What the care home does well The home works well with other agencies and other professionals and seeks advice when they need to to provide required care and support to people who live there. What the care home could do better: Staff must ensure that there is a record of all complaints/ concerns that people raise. Records should also detail actions that have been undertaken as a result of these concerns. There is a need to ensure that the number of staff on duty at the home is sufficient to care, and support people living their and also keep them safe. Appropriate door locks should be fitted to bedroom doors that lock the door when it is closed but enable people to open it from inside by the door handle. That proposals are received from Southern Cross Healthcare Facilities Limited in respect of unitising the home within fours weeks of the date of registration. That the seven younger adults with mental health needs (MH) are found alternative placements within fours weeks of the date of registration. That the variation granted on the 13 September 2005 for one female service user who is 57 years and has dementia will only be for the lifetime of that service user and whilst the home is able to meet her needs. Page 2 of 8 able to meet their needs. That the maximum occupancy of the home is 59. Date of last inspection Brief description of the care home Fernwood Court is a purpose built, four storey building. Southern Cross Healthcare Services Ltd has privately owned the home since June 2005 since which time it has changed its name to Fernwood Court. The care home can accommodate up to 59 older people with dementia requiring personal care. There are communal rooms and assisted bathrooms on each floor. A passenger lift provides access to each floor. The home has a laundry and kitchen situated on the ground floor. The home has an enclosed garden at the rear of the building and a large car park at the side. All bedrooms have en-suite facilities. Concerns have been highlighted to the Commission for Social Care Inspection about injuries and care received by one identified person living at the home. We found that the home does sometimes struggle to meet this persons needs and also minimise the risk of aggressive incidents between people living at the home. We found that the identified people can be unsettled at night and wander constantly. The person whose care we were reviewing frequently does not settle until between 3 and 5 am and frequently wanders into other peoples rooms. We found that this can distress other people and that this can also result in aggressive incidents. One incident detailed when a persons face was scratched when they entered another persons room during the night. We saw records to demonstrate that a Safeguarding referral had been by the home following this incident and a review of this persons care will be undertaken. It was positive to see that medical advice had also been sought about this person and their health. We found that the bedroom doors in the home are the type that the doors have to be locked from outside or when in the room people need to use a thumb lock to keep the door locked. This type of door lock is not appropriate for people who have dementia who may not able to use the door lock and therefore stop people from coming into their room. A door lock which is locked when the door is shut but can be opened from inside by just using the door handle would provide greater assurance of stopping other people entering their room when not invited. We found that although the complainant had told the Commision for Social Care Inspection that they had already highlighted their concerns to the home there was no record of any complaints made. Staff agreed that the person had highlighted concerns but as they were not in a written format did not realise that they should be logged. We were found that other actions had been undertaken as a response to the concerns raised. We found that the number of night staff is not sufficient to meet peoples needs. There are five night staff covering four floors at night. This means that when staff breaks are covered there may not be a member of staff on each floor. This is a concern as some people require the support of two members of staff and means that all floors are not supervised. What the care home does well: What they could do better:
