by displaying signs all around the home and by giving advice and guidance when the smoking policy is not followed. What the care home could do better: There were no areas where the home did not meet the Care Homes Regulations 2001. People living at the home said that they were happy with the support they receive from staff, the manager and the owner. One Recommendation about improving the way that risk reduction measures are recorded has been made. Care Homes for Adults (18-65 years) Page 3 of 9 What we found: A concern was raised with the Care Quality Commission by a visitor to the home that staff did not know sufficient detail about the needs of people they were caring for. Inspection Activity During the visit three care staff and the manager were asked (separately) to describe the health and personal care needs of three people who were, or had been resident at the home. All of those asked were able to give detailed accounts of the people they cared for including those areas where staff have particular actions they take to support peoples physical or emotional wellbeing including important medical and personal history. Staff described how one person had been resident at the home for over 10 years and it was good to see that staff gave an excellent account of his health and personal care needs and how he is to be supported. Another person who was described by staff had recently been admitted to hospital and had continued to be visited by staff whilst remaining there so that hospital staff were kept aware of his individual needs. The staffs descriptions of the care practices they carried out matched the homes care planning records and staff daily records / daily support plans. These have sufficient detail in them so that everyone working at the home could understand what they need to do to be able to give effective support to people. Care staff also sign these to show that they have carried out the work that they are meant to do. Senior staff administer medication at the home. We sampled the records and asked senior staff about how and why they administer them. It was good to see that the senior on duty at the time of this inspection gave an accurate account of the medication taken (and its effects). Senior staff have a good level of knowledge about different types of medication that have been prescribed to treat people who live at this residential care home. Medication is kept locked away, is well organised and records sampled were accurate. The home is visited almost daily by various professionals who are responsible for the healthcare support that the people who live there need. This includes, district nurses, general practitioners and community psychiatric nurses. Healthcare practitioners usually make sure that staff at the home are aware of peoples appointments although when visits occur unexpectedly, staff will rearrange their work to make sure that they are available so that people have the support they need. Inspection Activity Staff rotas for the previous 2 weeks including weekends were examined. Staff were observed carrying out their daily / evening routines. The way that people are supported by staff was observed. Care Homes for Adults (18-65 years) Page 4 of 9 Outcome Records show that there are always at least three care staff including one senior on duty at the home at any time. The manager and deputy manager also work full time at the home. They and the owner also live next door to the home and can offer additional assistance if this be required. By observing how staff supported people we could see that there were enough staff available to meet their needs. People living at the home also commented that there were always staff around to help them. It was good to see that people were happy with the number of staff available to support them. Staff commented that they were supported by the manager and the owner if there are incidents or if people need to have more support than was expected. Inspection Activity The safeguards that have been put in place to protect non smokers who live, work or visit the home were examined during the inspection visit. Outcome The owner and manager have put in place a number of safeguards to minimise the risk of cigarette smoke. There are two smoking rooms at the home which have been identified as the only places where people living there are able to smoke indoors. These have been fitted with fans which treat and or extract the air. There are signs to prevent smoking in each corridor, room and thoughout the home. There are also many notices advising people where the permitted smoking areas are situated. In practice whilst staff and the manager constantly remind and encourage people only to smoke in permitted areas, occasionally some people who live at the home choose to smoke in other areas regardless of the house rules. Some people who live at the home also prefer to leave the smoking room doors open despite encouragement by staff and the manager not to do so. When the doors are left open or people smoke in areas which are not permitted, the smell of smoke can get into other areas of the home. Staff and the manager continue to be vigilant and remind people of the rules about where it is acceptable to smoke in the home.
