What the care home does well The acting manager and organisation have made good progress to improve the standard and quality of care provided at the home. They have worked cooperatively with both us and the local authority to make the improvements for the people living at the home. All of the requirements assessed at this inspection have been met. We will continue to monitor these key areas at the subsequent inspections. During the inspection we spent over an hour sitting in the lounge on the dementia care living unit. Whilst in this lounge we observed a relaxed and friendly atmosphere. There were positive interactions between staff and people living at the home. People were relaxed, calm and laughed with each other. Staff involved people who chose to in activities such as throwing a ball, singing, dancing or just talking. The staff observed interacted with people at a level that was appropriate to them. What the care home could do better: The record keeping has improved at the home and now reflects the personal care and support provided to people. However, the daily records do not reflect how people have been spending their time. Staff should record how people have been spending their time. This is so records give a more accurate reflection of people`s day to day lives and not just the care ad support provided to them. Care plans should be reviewed on a monthly basis and this information be recorded in the review record. This is to make sure that care plans and information for staff is up to date. Date of last inspection Brief description of the care home Fairlawn is a residential care home registered with the Commission for Social Care Inspection to accommodate a maximum of 60 older people including up to 20 with dementia. The premises are operated by Care South, a not for profit organisation. The home is purpose built and was opened on the 9th June 2003. Service users are accommodated on the ground, first and second floors. The second floor provides specialist care for up to 20 people with dementia. All bedrooms are for single occupancy and have en suite facilities. Communal areas are provided on all the floors Care Homes for Older People
Page 2 of 9 Brief description of the care home and include kitchen areas where snacks and drinks can be made. On the ground floor is a large area including a conservatory which is used for large group activities. There are three staircases and two passenger lifts. Outside the grounds are landscaped and one area provides a safe garden with varieties of sensory plants and other features. Fairlawn is located in the centre of Ferndown with shops and amenities available within a short walk. Care Homes for Older People Page 3 of 9 What we found:
Since the last inspection there has been three private admissions into the home. We looked at the assessments for three people including the latest admission into the home and two people that we looked at during the last inspection. There was a comprehensive pre-admission assessment completed for the last person to move in. The assessment covered all of the individuals areas of need and has been used to develop the care plan. The assessments and care plans for the two people already living at the home had been updated and reviewed to reflect their current needs and how staff are to meet them. On the day of the inspection there were 43 people living at the home, this is ten less than at the key inspection. We looked at three peoples care plans including two of the people we case tracked at the key inspection. The care plans seen now include the information from peoples assessments and included descriptions of what staff need to do to care for people. The manager told us that care plans are now being reviewed on a monthly basis. However, one persons plan had not been formally reviewed for a three months. There was evidence in the records that all elements of the individuals needs and care had been updated but the monthly review process had not been recorded. Up to date risk assessments were seen for all three people whose records were looked at. The areas risk assessed were falls, pressures area, nutrition and moving and handling. For one person who was nutritionally at risk and presents some challenges they had fluid, nutrition, bowel and behavioural monitoring charts completed. The acting manager has identified that there are some gaps in the record keeping but that these have been identified and followed up at monthly reviews. Health professionals are now recording in peoples care records. This means that it is easier to see whether appropriate health care and advice is being sought for people living at the home. We looked at the medication records and medication storage. Two bottles of eye drops were dated when they were opened. The new deputy manager completes a medication audit on a monthly basis. The acting manager has informed us when there has been any medication errors. Actions have been taken to minimise any further errors and advice sought from the pharmacist. We found that prescribed medications were being administered only to the people who they were prescribed to. The environment has improved and there are now different areas of interest and things for people to pick up and do. For one person with complex needs they have a much more stimulating environment in their bedroom. They have a tactile apron and now have music playing if they choose. Staff told us they are still finding out what type Care Homes for Older People Page 4 of 9 of music the individual prefers. The acting manager has been informing us by both telephone and regulation 37 notifications about any incidents at the home. We made an immediate requirement at the key inspection to increase the staffing levels to ensure that peoples needs were met. This was actioned at the time. As the numbers of people living at the home has reduced by ten people we will fully assess whether the current staffing levels are suitable at the key inspection. During the inspection their appeared to be sufficient staff on duty. The requirement relating to staffing records will be assessed at the key inspection.
